Page 3 of 3 This is a Sources Sought Notice for Intraoperative Neurophysiologic Monitoring (IONM) services at the Durham VAMC. IONM services are key technical support for successful outcomes of complex brain and spinal cord procedures. Estimated number of IOM services is estimated at 200 OR cases per year. Please respond if you have the ability and intent to complete this requirement. Include in your response a copy of your capabilities statement and FSS or GSA Contract Number, if applicable. This Sources Sought Notice is intended to locate Service-Disabled Veteran Owned Small Businesses (SDVOSB), Veteran Owned Small Businesses (VOSB), and Small Businesses. The anticipated NAICS Code for this requirement is 621399. This is not a Request for Quotation and quotes shall not be reviewed at this time. The Department of Veterans Affairs shall not be responsible for any costs incurred by the contractor as a result of this Sources Sought Notice. This notice shall not be construed as a commitment by the Government to ultimately award a contract, nor does it restrict the Government to a particular acquisition approach. All information submitted in response to this announcement is voluntary; the Government will not pay for information requested nor will it compensate any respondent for any cost incurred in developing information provided to the Government. Minimum needs of the Government are as follows: Certifications Minimal requirement-Registered Electroencephalographic and Evoked Potential Technologist certification (R.EEG T) Preferred requirement- Certified Neurophysiologic Intraoperative Monitoring Technologist (CNIM). Basic Life Support (BLS) certification and additional credentialing and privileging forms. Employed in Neuro diagnostics field with minimum 2 yrs. clinical experience. SERVICES REQUIRED: Intraoperative Neuromonitoring Contractor will provide required monitoring equipment and probes Confers with the surgeon regarding structures at risk, modalities to be monitored, and documents the conversation Communicates IONM preferences to the anesthesia team and other operating room personnel in a clear, definite, and collegial manner and documents conversation(s) before the patient enters the operating room suite, and/or during intubation and prepping: Sets up and confirms proper operation of all equipment Applies electrodes (primary and backup) and secures placement Tests equipment and checks integrity of electrodes by checking and documenting impedances Arranges head box, cables, and electrodes for minimization of artifacts and electrical hazards preventing electrodes from being dislodged, dried out, or contaminated with fluids Obtains initial responses and obligate peaks after induction and positioning prior to incision and sets baselines as appropriate to the procedure; marks waveforms and calculates the absolute latencies, amplitudes, or inter-peak intervals at baseline. Sets or resets baselines as appropriate to the surgical procedure per the facility policy and procedures, but in all cases prior to any part of the surgery that puts neural tissue at risk Obtains an interpretation of baselines from the attending neurophysiologist and communicates the information to the surgeon Throughout the surgery, reports any change in data which meets the alarm criteria outlined in the facility policy and procedure manual During the recording, changes methods of data collection as needed per facility policies and procedures including: Adjusting stimulus rate as needed to reduce time-locked artifacts Establishing and documenting that stimulating parameters are within safe limits Recording from additional electrode derivations in case of technical problems to allow continuous recording Use a montage that records obligate peak responses from peripheral nerve, spinal cord, subcortical structures, and the cerebral cortex as appropriate. At the end of the procedure, removes and discards disposable supplies, especially sharps and contaminated items per facility policies and procedures; cleans and disinfects equipment, cables, etc. All responses shall be submitted via email by Friday, June 16, 2023 at 13:00 EST.
