Description
Title of Project: Folding patient privacy screens for activation of new Primary Care Unit at Lebanon VA Medical Center (LVAMC). Background: The Lebanon VA Medical Center (LVAMC) has a brand-name or equal to requirement for twenty-four (24) folding patient privacy screens with twenty-four (24) wall mounting hardware kits for the Primary Care Unit Activation Project. The brand name provided for reference is the Silentia folding privacy screen with wall mounting hardware hits. Quantity 24 each. Scope of Work: The vendor shall provide all resources necessary to accomplish the deliverables described in this statement of need (SON), except as may otherwise be specified. Due to space limitations and room configuration, screens to protect patient privacy will be needed for the new Primary Care Unit. Performance Period: Delivery required no later than September 30, 2023. Equivalent products will be considered if they meet the following salient characteristics: Hard smooth surfaces that withstand all disinfectants utilized in healthcare setting All surfaces must be accessible for cleaning including removeable wheels Built-in feature that enables one hand pull and retraction for easy handling and prevention of cross contamination Ergonomically designed handle Shall be wall mountable with the option to mount on the left or right side Wall mount kit included Size: 9 length, 5 5 height, less than 50lbs Folded size: 8.7 depth, 9.8 width Color: Birch or Tan Delivery: a. The Contractor shall deliver all equipment required under this requirement by no later than September 30, 2023. This is a drop shipment, therefore, no installation is required. Access to an unloading dock will be available. b. Delivery of equipment shall be made to: Lebanon VA Medical Center Attn: Primary Care Activation 1700 S. Lincoln Ave. Lebanon, PA 17042 Delivery to the site shall be coordinated at least 72 hours prior to required delivery date with the designated Point of Contact (POC).
