- B.3 STATEMENT OF WORK - UPDATED Fisher House Lodging VA Greater Los Angles Medical Center 1.0 INTRODUCTION The Department of Veterans Affairs Greater Los Angles Medical Center (GLA), 11301 Wilshire Blvd, Bldg. 523, Los Angeles, CA 80073 has a daily requirement for up to ten (10) hotel rooms for lodging of eligible family members as described herein. These families are normally lodged in the Fisher House, a temporary lodging facility for family members or caregivers of patients admitted for treatment or rehabilitation. Contracted hotel rooms are required for overflow to accommodate family members or caregivers who live at least 50 miles from the VA Greater Los Angles Medical Center in order for them to be at the bedside of the patient and/or involved in the patient s rehabilitative care. Contracted rooms will be for a period of a few days up to 4 weeks or more. The number of required lodging rooms will vary from week to week. 2.0 Hotel Room Requirements. Contractor shall provide daily up to ten (10) private rooms able to accommodate four guests maximum in each room. Contractor shall provide hotel rooms that meet the following guidelines: Four (4) of the ten (10) hotel rooms shall be private American Disabilities Act (ADA) compliant hotel rooms. ADA compliant rooms shall be wheelchair accessible, have no physical barriers and shall comply with the applicable National Fire Protection Association (NFPA) 101 and ADA guidelines. Hotel rooms shall be within a five (5) mile radius of the VA Greater Los Angles Medical Center, 11301 Wilshire Blvd, Los Angeles, CA 90073 without exception. All hotel rooms shall have a minimum 3 star rating with the following internet websites: www.hotels.com www.expedia.com Hotel rooms shall be fully furnished to include utilities, TV, clock radio, refrigerator, microwave, coffee maker, hair dryer, iron/ironing board and local phone access. Laundry facilities available for guest usage on site. WIFI shall be provided at no cost to the occupant. Hotel shall offer a complimentary breakfast. Hotel shall comply with city, county and state building codes and ordinances applicable to the ownership and operation of the building in which the rooms are located. If elevators are a part of the room complex, they shall conform to the latest requirements of the American National Standard (ANS) A171 and A171a Safety Code for Elevators, except for elevator emergency recall and shall be inspected and maintained in accordance with ANS A17.1, Inspector s Manual for Elevators or equivalent local code. The elevators shall also comply with local codes and ordinances. Hotel rooms shall be non-smoking. Hotel shall have cribs and cots available as needed for minimal charge. Hotel shall allow service animals at no additional charge. Hotel shall offer early check-in as available for guests needing this service. Parking shall be included in the hotel room rate. 3.0 ROOM RESERVATIONS: It is estimated that the Government will need up to 360 rooms per year at this time. The requirement for up to 10 hotel rooms daily is an estimate as the number of Veterans residing at the hotel is expected to vary daily. The Fisher House located at VAGLAHS is the primary housing location for veterans receiving planned and unplanned procedures. As the planned procedures per Veteran are received from the hospital by Fisher House (FH) prior to the scheduled procedure at VA Greater Los Angeles Healthcare System (VAGLAHS), the Government will evaluate the space available within FH. If FH is fully booked, the Government then identify the number of Veteran(s) or Veteran families who need a hotel room and for how many nights. For same day lodging requests, the hospital Emergency Room (ER) informs FH by 3:30pm PST or later if a Veteran has a same day admission or need. Once FH receives the request, the Veteran is then contacted to ensure that they can arrive at the FH to check in on or before 6:00 pm PST. If the Veteran will arrive after 6:00 pm PST, FH will then refer them to the hotel for one (1) night and then reassess the FH lodging availability the following day. Hotel rooms shall be occupied only by the specific individuals referred to the Contractor by the Contracting Officer s Representative (COR). The COR, who is the Fisher House manager, will inform the Contractor of the individual s name, arrival date, and check-out date. The Contractor shall make the reservation with their contracted hotel and provide the information including hotel name, address, phone number and confirmation number to the COR. The COR will communicate any changes to the arrival or departure date to the Contractor to make the adjustment with the hotel within 24 hours of a scheduled arrival or departure date. The COR will brief referred guests of their assigned hotel accommodations including check-in and check-out times and any other necessary information. The Contractor s hotel shall have a 24-hour cancellation policy. Authorized dates and length of stay will be documented on e-mail communication between the COR and the Contractor. In the event a Fisher House referred guest refuses to leave the room, the Contractor shall immediately notify the COR. Should a Fisher House referred guest check out prior to their scheduled departure date, the Contractor shall immediately notify VA COR. The VA COR may request additional rooms in excess of ten (10) on an as needed basis. Contractor shall provide additional rooms at the same rate. INVOICES Contractor shall only invoice for hotel rooms occupied by referred guests. Contractor shall provide a report of the number of rooms occupied by referred guests, the names of authorized individuals, the assigned room numbers, and occupancy dates. This list will be sent to the COR for pre-approval prior to submitting invoices into tungsten for payment. Payment terms are monthly in arrears. The beginning of each month an invoice will be submitted for all rooms used during the previous month 5.0 EMERGENCIES If the Contractor is made aware of any emergency pertaining to referred guest residing in their contracted hotel such as a 911 call or hospitalization, the Contractor shall notify the VA COR immediately. 6.0 INSPECTION The Government shall secure permission from the hotel to inspect the hotel rooms and all other areas of the complex to which access is necessary to ensure a safe and healthy environment for Fisher House referred guests. In addition, a Fisher House representative may monitor facilities, to include rooms, for cleanliness, odors, mold, and housekeeping procedures to include the upkeep of linens, towels etc. 7.0 REPORTS Contractor shall provide the COR with monthly reports to include any specific issues for the previous month along with solutions taken to correct any issues. Contractor shall provide to the COR quarterly activity reports that includes the following: any complaints regarding wheelchair (special mode) accessibility noted by Fisher House referred guests any complaints regarding rooms not cleaned upon check-in noted by Fisher House referred guests any complaints noting non-working available electronic equipment in Fisher House referred guests room such as the clock alarm, lighting, television, etc.
