- Description
- The Medical Readiness Contracting Office-Europe issued a Request for Quote (RFQ) for non-personal services to provide blood donor testing for the Armed Services Blood Bank Center - Europe (ASBBC-E).� The resultant contract type will be a Firm-Fixed-Price (FFP) Purchase Order.� It is incumbent upon interested parties to review this site frequently for any updates/amendments to any and all documents.� The solicitation will be issued for full and open competition.� QUOTE SUBMISSION 1. INSTRUCTION a. ELECTRONIC QUOTE.� Quoters may submit electronic quote in response to this Request for Quote.� The quote must be received by the quote due date/local time as specified in block 8 of the SF 1449 cover page to the inbox of the following email address: henningfalk.mayer-goldstein.ln@health.mil.��Vendors may call Mr. Henning Mayer-Goldstein at +49 (6371) 9464 6789 to verify receipt of the quote.� If the vendor chooses to email the quote, the Government will not be responsible for any failure of transmission or receipt of the quote, or any failure of the vendor to verify receipt of the emailed quote. b. QUESTIONS.� Prospective quoters are strongly encouraged to submit any questions regarding this Request for Quote no later than five (5) business days prior to the request for quote due date specified in block 8 of the SF 1449 cover page.� Questions can be emailed to henningfalk.mayer-goldstein.ln@health.mil.� All questions received will be answered officially through an amendment to the Request for Quote for distribution to all prospective quoters.� Questions received after the CET cut-off date/time will not be accepted by the Government. c. ACQUISITION PROCEDURE.� This is a commercial item under FAR Part 12 and the acquisition procedures to be used for this purchase will be in accordance with FAR Part 13.5, Simplified Procedures for Certain Commercial Products and Commercial Services. d. SPECIAL NOTE.� All vendors must be registered in the System for Award Management (SAM) at the time an offer or quotation is submitted, and lack of registration shall make a quote ineligible for award.� Vendors may obtain information on registration and annual confirmation requirements by calling 1-866-606-8220 or via Internet at https://www.sam.gov. FACTOR 1 � TECHNICAL: The quoter shall demonstrate compliance with the requirements in the solicitation by submitting a written quote which addresses compliance with each of the technical subfactors specified in 52.212-2 Evaluation � Commercial Items. FACTOR 2 � PAST PERFORMANCE: (i) List at least three (3) references, but not more than five (5), to include Point of Contact, Email address, Telephone Number, Nature of Work, Annual Dollar Value, Contract Number, Location and Time Period involved.� Past performance information should be for work performed within the past three (3) years.� Any/all references received beyond the maximum number will not be evaluated. Quoters submissions proposing the use of business partnerships or subcontractors shall also include at least three (3) references for the partnership or subcontractor. FACTOR 3 � PRICE: The quoter shall ensure to provide the quoted prices on the SF 1449 and be in compliance with the following: (1) The quoter shall provide a unit price and extended amount (i.e., quantity multiplied by unit price) shall be submitted for each Contract Line Item Number (CLIN).� In case of in case of a discrepancy, the unit price on the Standard Form (SF) 1449 will take precedence. (2) The quoter shall limit their prices to two decimal places (example: $3.45). CONTRACT INFORMATION The Contractor shall provide all required facilities, personnel, services, materials, equipment, and supplies required to meet the requirements outlined in this Performance Work Statement.� All testing shall be in accordance with the Food and Drug Administration (FDA) regulations for the manufacturing of transfusable blood products and for blood bank reference lab testing.� The standard test shall be a platform approved by the FDA (shall be performed in accordance with Code of Federal Regulation, Title 21 (CFR) and applicable industrial standards).� Donor blood testing services will support the Armed Services Blood Bank Center - Europe (ASBBC-E) located at Landstuhl Regional Medical Center, Landstuhl (LRMC), Germany. Blood Donor Testing - Reportable Tests Non-Personal Services.� Provide FDA licensed Blood Testing for Infectious Disease for the Standard Test Panel and Confirmatory Testing in accordance with the Performance Work Statement (PWS), Paragraph 5.� The Government will only pay for Reportable Tests (Patient Results).
