NOTICE OF INTENT TO AWARD SOLE SOURCE Project Title:� Newborn Screening Services for Phoenix Indian Medical Center Announcement No.: 75H71223Q00014 This is not a solicitation. Phoenix Indian Health Services (PIHS) hereby submits a notice of intent to award a sole source firm fixed price to acquire Newborn Screening Services for Phoenix Indian Medical Center located at 4212 N 16th St, Phoenix, AZ 85016. PIHS intends to negotiate and award a firm fixed price contract in accordance with Federal Acquisition Regulation (FAR) 13.501(a) Sole source acquisitions and FAR 13.106-1(b)(1)(i), only one source being available to meet the Government�s needs, to: STATE OF ARIZONA HEALTH SERVICES DEPARTMENT 150 150 N 18TH AVE PHOENIX AZ 850073232 The period of performance for this effort will be a 12- month base period and four (4) 12-month option periods.� The total estimated value is $1,064.475.00. The applicable NAICS code for this requirement is 621511, and the size standard is $41.5M. A determination by the Government not to compete this proposed contract acquisition based upon responses to the notice is solely within the discretion of the Government. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement. This notice does not obligate the Government to award a contract or otherwise pay for the information provided in response. The Government reserves the right to use information provided by respondents for any purpose deemed necessary and legally appropriate. Any organization responding to this notice should ensure that its response is complete and sufficiently detailed. Respondents are advised that the Government is under no obligation to acknowledge receipt of the information received or provide feedback to respondents with respect to any information submitted. Any solicitation resulting from the analysis of information obtained will be announced to the public in Federal Business Opportunities in accordance with the FAR Part 5. Furthermore, responses to this notice will not be considered adequate responses to a solicitation. No proprietary, classified, confidential, or sensitive information should be included in your response. The Government reserves the right to use any non-proprietary technical information in any resultant solicitation(s).
