- ATTACHMENT 1 Special Notice: Sources Sought The Department of Veterans Affairs, VISN 08, Bay Pines Veterans Affairs Healthcare System (BPVAHCS) located at 10000 Bay Pines, Blvd. Bay Pines, FL 33744 is looking for certified, Service-Disabled Veteran Owned Small Business (SDVOSB) and Veteran Owned Small Business (VOSB), Small business, and/or other than small businesses sources for the following: 30-90 Day, with the option to extend based on clinical need, Community-Based Emergency Transitional Residential Environment for Veterans who are Homeless with a need for Enhanced Medical Services. The authority for this procurement is Federal Acquisition Regulation (FAR) 13 Simplified Acquisition Procedures and FAR Part 12, Acquisition of Commercial Items. Place of Performance: At the Contractor s facility in Pinellas County. Period of Performance: The anticipated period of performance for this contract will be from 09/01/2023 through 08/31/2028 for the base year and four (4) one (1) year option periods. Description of Services: Five (5) Contracted Emergency Residential Services (CERS) beds that prioritizes homeless Veterans who are eligible for VA health care, and are transitioning from literal street homelessness, Veterans being discharged from institutions (including those in need of medical respite), and Veterans who recently became homeless and require safe and stable living environments while they seek permanent housing. In addition, there is a focus on those Veterans requiring Enhanced Medical Services where typical emergency housing programs may not be suitable. Contractor shall provide emergency housing and supportive services for homeless Veterans who are also recovering from a medical illness or injury for the BPVAHCS in accordance with the Health Care for Homeless Vets (HCHV) CERS model requirements. The contractor will have five beds dedicated to the provision of this service. Services are expected to consist of supportive, secure housing for homeless Veterans, many of whom may also be dealing with medical, mental health disorders, and/or substance use disorders. The program places an expectation on the Veteran to engage in supportive case management services, and on the Contractor to provide expertise and guidance for improving housing stability to ensure a transition from homelessness to permanent, stable housing. Medically enhanced programs will provide medical oversight and must comply with all HCHV CERS requirements as identified below. The type of Veterans to be cared for under this contract will require care and services above the level of meals, room, and board. Relevant care will include, not be limited to medical oversight and management, consultation with VA primary care physician when needed, health education, case management development of an independent housing plan, assistance with housing related resources, active affiliation with community support groups, and family involvement. Beneficiaries receiving care under this contract will receive basic health and personal hygiene maintenance, storing of medications and other activities that support the overall recovery of the Veteran. The rate established will be an all-inclusive rate. This is a request for sources and information only and it is intended for market research and planning purposes, which may or may not lead to a future solicitation. This is not a request for quotes. The VA will not pay for any information received resulting from this sources sought notice. Requests for copies of a solicitation shall not be honored or acknowledged. SDVOSB or VOSB must be certified in the Veterans Small Business Certification website at: Veteran Small Business Certification (sba.gov) Services and documentation of care provided under the resultant contract shall be subject to quality management and safety standards as established by VA, consistent with the standards published by the Joint Commission (TJC) or equivalent. The contractor shall develop and maintain Quality Improvement/ Quality Assurance Programs and provision of care equal to or exceeding VA Standards for employees. The results of all Quality Improvement activities performed by the contractor involving VA patients will be shared with VA Quality Management Office. Responsibilities of the contractor: It is the contractor s responsibility to provide 30-90 Day Community-Based Emergency Transitional Residential Environment, with the option to extend based on clinical need, for Veterans who are Homeless with a need for Enhanced Medical Services in the service location of Pinellas county. Conflicts of Interest: Contractor has reviewed applicable Federal, State, and local conflict of interest laws to assure that conflict of interest situations does not inadvertently arise. Confidentiality: Contractor agrees to abide by all applicable Federal laws and regulations pertaining to the privacy of patients and or VA employees, including all Health Insurance Portability and Accountability Act (HIPAA) of 1996/ Information Security regulations as they are implemented within the VA. The contractor also agrees to abide by the confidentiality requirements of the protocols. Only responses from authorized representatives of the firm will be considered. Adequate knowledge, skills, and abilities providing 30-Day Community-Based Emergency Transitional Residential Environment for Veterans who are Homeless will be considered.
