Description
SCOPE OF WORK Facility Pressure Washing & Cleaning Patient Care AHUs and Equipment Temple VA Project: 674-23-3-5066-0060 The contractor shall be responsible for providing supervision, labor and materials for the services required to provide pressure washing and cleaning as listed below in project requirements. Perform specified repairs as needed on Air Handler Units (AHU), Ducts, Return and Exhaust Fans and Emergency repairs as needed for Temple VA Campus located at the Central Texas Veterans Health Care Facility in Temple, 1901 Memorial Drive, Temple, Texas 76504. After all work is completed, the contractor will have a walkthrough with a VA representative to ensure all is completed and ready for payment. Site Visit: The Contractor shall visit the site to determine the full extent of the work. Failure to inspect the site will not be grounds for a claim after the contract award. A site visit will be scheduled through the Contracting Officer. Working Conditions: The buildings and all surrounding site area will be occupied, and Government operations will continue for the duration of the contract. The Contractor shall take all precautions to ensure that operations are conducted in a manner so as not to interfere with normal operation of surrounding facilities. The Contractor shall execute work in a safe manner and shall not subject personnel to unsafe conditions. The Contractor shall provide safety barricades, devices, and equipment to protect personnel and property. The contractor shall provide all personnel, services, vehicles, transportation, tools, equipment, materials, supplies, facility supervision, administration supervision and any other item(s) necessary to perform and accomplish this service. The contractor shall be familiar with all the terms, conditions, and requirement herein contained. All work must comply with federal, state, and local regulations. Workmanship: The contactor must be qualified/certified to work on commercial heating and cooling system. This includes but not limited to the startup, installation, and maintenance of all popular commercial air handling systems. All work shall be done according to the Infection Control Risk Assessment (ICRA) specified for the respective areas where work is performed. Documentation: Contractor shall, within 5 business days, provide a detailed report and analysis, both electronic and hard copy of all work performed and results to the COR or PM. Report shall contain, at a minimum, company logo, date of service, amount of time spent on service, reason and results, color photos of before and after and technician s name(s). At a minimum, The Contractor shall provide the following reports: Weekly, Monthly and Final. Note 1: The C&A requirements do not apply, and that a Security Accreditation Package is not required for this contract. Note 2: Contractor shall provide service calls as requested by the VA COR or PM. These calls will be billed separately and are not a part of this contract other than providing a set unit price for each service call. Price for each service call, not to exceed 2 hours on site time = $___1,500________. Service calls will provide basic troubleshooting, repair, and minor part replacement inclusive in set price quoted above. Major parts are not covered in the set price quoted above. Major repairs and parts will be billed separately, time and materials. All service calls shall be performed within 4 hours (on site) of notification. Note 3: a. It is the Contractors responsibility to ensure that all precautions and safety measures are taken to make the AHU system safe before and during the service. b. It is the Contractors responsibility to ensure that the AHU System is operational at completion of the service. If AHU is deemed Out of Service for any reason provisions shall be provided and implemented by the contractor to provide immediate emergency repairs. c. It is the Contractors responsibility to verify and record all types, model numbers and serial numbers for all serviced equipment and include this information in all reports. d. Label items above the ceiling to include duct access panels, VAV Boxes, dampners, etc. with 1 x 3-inch vinyl labels with black box lettering. The labels shall be attached to the ceiling grid. e. It is the Contractors responsibility to transport materials from the CONEX to the worksite. f. It is the Contractors responsibility to coordinate with the COR all shutdowns to include area accessibility, three weeks prior to proposed work date. g. Removal of demo through the hospital will be done in compliance with infection control standards. h. Provide a written Method of Procedure MOP for all work three weeks prior to outage. i. The Contractor shall move, cover, and protect furnishings as needed to access the ducts, coordinating with the COR and space users to ensure no vital equipment is disturbed. j. The Contractor shall put back any furnishings moved for performance of the work. Note 4: Most AHU work shall be performed after normal business hours and weekends to minimize patient care disruptions as needed. ALL shutdowns shall be coordinated and approved by the VA COR/POC. Note 5: The Contractor shall have an adequate number of crews on hand to meet the timeline and requirements of all services being requested. The Contractor shall provide a work schedule to include a detailed list of work to be done for each requirement and the sequence in which it will be accomplished. The schedule shall be in the form of a progress chart of suitable scale to indicate the work scheduled for completion by any given date during the work period. The work plan shall indicate time frames and programmed power shut-off, if