- ** AMENDMENT A00001 Backflow Prevention Testing Leeds VAMC Contractor Questions and Government Responses and Solicitation Due date update ** Question 1. At some locations the city requests that reports are sent to them and the vendor must be certified in that state. Do we (the vendor) need to be certified in MA or do you know if this is a requirement or if we can use our backflow licensing from another state? Answer 1. Testing shall be conducted by a Massachusetts Certified Backflow Prevention Device Tester in accordance with 310 CMR 22.22. Question 2. Is this a new requirement or a follow-on requirement? If this was done in the past can you provide the contract number? Answer 2. This is a new requirement. Question 3. Is there a backflow preventer down a 70 ft tunnel that you need special equipment and training for? If so, how many and is it acceptable to provide a quote omitting those items? Answer 3. There is not a backflow preventer down a 70 ft tunnel, but there is a backflow preventer in the crawl space of building 9A and building 60 that will require the following, All confined space entry shall comply with 29 CFR 1926, Subpart AA except for specifically referenced operations in 29 CFR 1926 such as excavations/trenches [1926.651(g)]. A site-specific Confined Space Entry Plan (including permitting process) shall be developed and submitted to the COR. A required confined space permit or a permit exempt space entry certificate will be required depending on the type of work. See both forms attached. Both forms require an attendant to be present when the authorized entrant is in the crawl space. Atmospheric testing is also required, readings need to be sent to Safety before entry. Question 4. Will the VA require a person to be at the Fire Command center located in the PD during the testing? Answer 4. No Solicitation Due Date: Original: (xv) RFQ responses are due June 2nd, 2023, at 3:00 PM EST. RFQ responses must be submitted via email to: Joshua.Gallien@va.gov.
