- Description
- The Regional Health Contracting Office - Pacific at Tripler Army Medical Center (TAMC), Hawaii intends to award a firm-fixed price contract for the non-personal services of disease/case manager services at the 8th Medical Group (8 MDG), Kunsan Air Base, Republic of Korea.� The Disease/Case Manager uses tools of population health and health care integration to ensure health care interventions for both at-risk individuals and patient care populations, promotes optimum health and disease prevention, works to reduce healthcare costs, and improve patient quality of life. The Disease/Case Manager also assists the primary care teams and performs clinical nursing care in the execution of care coordination and disease prevention services; serves as a medical liaison/care coordinator within the military treatment facility (MTF) between specialty care (e.g., physical therapy, mental health) and primary care providers. Also serves as a medical liaison with network providers, TRICARE Operations and Patient Administration (TOPA), and the Managed Care Support Contractor to ensure appropriate follow-up care to include durable medical equipment (DME) issue. Required qualifications include, but are not limited to: Education:� DM/CM shall have a minimum of associate�s degree in nursing, obtained from an agency accredited by the US Department of Education. The DM/CM should have a working knowledge of performance standards and successful completion of associated healthcare coursework (i.e., experience in care coordination or health care integration or case management). Licensure/Certification/Registration:� DM/CM should be a licensed RN and shall maintain an active, valid, current, and unrestricted license (with no limitations, stipulations or pending adverse actions) to practice in any US state or jurisdiction. Experience:� DM/CM shall have a minimum of two (2) years nursing experience as a Registered Nurse (RN) within the last seven (7) years, with experience in utilization management, discharge planning, disease management or case management of adult patient populations or previous experience as a health care integrator. If DM/CM is not experienced in any medical management field, they shall complete a relevant Disease Manager or Health Care Integrator course within six months after contract award. Failure to do so will result in contract termination. DM/CM shall have knowledge of clinical application of principles and practices related to the nursing process or healthcare management. This requirement is for a one-year period of performance, 1 May 2023 through 30 April 2023, plus four one-year options, plus the FAR 52.217-8, Option to Extend Services.� The NAICS code for this proposed procurement is 621399 and the annual size standard is $9M.� This solicitation package will be available on the SAM.gov website (http://SAM.gov/) approximately in February 2023.� No paper copies of the solicitation will be issued and no solicitation mailing list will be compiled.� The Contracting Officer is Ms. Kanoe Sing-Ching, (808) 438-5135, kanoelani.c.singching.civ@mail.mil, and the Contract Specialist is Ms. Charis K. NOTICE TO OFFERORS All sources may submit a quote as applicable.� However, the Republic of Korea (ROK)-United States (U.S.) Status of Forces Agreement (SOFA), may preclude the U.S. Government from granting Invited Contractor or Technical Representative Status to any offeror, for this requirement.� All sources submitting a quote, including U.S. sources, must already have OR confirm their ability to obtain the requisite business licenses directly from the ROK Government, and conduct business in accordance with all applicable ROK laws, including but not limited to, rules and policies regarding labor, taxation, and immigration.� If an initial quote does not fully demonstrate this authorization and capacity, it will not be further considered for award. Whether an offeror demonstrates this authorization and capacity shall be determined by the Contracting Officer.
