- Description THIS REQUEST FOR INFORMATION IS ISSUED SOLELY FOR INFORMATION AND PLANNING PURPOSES ONLY AND DOES NOT CONSTITUTE A SOLICITATION. This is not a solicitation announcement. This is a Sources Sought synopsis only. The purpose of this synopsis is market research to gain knowledge of potential vendors qualified under NAICS Code 339113. Responses to this synopsis will be used by the Government to make appropriate acquisition decisions. After review of the responses to this Sources Sought synopsis, a solicitation announcement may be published in Contracting Opportunities or SAM.gov. The Veterans Integrated System Network (VISN 1), Department of Veterans Affairs (VA) has an ongoing program to furnish orthotic devices / appliances and related services (including - custom orthotics, braces, diabetic shoes, inserts, elastic supports for joints, stockings, compression garments, other new devices and repairs) to those Veterans that require them and are eligible for assistance. The range of services shall encompass Healthcare Common Procedure Coding System (HCPCS) Codes: L0100-L4999; A4466; A5500-A5513. and A6501-A6549. The orthotic appliances and related services are to be provided to the following facilities, and its associated Community-Based Outpatient Clinics (CBOCs). Vermont: White River Junction VA Medical Center Vermont 215 North Main Street (Mailing) 163 Veterans Drive White River Junction, VT 05009-0001 Bedford: Edith Nourse Rogers Memorial Veterans Hospital 200 Springs Road Bedford, MA 01730-1114 Boston: VA Boston Healthcare System (includes all three sites for Orthotics) Brockton Division 940 Belmont Street Brockton, MA 02301-5596 Jamaica Plain Division 150 S. Huntington Avenue Boston, MA 02130-4817 West Roxbury Division 1400 VFW Parkway West Roxbury, MA 02132-4927 Manchester: VA Medical Center Manchester, New Hampshire 718 Smyth Road Manchester, NH 03104-7007 Northampton: VA Central Western Massachusetts Healthcare System (Central and Western MA) 421 North Main Street Leeds, MA 01053-9764 Providence: Providence VA Medical Center 830 Chalkstone Avenue Providence, RI 02908-4734 Connecticut: VA Connecticut Healthcare System (Includes both sites for Prosthetics) Newington Campus 555 Willard Avenue Newington, CT 06111-2631 West Haven Campus 950 Campbell Avenue West Haven, CT 06516-2770 GENERAL REQUIREMENTS Approved Facilities the Contractor s facilities shall be accredited either by the Facility Accreditation Program of the American Board for Certification in Orthotics and Prosthetics, Inc. (ABC) or the Accredited Facilities Program of the Board for Orthotist- Prosthetist Certification (BOC). Contractor s facility must be within 100 miles of one of the VA locations as listed above. Educational and Training Requirements All contractors shall have a full-time American Board Certified (ABC), or Board for Orthotist-Prosthetist Certification (BOC),Orthotist on staff in good standing at each of their location with current and valid credentials for the State in which he/she practices in good standing. Requested Information If you are interested, and are capable of providing the required supplies, please provide the requested information as indicated below. Responses to this notice should also include: Company name address point of contact DUNS number and size of business Please indicate the size status and representations of your business, such as but not limited to: Service-Disabled Veteran Owned Small Business (SDVOSB) Veteran Owned Small Business (VOSB) with an active registration in VETBIZ (https://www.vip.vetbiz.va.gov). HUBZone Woman Owned Small Business(WOSB) Large Business, etc. Does your company have an FSS contract with GSA or the NAC or are you a contract holder with any other federal contract? If so, please provide the contract type and number. A copy of the ABC/BOC Facility Certification for each proposed facility location must be included in the response to this notice. Is your facility within 100 miles of one or more of the listed VA Medical Centers. Please specify which medical center(s) each facility you would support. Notes: Responses to this notice shall be submitted via email to Antoinette.sebastian@va.gov no later than May 10th, 2023, by 5:00PM. This notice is to assist the VA in determining sources only. A solicitation is not currently available. If a solicitation is issued it shall be announced at a later date, and all interested parties must respond to that solicitation announcement separately from the responses to this Sources Sought announcement.
