Notice of Intent to Sole Source HT941023N0036 THIS IS A NOTICE OF INTENT TO AWARD A SOLE SOURCE NON-PERSONAL SERVICES CONTRACT AND IS NOT A REQUEST FOR COMPETITIVE QUOTES. The Defense Health Agency Contracting Activity (DHACA), Western Markets Contracting Division (WMCD), San Diego, intends to negotiate and award a firm fixed price contract for Cosmed Bod Pod Preventive Maintenance Services in accordance with Federal Acquisition Regulation FAR 13.106-1(b)(1)(i), only one source being available to meet the Government�s needs, to: COSMED USA, INC. 1850 Bates Ave Concord, California 94520-1240 The North American Industry Classification System (NAICS) code for this requirement is 811210 � Electronic and Precision Equipment Repair and Maintenance; the business size standard is $34.0 (millions of dollars). The Product Service Code (PSC) is J065 � Maintenance, Repair and Rebuilding of Equipment: Medical, Dental, and Veterinary Equipment and Supplies. The Los Angeles Air Force Base Medical Logistics Department requires for Cosmed Bod Pod Preventive Maintenance Services that meets the standards in the statement of work. It requires the manufacturer to provide annual preventive maintenance and calibration for Bod Pods at the El Segundo Clinic. The objective of this sole source award is to procure non-personal services for Bod Pod Preventive Maintenance Services. All scheduled servicing will be accomplished in accordance with the manufacturer�s recommendations and/or local procedures. The anticipated period of performance is for base with four (4) option years. Services include providing ongoing service of the Bod Pod GS/GS-X or Express. Coordinate all work requirements and scheduling for complete service of all materials with the department designated point of contact. Coordinate service with Los Angeles Air Force Base (LA AFB) � 61st Medical Squadron, following all command policies and procedures. Provide all services, replacement parts, labor, travel fees, and all tools/equipment necessary to supply and service at LA AFB. The service plan shall include an annual pre-scheduled Calibration Service with certification, software updates, and product training at time of the visit. Any damages will be repaired at the expense of the contractor to the satisfaction of the customer. Be responsible for the cleaning up of work areas immediately after their service have been completed. Cosmed USA Inc. is the Original Equipment Manufacturer (OEM). All replacement parts manufactured or marketed by Cosmed USA Inc. are compliant with OEM specifications. Cosmed USA Inc. has trained personnel who maintain the equipment through preventive maintenance agreements assuring Cosmed-manufactured equipment continues to operate in compliance with OEM specifications. Cosmed USA Inc. is the only source that can provide the service maintenance required. All completed work shall be certified to meet OEM specifications. Services through any third-party vendors will void warranty. The work shall be performed at: Los Angeles Air Force Base � 61st Medical Squadron 2305 Mercury Ave Bldg. 210 El Segundo, CA 90245 This notice of intent is not a request for competitive proposals, and no solicitation document exists for this requirement. Sources interested in responding to this notice are required to submit a capability statement that includes management and technical data and cost information, in sufficient detail and with convincing evidence that clearly demonstrates the capability to provide the required services for Los Angeles Air Force Base � 61st Medical Squadron. Capability statements shall not exceed six (8.5 x 11 inch) pages using a font size no smaller than 10-point. All capability statements received by the due date of this notice will be considered by the Government. A request for documentation or additional information or submissions that only ask questions will not be considered as an affirmative response. A determination by the Government not to compete based on responses to this notice is solely within the discretion of the Government. Information received will be considered solely for the purpose of determining whether to conduct a competitive procurement or to proceed with a sole source purchase order.
