- THIS REQUEST FOR INFORMATION (RFI) 36C26223Q0654 IS ISSUED SOLELY FOR INFORMATION AND PLANNING PURPOSES ONLY AND DOES NOT CONSTITUTE A SOLICITATION. THE SUBMISSION OF PRICING, CAPABILITIES FOR PLANNING PURPOSES, AND OTHER MARKET INFORMATION IS HIGHLY ENCOURAGED AND ALLOWED UNDER THIS RFI IN ACCORDANCE WITH (IAW) FAR 15.201(e). SOURCES SOUGHT DESCRIPTION This is NOT a solicitation announcement. This is a source sought/RFI for market research only. The purpose of this source sought/RFI is to gain knowledge of potential qualified sources and their size classification relative to NAICS 561621 (size standard is $25.0 Million). Responses to this source sought will be used by the Government to make appropriate acquisition decisions. After review of the responses to this source sought, a solicitation announcement may be published. Responses to this source sought synopsis are not considered adequate responses for a solicitation announcement. The Department of Veterans Affairs (VA), VISN 22 Network Contracting Office, is seeking sources that can provide Identiv Memory Board MEB/CB128 or Equal To and Installation that at a minimum meets the following salient characteristics in the Statement of Work for the VA SAN DIEGO HEALTHCARE SYSTEM: Line Item Description/Part Number Quantity Unit Unit Price Amount 0001 Identiv Memory Board MEB/CB128 or Equal To 1 EA 0002 Installation 1 JB TOTAL Include shipping cost on the items price. Vendor Requirements: Period of Performance for installation if 30 days after receipt of order (ARO). Vendor shall quote all items under this requirement. Vendor shall meet the Salient Characteristics in the Statement of Work (SOW). Vendor shall respond questions 1 through 8 of the Request of Information (RFI), see below. STATEMENT OF WORK FOR VA SAN DIEGO HEALTHCARE SYSTEM (VASDHS) 1. Introduction The VA San Diego Healthcare System (VASDHS) is one of the most diversified tertiary health care providers in the VA. It is a teaching hospital, providing a full range of patient services, with state-of-the-art technology as well as education and research. 2. Background Purchase and installation of 31 Identiv memory modules or Equal to that are compatible with existing Identiv Hirsch Controllers in asbestos and nonasbestos areas. All personnel must have the following certifications and comply with VA safety standards: Hirsch Identiv Silver partner Asbestos O&M certified 30 hour OSHA safety Supervisor 30 hour OSHA safety Worker All installations will be in accordance with manufacturer specifications, applicable VA Technical Information Library (TIL) guidelines, and any system configuration necessary for operation. All additional equipment, necessary labor, supervision, and material required to complete the project will be the responsibility of the contractor to procure. Once installation and configuration are complete two one-hour training sessions will be conducted on the HDVA operation. All existing equipment that has been identified for removal will be returned to the VA San Diego Police Service. Period of Performance 30 Days After Receipt of Order (ARO) 4. Specific Tasks Contractor shall conduct site preparation to include setting up barriers for any breach of walls and ceiling barriers at any location where dust generation activities may occur due to the installation of security cameras. Contractor shall install all devices, cables, and system accessories in accordance with the manufacturer s specifications. Contractor shall perform any configurations to the system, if necessary, to ensure the camera functions as intended. Contractor shall implement and maintain infection control measures for the duration of the project including site construction barriers, and negative pressure measures. Construction barriers shall be in accordance with VASDHS Infection Control Risk Assessment. Contractor is responsible for providing all tools and equipment necessary for installation. Upon completion of work, the Contractor shall leave the worksite in a clean and orderly condition. Contractor shall ensure all newly installed equipment, software, and hardware are up to date and operation as per manufacturer specifications and operating properly. Contractor shall provide equipment manufacturer documentation providing material specifications. Material and equipment must be compatible with the hospital s existing and planned environment. Contractor shall perform work during normal working hours whenever possible and shall coordinate work with the designated COR and the service Nurse Supervisor on duty to minimize the impact on routine operations. When routine operation does not permit installation of equipment, the contractor shall accommodate the unit operating hours and perform construction work after-hours. Contractor shall provide and install CAT 6A cable for all new devices. The contractor shall follow the Technical Information Library for all work performed. 5. Vendor Responsibilities Shop drawing of the new system (cable run and equipment location within the designated areas) prior to the start of installation. The contractor will have a meeting with the COR to review the shop drawings and system layout prior to the start of work. The contractor must provide a project schedule within 7 calendar days of NTP. The contractor must provide a work plan prior to the installation of the new security cameras. The plan should identify infection prevention control measures and the proposed sequence of work. The contractor will provide As Build drawings of the installation in full-size drawings and electronic format in AUTOCAD and PDF format at the end of the project. Training for users/ O&M manual one hardcopy and two CDs. 6. Salient Characteristics Compatibility with Hirsch Mx and M series controllers currently in use at VA San Diego Expands to the memory capacity of Hirsch Mx and M series controllers currently in use at VA San Diego to support 128,000 user records or more per memory board Allows for additional users and/or events and alarm codes to be stored on Hirsch Controllers Expands the alarm and events buffer, or provides a combination of both records and buffers 7. Warranty Vendor shall provide properly functioning equipment and products within useful life cycle, and immediately replace any equipment utilized by the VASDHS that ceases to function due to manufacturer defect, or all other discrepancies not caused by VASDHS staff at no additional cost. 8. Safety Contractors shall comply with all applicable federal, state, and local requirements regarding workers health and safety. The requirements include, but are not limited to, those found in federal and state Occupational Safety and Health Act (OSHA) statutes and regulations. In the performance of this contract, the contractor shall take such safety precautions as necessary while operating in the construction area. Construction shall provide all necessary tools, equipment, labels, and Personal Protective Equipment to perform the work safely, effectively, and timely. Work may be conducted in areas identified as containing asbestos materials. The contractor shall therefore comply with the public laws and statutes including all changes and amendments to federal, state, and local environmental statutes and regulations governing asbestos-containing materials (ACM). Contractor shall be licensed, qualified, and certified to meet all EPA regulations under National Emission Standards for Hazardous Air pollutants-NESHAPS and all applicable federal, state, and local regulations. All contractor personnel on the premises involved installation must have the following certifications and comply with VA safety standards: Hirsch Identiv Silver Partner Asbestos O&M certified 30 hour OSHA safety Supervisor 30 hour OSHA safety Worker VASDHS La Jolla facility is Federal property. The contractor shall coordinate with the facility Engineering Service if warranted for access in restricted spaces of any required support in the performance of this work. The designated COR shall coordinate the request. Inspection and Acceptance Criteria The VA POC or COR may conduct an inspection of the work performed by the Contractor without the presence of the Contractor and determine whether it is within the standard expressed in this scope of work. It is the VA s prerogative to hire an independent qualified third party to check the validity of the work performed. The information identified above is intended to be descriptive, not restrictive and to indicate the quality of the supplies/services that will be satisfactory. It is the responsibility of the interested source to demonstrate to the government that the interested parties can provide the supplies/services that fulfill the required specifications.
