SOURCES SOUGHT NOTICE This is a Sources Sought NOTICE (SSN) ONLY. The U.S. Government is conducting market research only to determine the availability of qualified sources capable of providing Courier Services to include transportation of clinical laboratory specimens which may consist of human tissue, blood, urine, and other bodily fluids. Potential Contractors are invited to provide a response via e-mail to Contract Specialist at Robert.weeks@va.gov by April 13, 2023 11:00 am (PT). Responses will be used to determine the appropriate strategy for a potential acquisition. Please clearly identify any information your company considers sensitive or proprietary. This notice is issued solely for information and planning purposes - it does not constitute a Request for Quotation (RFQ), or a promise to issue an RFQ in the future. This notice does not commit the U.S. Government to contract for any supply or service. Further, the U.S. Government is not seeking quotes, or proposals at this time and will not accept unsolicited quotes in response to this sources sought synopsis. The U.S. Government will not pay for any information or administrative costs incurred in response to this notice. Submittals will not be returned to the responder. Not responding to this notice does not preclude participation in any future RFQ, if any is issued. BACKGROUND: The Alaska VA Alaska HealthCare System, has a requirement for a Contractor to provide Courier Services for Laboratory specimens. Please draft Statement of Work below for requirements. Please state if your FSS Schedule is under Group 66 III Cat 605-2A________ 2) Please submit a brief capability statement (maximum two pages) with enough information to demonstrate to the Veterans Affairs that you have the capability to perform courier services for AVAHS and locations within the ""Anchorage Bowl"" (40-mile radius of the AVAHS Clinic). **Draft Statement of Work 1.0. PURPOSE: To provide courier services for the Department of Veterans Affairs Alaska VA Healthcare System (AVAHS) sites within the Municipality of Anchorage and the Matanuska-Susitna Borough. Courier service includes transportation of clinical laboratory specimens which may consist of human tissue, blood, urine, and other bodily fluids. Occasionally administrative paperwork, imaging CDs, and other small items will be included for pickup and delivery. 2.0. LOCATIONS OF SERVICES: Contractor to provide clinical laboratory specimen courier services to the Department of Veterans Affairs Alaska VA Health re System (AVAHS), 1201 N. Muldoon Road, Anchorage, AK 99504. The Contractor shall provide services in accordance with the requirements of this contract. Courier services are required between the AVAHS and locations within the ""Anchorage Bowl"" (40-mile radius of the AVAHS Clinic). 3.0. SERVICES: 3.1. Domiciliary: Laboratory: Contractor shall pick up clinical laboratory specimens from the Domiciliary (DOM) at 3001 C Street, Anchorage, AK 99509 and transport them to the AVAHS Laboratory. Service is required once daily unless notified otherwise, Monday through Friday, excluding weekends and Federal holidays. Contractor shall pick up the specimen no later than 12:00pm and deliver to the AVAHS Laboratory no later than 1:30pm. 3.2. Pharmacy: Contractor shall pick up from AVAHS Pharmacy twice (2) daily at 10am and 2pm and deliver to the Domiciliary (DOM) at 3001 C Street, Anchorage, AK 99509. Monday through Friday, excluding weekends and Federal holidays. 3.3. Matsu CBOC: Contractor shall pick up clinical laboratory specimens from the Mat-Su CBOC at 865 N. Seward Meridian Parkway, Suite 103, Wasilla, AK 99654 and transport them to the AVAHS Laboratory. Service is required once daily, Monday through Friday, not including Federal holidays. All Monday through Friday pickups shall be made no earlier than Noon and no later than 13:30 with delivery to the AVAHS Laboratory by 15:00. All specimens must be delivered to the AVAHS Laboratory on the day they are picked up 3.4. Juneau CBOC: pick up clinical laboratory specimens from the Juneau CBOC at 709 W 9th St, Suite 150 Juneau 99802 and transport them to a third party at Juneau International Airport that provides the fastest mode of delivery to Ted Stevens Airport. Service is required twice weekly, Monday through Thursday, not including Federal holidays. All Monday through Thursday pickups shall be made no earlier than 13:30 and no later than 15:00 with delivery to the Juneau International Airport that provides same day transport to Ted Stevens International Airport. Vendor is responsible for pick-up from Ted Stevens International Airport and delivery to the Anchorage VA Medical Center. Vendor will be responsible for establishing the account(s) and payment of services provided by the vendor/carrier chosen. 