applicable. Contractor shall highlight on the schedule any off-hours and overtime work requirements. Project Special Task Order Funds The Contractor shall provide all labor, supervision, materials, equipment, tools, and any other supplies to service or repair AHUs and Pressure Washing services that may arise or be discovered that are not specifically covered. This will be on a as needed basis and there will be funds allocated to awarded contract for repairs. Contractor shall provide a detailed narrative and line-item cost estimate for repairs or replacement not covered, within 2 business days to the CO and COR for approval. Contractor shall provide a single point of contact with all contact information. Base Year The Contractor shall provide all labor, supervision, materials, equipment, tools, and any other supplies to perform complete services of AHU, Duct Work and Return & Exhaust Fans for building 204 per the provided list and drawings and perform power washing two (2) times per quarter on selected Loading Docks and Decks, and once a year on Solar Field Panels, as defined in the Project Requirements. Exhaust Fan Isolation Rooms Option Year 1 The Contractor shall provide all labor, supervision, materials, equipment, tools, and any other supplies to perform complete services of AHU, Duct Work and Return & Exhaust Fans for building 163 per the provided list and drawings and perform power washing two (2) times per quarter on selected Loading Docks and Decks, and once a year on Solar Field Panels, as defined in the Project Requirements. Option Year 2 The Contractor shall provide all labor, supervision, materials, equipment, tools, and any other supplies to perform complete services of AHU, Duct Work and Return & Exhaust Fans for building 202 per the provided list and drawings and perform power washing two (2) times per quarter on selected Loading Docks and Decks, and once a year on Solar Field Panels, as defined in the Project Requirements. Option Year 3 The Contractor shall provide all labor, supervision, materials, equipment, tools, and any other supplies to perform complete services of AHU, Duct Work and Return & Exhaust Fans for building 221A and 221B per the provided list and drawings and perform power washing two (2) times per quarter on selected Loading Docks and Decks, and once a year on Solar Field Panels, as defined in the Project Requirements. Option Year 4 The Contractor shall provide all labor, supervision, materials, equipment, tools, and any other supplies to perform complete services of AHU, Duct Work and Return & Exhaust Fans for building 221C and 221D per the provided list and drawings and perform power washing two (2) times per quarter on selected Loading Docks and Decks, and once a year on Solar Field Panels, as defined in the Project Requirements. Project Requirements 2 Cleaning of Air Handlers Chemically wash coils (chilled and heating) in air handler with Blue Max Condenser Coil Cleaner by Airtron Corp. part # 1830 or approved equal. Flush condensate pans and drain lines. Wash and clean blower wheel and housing Wipe down blower shafts, bearings to remove excess grease and dirt. Wash and clean air handler interior housing to include all walls, floor, and ceiling. Seal any holes or cracks that may be found during the cleaning process. Use proper material that will adhere to the surface. Take before and after pictures to be turned in on the weekly report. Cleaning of Duct Work Clean, vacuum and sanitize all supply ducts from air handler to supply grille. Clean, vacuum and sanitize all return ducts from air handler to return grille. Clean, vacuum and sanitize all exhaust ducts from exhaust fan to exhaust grille. Seal all duct with interior insulation with Fosters 40/20 or approved equal upon completion of cleaning. Clean all supply, return, and exhaust grilles. Paint, repair or replace any grille that is damaged or stained beyond repair. Seal insulation blankets on top of grilles to prevent condensation. Repair or replace any blankets that may be damaged or missing. Replace all damaged flexible duct drops to registers. Take before and after pictures to be turned in on the weekly report. Note-It is the contractor s responsibility to cut and install gasketed access panels on all duct work for present and future cleanings. Cleaning of Return and Exhaust Fans Shutdown and lockout/tag out power supply Cut in gasketed access panels if needed to gain access to the interior of the fan. Clean, vacuum and sanitize the interior of the fan to include housing, motor, wheel/propeller, bearings, and shaft. Seal any holes or cracks in fan to prevent air leakage. Take before and after pictures to be turned in on the weekly report. Patios / Docks / Solar Field Panels Pressure washed with a biodegradable chemical twice (2) quarterly. The suggested chemicals to be used are liquid pool chlorine, surfactants, and vital oxides. Any other chemical used must be pre-approved by the VA POC. Borehair brushes shall be used on the solar panels. Some work will need to be completed after normal business hours for patient safety. Protect all surrounding areas from overspray and runoff. Take before and after pictures to be turned in on the weekly report. Other Considerations The contractor shall be responsible for the verification of all requirements for this project. All measurements and conditions must be verified by the contractor. Any discrepancies between field conditions, project documents, and this SOW shall be immediately brought to the Contracting Officer s attention. The contractor will be required to submit a safety plan for VA review and approval prior to the performing work. It is the contractor s responsibility to provide