- Addendum to 52.212-1, Instructions to Offerors � Commercial Products and Commercial Services. Army Contracting Command � Rock Island (ACC-RI) is issuing a Request for Quote (RFQ), W519TC-23-Q-JMCA for a 360 Assessment Tool and Consultation in accordance with Performance Work Statement (PWS) Attachment 0001 � 1.6.1 PWS JMC Leader Assessment Project in support of US Army Joint Munitions Command (JMC). This requirement is issued in accordance with (IAW) Federal Acquisition Regulations (FAR) Part 12 � Acquisition of Commercial Items and FAR Part 13 �Simplified Acquisition Procedures. This requirement is a 100% Small Business Set-Aside; only small business vendors under North American Industry Classification System 611430 (Size standard: $15 million) that can perform all requirements in accordance with Attachment 0001 � 1.6.1 PWS JMC Leader Assessment Project authorized to provide a quote. Vendors that do not meet these requirements will be determined non-responsive and removed from the competition and will not be further considered for award. ACC-RI intends to issue a single Firm Fixed Price (FFP) Purchase Order.� A.�� �DESCRIPTION On behalf of JMC, ACC-RI seeks to purchase a 360 Leader Assessment and Consultations with specific requirements annotated in section 1.3 Objectives of the Attachment 0001 � 1.6.1 PWS, Attachment 0001 JMC Leader Assessment Project. Failure to quote all items may render the quotation non-responsive. If a quotation is deemed non-responsive, the quotation may not be further evaluated and may not be further considered for award. The Period of Performance (timeline) shall be 90 days upon the Purchase Order award date and the number of calendar days between events. Time allocations are maximums for each stage. It is acceptable to complete a stage in fewer days than shown on the timeline and move on to the next stage. The Period of Performance (timeline) is provided in Attachment 0008.� B. INSTRUCTIONS TO OFFERORS: B.1. SUBMISSION OF QUOTATION GUIDELINES: The quotation shall be prepared and written in a clear, concise, and legible manner. In addition, the quotation shall be written in English and must be specific and complete as described in these instructions. Adherence to the prescribed format is required. Failure to provide a quotation in compliance with the instructions specified in this RFQ may render the quotation non-responsive. If a quotation is deemed non-responsive, the quotation may not be further evaluated and may not be further considered for award.� No classified material shall appear anywhere within the quotation.� Submission of a quotation shall be on an all or non-basis. Partial quotations are not acceptable and shall not be evaluated.� Quoter must be registered in the System for Award Management and have a Commercial and Government Entity Code.� Digital Microsoft Word files and Microsoft Excel files shall be compatible with Microsoft Office 2013 and on.� Please note: Do not lock or password protect any file (e.g., *.doc, *.pps, *.xls, *.txt, *.msg).� Adobe PDF (Portable Document Format) Files: Scanned documents must be legible and shall have the ability to be viewed in Adobe Acrobat.� Compressed (e.g., *.zip) and executable files (e.g., *.bat, *.exe, *,jar, *.vb, *.wsf) will NOT be accepted.� The Government�s required method of quotation submission is via email to the POCs listed in Section L of this RFQ. NOTE: The Government will not accept hard copy, facsimile, or CDs/DVDs.� B.2. SUBMISSION OF QUOTATION FORMAT:� Submission of a quotation shall provide the following TWO volumes:� B.2.1. VOLUME I: TECHNICAL: The quote shall be submitted in accordance with the following instructions:� 1.�� �Sub-Factor: Leader Assessment Tool a.�� �The vendor shall submit a sampling of questions from the Tool that identifies and addresses criteria to measure the six attributes and competencies of the Army Leadership Requirements Model as they are defined by ADP 6-22, Army Leadership and the Profession (Attachment 0004). b.�� �The vendor shall submit a total of six sample questions that address the following attribute and competency criteria: i.�� �Attributes 1.�� �Character: Army values, Empathy, Warrior /Service Ethos, Discipline, Humility 2.�� �Presence: Military/Professional bearing, Fitness, Confidence, Resilience 3.�� �Intellect: Mental agility, judgement, Innovation, Interpersonal Tact, Expertise. ii.�� �Competencies 1.�� �Leads: Leads others, Builds trust, Extends influence, Leads by example, Communicates. 2.�� �Develops: Prepares self, Creates a positive environment, Develops others, Stewards the profession. 3.�� �Achieves: Gets results � anticipates, integrates tasks, roles, resources and priorities; improves performance; gives feedback; executes; adapts. 2.�� �Sub-Factor: Individual Reports a.�� �IAW paragraphs 5.2.8, 5.2.9, and 5.2.10 of the PWS (Attachment 0001), the vendor shall submit with their quote an outline of the structure and content that the individual report will contain. At a minimum, the report should contain breakouts outlining the criteria for each attribute and competency.� 3.