3.5. Other sites within the ""Anchorage Bowl"" (40-mile radius of the AVAHS Clinic): Contractor shall pick up administrative paperwork, imaging CDs, and other small items by request of AVAHS staff. 3.6. Security: Contractor shall guarantee the security of the transport container and contents between pickup and delivery. Specimens are irretrievable and must be secure at all times. 4.0. VA FURNISHED PROPERTY OR SERVICES: The VA shall furnish containers for transporting clinical laboratory specimens. VA shall prepackage specimens according to transportation safety rules and place in provided container for transport by the contractor. 5.0. CONTRACTOR FURNISHED PROPERTY OR SERVICES: The Contractor shall provide all staff, vehicles, and other items not specifically stated as VA-furnished, necessary for performing services as defined in this statement of work. 6.0. ADMINISTRATION & MANAGEMENT: The Contractor shall be responsible for the administration and management of all aspects of this contract. This includes responsibility for all Contractor employees, subcontractors, agents, and anyone acting for or on behalf of the Contractor. 7.0. CONTRACT STARTUP REQUIREMENTS: The contract start-up requirements must be completed prior to the commencement of work. The Contractor shall comply with the following contract requirements within fourteen (14) calendar days of the Date of Award: 7.1. The Contractor will hire and train personnel 7.2. The Contractor will have completed any sub-contracting necessary to fulfill their obligations under the contract. 7.3. The Contractor, including subcontractors, must submit proof of insurance to VA.... 7.4 The Contractor will ensure that courier personnel are licensed and bonded and will enter into a Business Association Agreement (BAA) with the VA to protect information and address privacy concerns. 8.0. PERSONNEL REQUIREMENTS: The Contractor shall provide personnel, through direct hire or through subcontracting, in numbers and qualifications capable of fulfilling the requirements of this contract. The Contractor is responsible for assuring that all persons, whether they be employees, agents, sub-contractors, providers, or anyone acting for or on behalf of the Contractor, are properly licensed or always certified as applicable under Alaska state law. Contractor agrees to prepare and provide documentation of such prior to beginning work under the contract, whenever staffing changes occur, or upon demand by VA. 9.0. REIMBURSEMENT: Invoices must be electronically submitted to the VA Financial Services Center (FSC) for payment. Appendix 1 of this Statement of Work is the price list filled out by the Contractor when submitting their bid. The Contractor shall maintain records of all trips made during a particular month, and only charge for actual trips in accordance with the Itemized Price List in Appendix 1. Monthly invoicing shall be submitted along with the Contractor's list of trips made during that month for which the Government is being billed. 10.0. QUALITY ASSURANCE: 10.1. The Government will evaluate the Contractor's Performance using a Government Quality Assurance Surveillance Plan (QASP). The Government will measure the Contractor's control of quality against the Performance Thresholds specified in the Requirements Summary. If re-performance of a service delivery is required, the original inspection results will be recorded as unsatisfactory. When overall performance for a required service meets or exceeds the performance threshold, quality control is satisfactory. 10.2. If performance fails to meet requirements, the Contractor will be issued a Non-Conformance Inspection Report (NCIR) and given two hours after receipt of the notification to respond and to correct the non-conformance within 24 hours. If the contractor has ten or more minor deficiencies during the performance period, a Form 2 - Contract Discrepancy Report (CDR) may be issued by the Contracting Officer. CDRs may be issued for non-responsiveness, for repetitive minor discrepancies, for major discrepancies or at the discretion of the Contracting Officer. 10.3. For Units that are unsatisfactory and cannot be re-performed a deduction for that service will be withheld from Contractor's monthly payment. 10.4. The Contractor shall be available to meet with the Contracting Officer's Representative (COR) at least weekly during the first month of the contract and as necessary thereafter to discuss the contractor's performance. The time and location will be mutually agreed upon. The Government reserves the right to change the frequency of the meetings depending upon the quality of service provided by the Contractor.
