- Effective Date: 03/15/2023 Revision: 01 Description This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document will not be issued. This solicitation is issued as an RFQ. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2023-01 Effective Date: 12/30/2022. This solicitation is set-aside for: SDVOSB The associated North American Industrial Classification System (NAICS) code for this procurement is 339113, with a small business size standard of 750 employees. The FSC/PSC is 6530. Requirement: The Northport VA Medical Center is seeking to purchase Haag-Streit Reliance 940 Chair Glides with Tapered Tracks. Specifications: All interested companies shall provide quotations the following brand name requirement: Line Item Description Quantity Unit of Measure CS1RL940 Reliance 940 Chair Glide System 14 EA Salient Characteristics: Plate and track of stainless steel which guides the carriage and prevents lateral movement. Heavy duty stops on the base plate limit the distance of travel. Heavy duty cast aluminum carriage will accommodate the current Reliance chairs in the Ophthalmology clinic. Twenty-seven (27) inches of travel excursion. Track tapers to a 6 tail, which allows the practitioner to move an examination stool closer to the chair to provide greater accessibility to the patient. Foot lever activates and releases a friction lock that secures the chair in position anywhere along its 27 path. This device will make all examination spaces in which it is used ADA compliant, may be applied toward the tax credit provided by this legislation. Dimensions of the glide will be 52 in overall length, with a width of 20 . The overall carriage dimensions will be 29 x 26 , which can be accommodated in all current exam spaces for this to be installed. Delivery shall be provided no later than 45 days after receipt of award (ARA). Comparative evaluation is the side by side pairwise comparison of quotes based on factors resulting in a Contracting Officer decision for the quote most favorable to the Government. The following are the decision factors: Award shall be made to the quoter, whose quotation, offers the best value to the government, considering technical capability, past performance, and price. The government will evaluate information based on the following evaluation criteria: (1) technical capability factor meeting or exceeding the requirement, (2) past performance, and (3) price. The award will be made to the response most advantageous to the Government. Responses should contain your best terms, conditions. To facilitate the award process, all quotes must include a statement regarding the terms and conditions herein as follows: ""The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition.""
