Page 2 of 2 SOURCES SOUGHT NOTICE For HCHV (Health Care for Homeless Veteran) Medical Respite Care for the Richard L. Roudebush, VAMC in Indianapolis, IN. This is a Sources Sought Notice. This advertisement is not a solicitation for offers, nor is it a request for quotes. A request for quotes may be issued at a later date. This Sources Sought/Market Survey Notice is being conducted by the Network 10 Contracting Office, PO Box 492, Ann Arbor, MI 48106 to determine if responsible sources exist and if there are any businesses capable of providing the requirements listed herein. The proposed NAICS Code is 624221, Temporary Shelters ($13.5 Million). The Richard L. Roudebush, VA Medical Center (RLRVAMC), in Indianapolis, Indiana has a need for contractors to provide services as part of its community-based Health Care for Homeless Veterans (HCHV) program. The HCHV program s Contract Residential Services (CRS) programs target and prioritize Veterans experiencing homeless who are transitioning from literal street homelessness, being discharged from institutions, including those in need of medical respite, and who recently became homeless and require safe and stable living arrangements while they seek permanent housing. The VA contemplates making a single award. The government is interested in sources that can provide up to 2 Veteran beds. Interested parties are encouraged to respond with what they can provide (i.e. if you can only support a certain number of beds, please state what you can provide). Period of Performance: 12 months from date of Award plus five, 12-month option periods. Responses should include: (1) Business Name, GSA contract number (if applicable) and Address; (2) Point of Contact Name, Phone Number, and E-mail Address; (3) SAM UEID Number and NAICS code; (4) Business Size: are you SMALL or LARGE; and (5) Type of Business SERVICE-DISABLED VETERAN OWNED, VETERAN OWNED, 8A, HUB Zone, WOMEN-OWNED etc. All businesses eligible to provide this service are encouraged to reply as this information may be used to determine potential set asides for the above noted service. Prospective offerors shall respond to this Sources Sought no later than 12:00pm (Eastern), Wednesday, March 22, 2023. Send all replies to Cierra S.
