Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with (IAW) the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The Government reserves the right to make no award from this solicitation.   This is a request for quote (RFQ), and the solicitation number is 36C25023Q0694. The Government anticipates awarding a firm-fixed price Purchase Order resulting from this solicitation. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circulars 2023-02, effective 03/16/2023. The associated NAICS Code is 334510, Electromedical and Electrotherapeutic Apparatus Manufacturing The Department of Veterans Affairs, Network Contracting Office 10, is soliciting quotes from all sources to supply the Dayton VA Medical Center HFA3 model 860 visual field devices to accurately measure and analyze data to clinically assess and treat ocular conditions. This is a direct replacement of their existing visual field perimeter analyzers that are near life end expectancy. The requested analyzers must be able to upload data to both CPRS Imaging/DICOM and to our existing Carl Zeiss imaging viewer FORUM. Respondents to this solicitation must fully demonstrate their capability by supplying detailed quote information, along with any other documents necessary to support the requirements below. However, we believe only Carl Zeiss authorized distributors can offer this requirement. This requirement is set-aside to small businesses. Quotes are to be provided to Gina P. Crank, Contracting Officer, via email at gina.crank@va.gov, no later than May 17, 2023, by 5:00 PM EST. No telephone calls will be accepted. Third St. Dayton, OH 45428 Brief Description of Supplies/ Services required and the intended use/Estimated Amount: This transaction is for the purchase of two HFA3 MODEL 860. The visual field perimeter is standard diagnostic equipment used to accurately measure and analyze data in-order to clinically assess and treat ocular conditions. The two current perimeters, acquired Dec. 2012, will exceed life expectancy Dec. 2025 and they are already beginning to malfunction. Once the current visual field machines are no longer working they cannot be repaired. They are outdated and parts are no longer available to repair them. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used: This is a direct replacement of the existing visual field perimeter analyzer. Carl Zeiss is the only manufacturer of the HFA3 model 860 Visual Field Analyzer. This device is part of the standard of care when evaluating and treating patients for different types of glaucoma and neurological ocular conditions. The requested analyzers must be able to upload data to both CPRS Imaging/DICOM and to our existing Carl Zeiss imaging viewer FORUM. Description of market research conducted and results or statement why it was not conducted: The following methods were used when conducting market research for this devise: Participated in communication among industry personnel. Reviewed product literature published by manufacturers, distributors and dealers available on-line. Applied personal knowledge in procuring devices of this type. In addition, the Contracting Officer, using NAICS 334510 searched VetCert which resulted 116. Searched DSBS with the same NAICS which resulted 606, added key words resulted 0. The OEM stated they have two authorized distributors; Corps Biologics, a SDVOSB, and Proaim a SB. The OEM, Carl Zeiss, does not distribute directly rather uses their authorized distributors. This will post on Contract Opportunities, set-aside to small businesses. The VA Rule of Two is Unattainable.
