- Page 3 of 3 Evidence Based Practice Platform 36C77623Q0403 The Department of Veterans Affairs (VA) Veterans Health Administration (VHA) requires an existing commercial off the shelf web-based product named the KLAS Arch Collaborative Subscription that will be made available throughout the VA healthcare system. KLAS Enterprises, LLC., shall provide for the following: An annual subscription for access to a robust database of survey-based data on change management practices plus monographs on change management and case studies for research, standardized clinician surveys and benchmarking for healthcare organizations to uncover best practices. Services Include: -Subscription Services -Optional Consulting Face to Face Meetings This procurement is being conducted in accordance with FAR 6.302-1, Only One Responsible Source and No Other Supplies or Services Will Satisfy. The NAICS code for this procurement is 519290 and the Small Business Administration Size Standard is 1000 employees. This notice of intent to sole source is not a request for competitive quotes. No solicitation document is available and telephone requests will not be accepted. However, any responsible source who believes itself capable of meeting the requirements may submit a capability statement, proposal, or quotation, which shall be considered by the agency, only if received by the closing date and time of this notice. Responses received will be evaluated; however, a determination by the Government not to compete the proposed procurement based upon responses received to this notice is solely within the discretion of the Government. Responses to this notice are due on or before 6/21/2023, 12:00 pm ET to this procurement s contract specialist, Benjamin Niznik, at Benjamin.Niznik@va.gov. Submittal Information: Contractors having the skills and capabilities necessary to perform the stated requirements should submit a single spaced, 12-point font minimum, response to the information identified in the Sources Sought Notice. Please also submit a cover page which includes, at a minimum, the company s name, address, Dunn & Bradstreet (DUNS) number, SAM.gov Unique ID, General Service Administration (GSA) contract number (if applicable), point-of-contact name, phone number, and e-mail address. Companies shall provide clear and unambiguous evidence to substantiate their capability to fulfill the requirements outlined in this Sources Sought Notice.
