ANSWERS TO INDUSTRY QUESTIONS Q.1: The pricing indicates �4. �Unit pricing for each of the following items:� I understand everything but the mobilization.� Do for mobilization is there is up to 6 trips and each trip would cost $1 the total would be $6.� However, the pricing included in the proposal should only be the $1 for the period of performance not the $6 because there is a possibility that we will only perform between 2 and 6 trips.� A.1: The quoted unit price is per mobilization event location. If that event is one location, the task order cost is the unit price times one. If the event has more than one location, the task order cost is the unit price times the number of locations.�This covers the mobilization of the truck to each location requested. ______________________________________________________________________________________________________ Q.2: Do you have historical data on contract utilization and level of effort summary that can be provided?� A.2: In previous years, two task orders have occurred within a 12-month period _______________________________________________________________________________________________________ Q.3: Is travel to a local site for required services acceptable? A.3: No. Performance Work Statement section 1.4.1 identifies the specific performance locations of individual task orders. _______________________________________________________________________________________________________ 1.�� �GENERAL INFORMATION: A.��� This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR 12.603, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; a quote is being requested, and a written solicitation will not be issued.� B.�� �The Virginia Army National Guard (VaARNG) is seeking the performance of medical surveillance examinations for full-time and temporary employees in the state of Virginia. The contractor for this requirement shall provide phlebotomy, vision, pulmonary, and hearing testing examinations, as well as related medical administrative services in accordance with the tasks identified in the Performance Work Statement (PWS). The place(s) of performance will vary but may occur at armories in the following areas: North Chesterfield, Virginia Beach, Gate City, Troutville, and Blackstone. Hearing exams must conform to the audiogram data listed on Department of Defense (DD) Form 2216, https://www.esd.whs.mil/Portals/54/Documents/DD/forms/dd/dd2216.pdf. The following roles are considered key for meeting requirements. Individual task order requirements may require multiple individuals at one or more sites. Board-eligible or Certified Occupational Medicine Physician / Certified Pulmonary Function Technologist: Pulmonary Function Tests (PFTs) will be performed by licensed physicians or personnel certified through a program approved by the National Institute for Occupational Safety and Health (NIOSH) in accordance with 29 CFR 1910.1001. Medical Technicians: Credentialed but will work under the auspices of the licensed provider. Audiogram Screening Technicians: Shall be certified by the Council for Accreditation in Occupational Hearing Conservation (CAOHC). 2. ���The Government will furnish the necessary workspace at each location. The contractor will be expected to provide the personnel, equipment, tools, and materials required to perform all services indicated in each task order. The Period of Performance shall be one (1) base year of twelve (12) months and three (3) twelve (12) month option years. Specific performance dates will be indicated on individual task orders. C.�� �The Government will award one Indefinite Delivery/Indefinite Quantity Contract (IDIQ) contract resulting from this Request for Quote (RFQ), solicitation number W912LQ-23-Q-0019. The associated North American Industrial Classification System (NAICS) code for this procurement is 621111 (Offices of Physicians, except Mental Health Specialists). The Product Service Code (PSC) is Q201 (Medical: General Health Care), with a business size standard of $16,000,000.00. �This requirement is being solicited as a 100% Small Business Set-A-Side solicitation, and only qualified offerors may submit quotes. Multiple awards will not be made as a result of this solicitation. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2023-02, dated March 16, 2023.� D.�� �The Government will not pay for costs associated with the preparation of a quote in response to this solicitation. E.��� To be considered for an award, vendors shall have an active registration in the System for Award Management (SAM) at https://sam.gov. Vendors are responsible to ensure the representations and certifications in their SAM registration are accurate and current. F.�� �Payment will be made through the Wide Area Workflow (WAWF) application in accordance with DFARS clause 252.232-7006, Wide Area Workflow Payment Instructions. G.� Interested vendors agree to hold the prices in their quote firm for 60 calendar days from the date specified for the closing of this RFQ. H.� The Contracting Officer, on behalf of the Government, intends to evaluate the quote and make an award without discussions.
