Description
Source Sought for the Baltimore VA Medical Center for Alcon Phacoemulsification Units Service Coverage. A base plus four option years contract. See SOW for a full description of services. RFI closed 2/17/2023 at 8am EST. This is a request for information only. The VA intends to sole source these services to Alcon Vision LLC. All information received in response to this RFI that is marked as proprietary will be handled accordingly. In accordance with FAR 15.201(e), responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this RFI. Statement of Work GENERAL INFORMATION Title of Project: Alcon Phacoemulsification Units Service Coverage Scope of Work: Alcon will provide the following services: Alcon will contact the Baltimore VA Medical Center s Biomedical Engineering department to schedule preventative maintenance visits on a mutually convenient weekday between the hours of 8:00 a.m. and 5:00 p.m. The preventative maintenance visit will include a system verification of the equipment by a trained Alcon representative. If the Alcon representative determines that alignment(s), adjustment(s), and/or replacement of part(s) are necessary they will perform these services during the visit at no additional cost to the VA. Parts and Labor. Alcon will provide parts and labor (during the hours specified above) necessary to service and maintain the Equipment according to Alcon's published operating specifications at no additional charge. Unscheduled Service. During the term, Alcon will provide unscheduled service visits, at no additional cost, between the hours of 8:00 a.m. and 5:00 p.m., Monday through Friday. In most instances, Alcon will provide an on-site technician within 48 hours. Modifications for Safety or Reliability. If Alcon determines that a modification to the Equipment (whether hardware or software) is necessary to address safety or reliability concerns identified by Alcon, then Alcon will install the modification in the Equipment as soon as practical at no additional cost to the VA. Emergency Telephone Consultation. Alcon will provide emergency telephone consultation 24 hours a day, 7 days per week at no additional charge. Alcon will coordinate all service and maintenance on the equipment with Baltimore VA Medical Center s Biomedical Engineering department Alcon representatives will report to Baltimore VA Medical Center s Biomedical Engineering department in room 5B-183 before performing any service or maintenance on the equipment Alcon will provide the Baltimore VA Medical Center s Biomedical Engineering department with a copy of a detailed service report within 2 weeks of completed service or maintenance. Below is the list of equipment identified to be covered by this service contract: Model Serial Number PM s Centurion 1501751201 1 Constellation LXT 1203029401 2 Background: The Baltimore VA Medical Center requires full-service coverage of 2 phacoemulsification units including scheduled preventive maintenance in accordance with the manufacturer s recommendations to ensure that the equipment is properly maintained and to reduce downtime of the identified equipment that results in cancellations of ophthalmic surgical cases. Performance Period: 1 base year plus 4 option years Type of Contract: Maintenance services Place of Performance: Baltimore VA Medical Center 10 N. Greene St. Baltimore, MD 21201 Security: The contractor must have photo ID available when performing work at a VA Medical Center and be willing to present it to VA staff. The vendor is responsible for personal protective equipment. The C&A requirements do not apply, and a Security Accreditation Package is not required.
