Description
Exam Tables Exam Tables for VA Medical Center Wilkes-Barre, PA Purpose and Objectives: The intent of this Sources Sought Notice is to identify potential businesses, especially any Small Businesses, Service Disabled Veteran Owned Small Businesses (SDVOSB) or Veteran Owned Small Businesses (VOSB) offerors capable of providing Exam tables as listed in the below General Requirements utilizing NAICS Code 339112. Responses to this Sources Sought Notice shall be submitted via e-mail at ronald.kline@va.gov. Information provided will not be returned. All responses shall be in the English Language. Responses are due by 12:00 pm (EST) on Wednesday, May 17, 2023. GENERAL REQUIREMENTS: The Wilkes-Barre VAMC is looking to procure the following: BRAND NAME OR EQUAL TO MIDMARK EXAM TABLES model 626-005 with upholstery tops 121 ea. BRAND NAME OR EQUAL TO exam table model number 626-006 along with Upholstery tops and the needed accessories for the 4-model number 626-006 tables, 4 ea. The WILKES-BARRE VAMC will work with the awarded contractor for a desirable delivery schedule. The project shall require the awarded contractor to deliver, set up, and install the exam tables in each designated room in the facility. The contractor must use premium delivery service, which will include delivery, unboxing, and setup of the Exam Tables directly to each designated exam room. The awarded contractor shall work with the POC Dennelle Ripka, Supervisor Inventory Management Specialist NX, phone 570-824-3521 X 27362 to establish a delivery itinerary that will provide 4 separate weekly deliveries of 31/32 tables during a 30-day window. The awarded contractor must provide the serial numbers for each table that will be delivered during each weekly delivery at least 48 hours in advance. VAMC Wilkes-Barre has an established delivery plan, and the awarded contractor will work with the POC Dennelle Ripka in facilitating the delivery accordingly. EXAM TABLE SALIENT CHARACTERISTICS Salient Features (Brand Name or Equal): 125 exam tables total: 121 ea. of model 626-005 and 4 ea. of model 626-006 - include a Power height, Power backrest that will be Compress to at least a 14.6 low seat height upholstery allowing most patients to have feet flat on the floor for proper positioning for blood pressure capture and extend to a maximum 37 high height - have a Patient Transfer Position (PTP) that must have a low FLAT back position at 15.5 for true patient safety - meet US Access Board recommendations for accessibility with 15.5 uncompressed low seat height - have pelvic tilt and a heated drawer - include a Built-in digital scale with a minimum weight capacity of 500 lbs. to at least a maximum of 650lbs. - be able to provide automatic BMI calculation - include a wired hand and foot control - have manual adjustments in case of a power outage - be able to go from Table position or flat table position at any height - contain a front storage drawer with dividers for access/storage of speculums, gloves, instruments, and supplies - include Paper roll storage - Multi-position hideaway stirrups Upholstery top must be removable for cleaning or replacement - Mist base color; Stone molded accents - include automated vital device ports Color of upholstery top must be latte in color Salient Features for model # 626-006 (total 4 ea.) Same as above plus these 4 Model # 626-006 must include: have heated Upholstery top for Models 626-006-32 in premium Latte color Ritter 253 LED Exam Light - Exam Table Kit for 32 In Upholstery must be added to table Free plug-n-play attachment of accessories and have patient support rails Be equipped with patient support rails plus be field installed for model # 626-006 (total 4 ea.) INSTALLATION AND DELIVERY: Installation shall be performed upon delivery at the Wilkes-Barre VAMC by the awarded contractor using premier delivery which entails: Contractor must provide Premier Delivery Service should be valid for at least 30 days * Contractor must provide a single point of contact for all project coordination for the 4 consecutive weekly deliveries of 31/32 tables and must coordinate deliveries with Wilkes-Barre s POC Dennelle Ripka * Contractor must have a trained team to provide delivery and setup for all equipment and accessories * Delivery and setup service includes delivery, uncrating, trash removal, product and accessory setup, AND SET IN PLACE throughout the facility in accordance with the needs of the facility which will be coordinated with the Wilkes-Barre VAMC s point of contact Dennelle Ripka. *Contractor will assemble product and deliver to customer location per 4 determined phases *Remaining product will be stored at deliver agent's warehouse until all phases are complete *Two days are reserved for each phased delivery (no Saturdays) *Contractor delivery must be completed within 4 weeks {30 days} of ship date and can only be delivered to WB VAMC Monday -Friday 8am to 4pm. Excluding all gov t holidays. This RFI will be conducted in accordance with Federal Acquisition Regulation (FAR) Part 13. Telephone responses will not be accepted. Responses must be received via e-mail to Ronald.kline@va.gov no later than, 12:00 PM Eastern Standard Time (EST) on Wednesday, May 17, 2023. This notice will help the VA in determining available potential sources only. Do not contact VA Medical Center staff regarding this requirement, as they are not authorized to discuss this matter related to this procurement action. All questions will be addressed by the Contracting Officer Ronald Kline at ronald.kline@va.gov. Questions or Responses will include the Source Sought number 36C24423Q0833 in the subject line. All firms responding to this Request for Information are advised that their response is not a request for proposal, therefore will not be considered for a contract award. If a solicitation is issued, information will be posted on the beta.sam web site for all qualified interested parties at a later date and interested parties must respond to the solicitation to be considered for award. This notice does not commit the government to contract for any supplies or services. The government will not pay for any information or administrative cost incurred in response to this Request for Information.
