- VHAPG Part 813.106 Simplified Acquisition Procedures: Soliciting from a Single Source Attachment 1: Single Source Justification for SAP under the SAT DEPARTMENT OF VETERANS AFFAIRS Justification for Single Source Awards IAW FAR 13.106-1 For Over Micro-Purchase Threshold but Not Exceeding the SAT ($250K) Acquisition Plan Action ID: 36C262-23-AP-1666_ 1. Contracting Activity: Department of Veterans Affairs, Regional Procurement Office West (RPOW), Network Contracting Office 22 (NCO-22), 4811 Airport Plaza Drive, Suite 600, Long Beach, CA 90815. Transaction Number: 691-23 -1-5042- 0109 2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount: In order to maintain compliance with VHA directive policy 0320.07, the National Disaster Medical System and Joint Commission standards, GLA maintains medical supplies on the following emergency facility equipment: DUV kits, Traige and Treatment Kits, and Alternative Care Site kits. The DUV kits are used during evacuation events, T&T kits are used during surge events to the Emergency Department, and ACS kits are used if relocation at the GLA facility is needed outside of the main hospital. The medical facility cannot meet the requirements or effectively create an emergency management plan without knowing the exact number of supplies in its inventory. According to the VHA policy, the GLA facility has a mandate to deliver consistent and standardized emergency management training to all VA personnel. Currently, the safety office of VA GLA has been able to maintain the yearly emergency medical supply records and meet its mandate for assessing how crisis would impact the current supply; through the use of the packaged Response Systems from Vericore LLC, a small business vendor. GLA Healthcare facility is able to manage this in conjunction with the current equipment, which includes; Dual Use vehicles (DUV s) , various Triage and Treatment kits (TandT), and two Alternate Care sites (ACS). The replenishment of the organization and medical supply allotment is configured to the specifications of the GLA location and the request of the medical professionals in the facility departments. Services are required to accompany this supply replenishment. Estimated amount: $98,143.00 3. Unique characteristics or circumstances that limit the requirement to Brand Name Only commodities. The requested commodities supply, and replenishment service is specifically ordered for the GLA hospital s emergency management contingency plan, and specific to the departments within this location. The current equipment at this site houses sustainable and deployable DUV kits, Triage and Treatment trailer kits and Alternate Care Sites kits, which must be inventoried on a yearly basis and re-stocked with identical items. To support the emergency supply surplus, there is currently an easily accessible centralized list of supplies (SmartBook and Smartbook DVD) and training materials, that must be updated in order to meet the needs of the GLA staff in the event of a crisis. Factoring in the number of personnel, GLA Veteran patients, and the VHA directive policy; this supply surplus will continue to substantiate an approximate 96-hour time period in the event of an emergency evacuation. Since the VHAPG Part 813.106 Simplified Acquisition Procedures: Soliciting from a Single Source Attachment 1: Single Source Justification for SAP under the SAT current supplies are clearly marked with lot numbers, expiration and serial data for all future replenishment activities, this request is necessary in order to meet compliance and continue supporting the current actionable response system. 4. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used: The existence of the current ACS kits, DUV kits and TNT triage systems are systems that were put together by VeriCor LLC. Upkeep of critical data (lot #, serial #, expiration date and medical supply location), the introduction of new inventory items, and regular replenishment data in digital and printed form, which deems it necessary for the acquisition of the same replenishment and supply service for the purpose of meeting compliance. Upon further research, no other company is able to provide historical inventory supply data which will align with replenishment services for all three of these customized kits; thus, providing the totality of services which will be immediately deployable. The small business vendor, Vericore LLC; is the only vendor to provide the customizable training, replenishment services and inventory manuals necessary, for the GLA VA hospital to prepare for, respond to , and recover from a sudden crisis. 5. Description of market research conducted and results or statement why it was not conducted: A search of of the Sac did not return any mandatory contracts for the required items. Market research on the NAC database did not return any listings for this type of requisition supply. A search of the GSA Federal Supply Schedule returned a listing that VeriCore LLC is a GSA contract holder (GS-07F-082) but the required items are not listed under this contract. VeriCor is the sole proprietor of the Response Systems and kits, and a letter of proprietorship has been received and filed. VeriCore LLC is not listed on the VIP database. However, the small business vendor is registered in the Systems for Award Management (SAM.GOV) and does not appear on the Excluded Parties List (EPLS). 6. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b).
