- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement is a combined synopsis/solicitation. Therefore it constitutes the only solicitation. Quotes are being requested and a written solicitation will not be issued. The Navajo Area Indian Health Services, Kayenta Service Unit, Incident Command Systems is requesting for a Contractor to provide the following RESPIRATOR UNITS & ACCESSORIES. Delivery Date By: May 31, 2023. The Contractor shall provide the following items: Item# CA-DLC-CAPR-36, CAPR SYSTEM, MEDIUM BATTERY, Qty of 10 Item# 2061-05, HFR Filter Cover Cap (box of 3), Qty of 4 Item# 2782-06, Cart, includes 6 battery charging stations, Qty of 1 Item# 2000-SB, System Bag, Qty of 10 Item# 2000-201, Comfort Strips (box of 36), Qty of 5 Item# 2180-05, HE Filter (CAP) for use with CAPR and 2051-07 Cage (Box of 6) Item# 2365-02ML, DLC Lens-Cuff, MED/LG (box of 40), Qty of 10 Item# 2365-02SM, DLC Lens-Cuff, SM/MED (box of 40),Qty of 10 Estimated Shipping Fee This combined synopsis/solicitation will be in accordance to request for quotation (RFQ) utilizing FAR Part 12 Commercial Items and FAR Part 13 Simplified Acquisition Procedures. This solicitation is issued as full and open competition. SHIPPING ADDRESS: KAYENTA SERVICE UNIT Highway 160, SOUTH OF MILE POST 394.3 KAYENTA, AZ 86033 EVALUATION CRITERIA: This procurement is being conducted under FAR Part 13 Simplified Acquisition Procedures. The Government reserves the right to issue a firm-fixed price single award or multiple awards to the contractor(s) whose offer represents the Lowest Price Technically Acceptable (LPTA); award will be made on the basis of the lowest evaluated price of proposals meeting or exceeding the acceptability standards for non-cost factors.
