Description

This is a Request for Quotation prepared in accordance with FAR Part 13, as supplemented with additional information included with this notice. �A quote is being requested and a written solicitation will not be issued.� Requirement number OC-393-2023-0056 is issued as a Request for Quotation. �The Centers for Medicare and Medicaid Services (CMS) intends to make a purchase order award to a contractor using Simplified Acquisition Procedures with a Firm Fixed Price to provide subscription service for 55 licensed users. Detail information is provided in the attached Statement of Work and Deliverables.� The period of performance will be for twelve months. Your response shall be no more than 5 pages and shall include the following: 1) Contractor�s plan to perform the Statement of Work; 2) and fully loaded labor rates. All responses will be evaluated using the following Evaluation Criteria: 1) Technical Understanding 2) Personnel Qualifications, 3) Management Approach and 4) Price (Base Year).�� An award will be made to the responsible offeror whose quotation is the lowest price and is technically acceptable. Your response may be sent electronically in MS Word format or it can be submitted via mail in hard copy. Your quote should include the following additional information: 1.� Company Name, Address 2.� Contact Person, name, Telephone Number, E-Mail Address 3.� Business Size Classification, i.e. Small Disadvantaged, 8(a), Woman Owned 4.� UEI Number 5.� Tax ID NO. Note: You must be registered and the registration current in the SAM or System for Award Management in order to receive a federal government award. Your quote must be received by February 24, 2023 no later than 12:00pm. Questions should be referred in writing no later than February 22, 2023 to : Phillip.Harrell@cms.hhs.gov.