adequate documentation verifying that any equipment and/or materials proposed will meet or exceed all federal government requirements. Majority of the work shall be performed after normal working hours unless other wised noted by the COR. Normal working hours are 7:00 am 1800, Monday Friday. The contractor s supervisor shall always be available to address any needs the VA COR may have during the period of performance. The contractor will be required to coordinate work activities and shutdowns with VA COR and M&O Supervisors. The contractor shall thoroughly clean inside and outside of project site daily with contractor s own equipment, including vacuums, mops, buckets, wash rags, dust mops, etc. The contractor shall be responsible for the removal and disposal of demo. Contractor shall dispose of all materials off-site and all materials shall be recycled to the maximum extent possible. Codes and Standards The project shall be in accordance with the latest Uniform Building Code (UBC), National Fire Protection Associate (NFPA) standard, Uniform Federal Accessibility Standard (UFAS), VA Specifications and Standards, and local facility requirements. In the event there is a conflict between any of the standards/codes, the more stringent standard/code shall apply. The contractor shall provide all applicable safety signage and barricades applicable to OSHA, VHA and DOD regulations. Contractor shall comply with VHA DIRECTIVE 2004-012 and all VA Center Policy Memorandums, including but not limited to the following: Protection of patients, visitors, and employees from traumatic injury, as well as occupational and facility associated infections. Compliance with OSHA and State construction safety regulations. Construction contracts require that the general contractor s on-site competent person have completed the OSHA 30-hour construction worker course and that all other general and sub-contractor s workers have completed the OSHA 10-hour construction worker course or other relevant competency training. The determination for training is based on the project hazards and complexity, State and Federal regulations and VA requirements. Verification of completion of training will be required prior to performing work. Compliance with EPA and State environmental regulations. SAFETY PLAN REQUIREMENTS Prior to performing any work, the contractor will be required to submit a safety plan in accordance with OSHA and VA Regulations. The safety plan shall include, but not limited to the following: Safety and Health Policy (i.e. company policy) Safety and Health Objectives (i.e. company goals, procedures, etc) Job Site Inspections (i.e. safety inspection/potential hazards/prevention, etc) Contractor Safety Administrator (i.e. competent person/safety officer) Personal Protective Equipment (i.e. head, hearing, and eye protection, footwear, clothing, gloves, etc.) Safety Rules (i.e. rules applicable to the work/task being performed, equipment, ladders, confined spaces, open flame, etc.) Job Safety Training (i.e. OSHA, safety meetings, etc) 30 Hour OSHA Construction Safety Course competent person 10 Hour OSHA Construction Safety Course all others Safety Discipline (i.e. correction of violations, etc) Hot Work Permit (i.e. responsibilities, procedures, etc) Emergency Procedures (i.e. responsibilities, procedures, etc) Written Hazard Communication Program (i.e. responsibilities, procedures, list of hazards, MSDS, etc) SAFETY INSPECTIONS On a weekly basis, the VA will conduct a safety inspection of the work area. This inspection includes, but is not limited to the following: Means of Egress Fire/Smoke Doors unlocked, free of obstructions Means of Egress (includes Exits) free of obstructions and clearly marked Fire Alarm, Fire Detection, and Fire Suppression Systems. Fire Alarm Pull Stations are unobstructed Smoke Detector smoke caps in use, or removed at end of the day Hot Work Permits are current, in use Sprinkler Heads are unobstructed Housekeeping and Traffic Control Trash and debris are removed promptly and transported outside the construction area Floor mats and dust tack mats are located at the entrance to construction area Tools and equipment are properly stored Signs installed to restrict patient access (doors locked as needed) Dust control measures are in place Barriers are in place and properly sealed Hazardous Chemicals/Conditions Appropriate storage of hazardous chemicals MSDS sheets are readily available on-site Lock out/tag out procedures are in place Hot work permit is current and posted Clothing PPE is available and properly worn Clothing is relatively dust free with the construction area and patient areas Submit the proper documentation and reports to receive payment on this service. See information below: Invoices and Payment Payment to be made monthly, in arrears, following satisfactory performance by Contractor s personnel and upon receipt of a properly prepared invoice.  Contractor shall be required to invoice through the Tungsten System at http://www.tungsten-network.com/us/en/veterans-affairs/. Additional information regarding Tungsten shall be provided upon award. The contract shall have current registration in the System for Award Management (SAM) for this solicitation; URL: www.sam.gov. The invoice MUST be itemized to include the following information.  Any information listed below, and not provided on an invoice, will render that invoice incomplete and the invoice will be returned for immediate correction. Time period being invoiced for. Description of services performed. Unit Cost billed. Extended amount due. Invoice number, date, and; Contract and obligation numbers. Coordinate with VA COR before the start of this service.
