- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Products and Commercial Services, in conjunction with FAR Part 13 Simplified Acquisition Procedures, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a separate written solicitation will not be issued. The solicitation number is HT941023Q2001. It is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2023-01 effective 12/30/2022. The North American Industry Classification System (NAICS) Code for this acquisition is 513210. The size standard is $47,000,000. This is an unrestricted procurement, inviting full and open competition. It is the responsibility of the contractor to be familiar with the applicable clauses and provisions.This requirement is for supplies and non-personal services.Defense Health Agency (DHA) Western Markets Contracting Division requests responses from qualified sources capable of providing the following brand name requirement:All equipment, materials, supervision, and any other items and non-personal services necessary to provide and maintain the brand name Unique Device Identification (UDI) Tracker software and data within a secure supported operating and technology environment and to provide 24/7 support to track tissues and implants in accordance with the performance work statement (PWS) in support of the 374th Medical Group (MDG) at Yokota Air Base, Japan.See attached Combined Synopsis/Solicitation, Performance Work Statement (PWS), Past Performance Questionnaire (PPQ), and Quote Sheet.
