- Offerors are urged to ensure that their proposals are submitted on the most favorable terms in order to reflect their best possible effort, since less than that could result in exclusion of the proposal from further award consideration. The Government reserves the right to make award without discussion based upon the initial proposals. All information that the offeror wishes to have considered shall be submitted with the initial proposal. Vague or terse statements such as will comply, noted, understood, etc. are not acceptable. The evaluation will be limited to the information provided and nothing will be assumed. The evaluators may utilize the Offeror s response to the solicitation as a means for validating the Offeror s understanding and capability to perform as it relates to the evaluation criteria. The Offeror s proposal is presumed to represent its best efforts to respond to the solicitation; thus, any unexplained inconsistencies resulting from the Offeror s lack of understanding of the nature and scope of work for the overall solicitation may be grounds for rejection of the proposal. LATE OFFERS WILL NOT BE ACCEPTED. OFFERORS MUST ENSURE THAT PROPOSALS ARE SUBMITTED TIMELY TO ENSURE DELIVERY AND RECEIPT BY THE DUE DATE AND TIME IDENTIFIED IN BOX 8 ON THE SF1449. Proposals may be submitted electronically, via email, to Mohammed.hussain2@va.gov; or, via E-mail to Mohammed.Hussain2@va.gov: TECHNICAL, PAST PERFORMANCE, SDVOSB/VOSB STATUS AND PRICE INFORMATION MUST BE SUBMITTED AND IDENTIFIED AS FOLLOWS: VOLUME I TECHNICAL PROPOSAL VOLUME II PAST PERFORMANCE PROPOSAL VOLUME III SDVOSB/VOSB STATUS VOLUME IV PRICE PROPOSAL (Attachment 2) The Contractor shall include the below information when providing a proposal: Volume I Signed and dated SF 1449 All amendments to the solicitation signed and dated if applicable Name, Address, Phone Number, and email address of Solicitation point of contact Offeror Representations and certifications per FAR provision 52.212-3 Proof of SAM registration. (Contracting officers must be able to verify offerors and bidders comply with FAR 52.204-7(b)(1) requirements to be registered in SAM at the time an offer or quotation is submitted, consistent with the requirements of FAR provision 52.204-8.) A statement of technical capabilities that show the contractor s capabilities to perform contract in accordance with statement of work. At a minimum this should include warehouse quantities and locations, just in time delivery capabilities, delivery tracking systems, and quality assurance processes. Subcontracting plan for large businesses Volume II Past performance questionnaire of at least two recent and relevant references for contracts of similar size and scope. (Within the last 2 years.) (Attachment 5) References must contain email address and phone number of point of contact. Offerors must ensure that its references submit completed Questionnaires to the Contract Specialist by the closing date and time of the solicitation. The Questionnaires can be emailed to mohammed.hussain2@va.gov Relevancy Offerors must submit information detailing their work over the past five years that is relevant to the efforts required by this solicitation. The information must include: Contracting Entity s name and location (include the contract number if it s a governmental entity); Scope of services provided; Contract time period (i.e., length of time); Procuring Officer/Official s name, telephone number, and email address; and Contract value. Volume III SDVOSB/VOSB Status and a written declaration that the contractor can perform over 50% of the work if a small business concern or SDVOSB/VOSB. To be considered a Veteran Owned Business the Service-Disable Veteran-Owned Small Business (SDVOSB) or Veteran-Owned Small business (VOSB) the offeror must be registered and verified in the SBA database (Veteran Small Business Certification (sba.gov)) at the time of offer submission. For an Offeror, other than a SDVOSB or VOSB, who proposes to use SDVOSB or VOSB subcontractor, the proposed SDVOSB or VOSB must be registered and verified in the SBA database at the time of offer submission. An Offeror must state in their proposal the names of the SDVOSB and VOSB with whom they intend to subcontract and provide a brief description of the proposed subcontracts and the approximate dollar values of the proposed subcontracts. Volume IV Price/Cost Schedule (attachment 2). Failure to provide information above may result in a rejection of offer.