�� �Sub-Factor: Consultation sessions a.�� �IAW paragraphs 1.6.11.2 and 5.2.17 of the PWS (Attachment 0001), the vendor shall provide a bio for the consultant that demonstrates 3+ years of leadership coaching experience The bio should identify the consultant�s experience with both public and private sector customers. b.�� �The vendor shall submit with their quote an agenda/syllabus that outlines the discussion and activities that may occur during the consultation session. 4.�� �Sub-Factor: Period of Performance (timeline) a.�� �The vendor will provide confirmation of the Government�s (Agency) period of performance (timeline) as noted in paragraph A/attachment 0008 of this RFQ.� B.2.2 Volume II: Price 1.�� �Attachment 0003 � Price Matrix RFQ: Fill-ins highlighted in yellow completed by the Vendor. Vendor shall complete all areas highlighted in yellow. Failure to complete all highlighted blocks may render the quote non-responsive.� NOTE: Failure to provide all the data required for both volumes may result in a quote being determined non-responsive and eliminated from further consideration for award. C. EVALUATION CRITERIA:� C.1. TECHNICAL:� C.1.1 Technical Evaluation Criteria: � 1.�� �Sub-Factor: Leader Assessment Tool: The Government will evaluate the offeror�s total quantity of six sample questions as follows:� a.�� �ACCEPTABLE: To be determined acceptable a total of six samples questions shall be submitted with each attribute and competency addressed as defined below:� Attributes 1.�� �Character: Army values, Empathy, Warrior /Service Ethos, Discipline, Humility 2.�� �Presence: Military/Professional bearing, Fitness, Confidence, Resilience 3.�� �Intellect: Mental agility, judgement, Innovation, Interpersonal Tact, Expertise. Competencies 4.�� �Leads: Leads others, Builds trust, Extends influence, Leads by example, Communicates. 5.�� �Develops: Prepares self, Creates a positive environment, Develops others, Stewards the profession. 6.�� �Achieves: Gets results � anticipates, integrates tasks, roles, resources, and priorities; improves performance; gives feedback; executes; adapts. b.�� �UNACCEPTABLE: To be determined unacceptable the submission does NOT include a total of six sample questions with each attribute and competency addressed as defined below: Attributes 1.�� �Character: Army values, Empathy, Warrior /Service Ethos, Discipline, Humility 2.�� �Presence: Military/Professional bearing, Fitness, Confidence, Resilience 3.�� �Intellect: Mental agility, judgement, Innovation, Interpersonal Tact, Expertise. Competencies 4.�� �Leads: Leads others, Builds trust, Extends influence, Leads by example, Communicates. 5.�� �Develops: Prepares self, Creates a positive environment, Develops others, Stewards the profession. 6.�� �Achieves: Gets results � anticipates, integrates tasks, roles, resources, and priorities; improves performance; gives feedback; executes; adapts. 2.�� �Sub-Factor: Individual Reports: a.�� �ACCEPTABLE: To be determined acceptable, the outline shall be IAW paragraphs 5.2.8, 5.2.9, and 5.2.10 of the PWS, Attachment 0001 and provide an example analysis statement for each attribute and competency identifying it as a strength or a gap as determined from the analysis.� b.�� �UNACCEPTABLE: To be determined unacceptable, the outline is NOT IAW paragraphs 5.2.8, 5.2.9, and 5.2.10 of the PWS, Attachment 0001 and there no example analysis statements for each attribute and competency identifying it as a strength or a gap as determined from the analysis.� 3.�� �Sub-Factor: Consultation sessions a.�� �ACCEPTABLE: �To be determined acceptable, a Bio shall be IAW paragraphs 1.6.11.2 and 5.2.16 of the PWS, Attachment 0001 and demonstrate 3+ years of leadership coaching experience. Experience identifies type of customer, public or private sector customers. The vendor submitted a proposed agenda/syllabus that outlines the discussion topics regarding the Army Leadership Requirements Model (Ref. Attachment 0005) that may occur during the consultation session. b.�� �UNACCEPTABLE: In order to be determined unacceptable, a Bio was NOT IAW paragraphs 1.6.11.2 and 5.2.16 of the PWS, Attachment 0001 and did NOT demonstrate 3+ years of leadership coaching experience. Experience did NOT identify type of customer, public or private sector customers. The vendor did NOT submit a proposed agenda/syllabus that outlines the discussion topics regarding the Army Leadership Requirements Model (Ref. Attachment 0005) that may occur during the consultation session.� 4.�� �Sub-Factor: Period of Performance (timeline) a.�� �ACCEPTABLE: To be determined acceptable, the vendor confirms they are not taking any exceptions to the Government�s (Agency) timeline to complete the leader assessment project within 90 days from the start of the period of performance. Confirmation shall be made by marking �Y� to indicate �yes� on attachment 0003-Price Matrix.� b.�� �UNACCEPTABLE: To be determined unacceptable, the vendor takes exception to the Government�s (Agency) timeline to complete the leader assessment project within 90 days from the start of the period of performance. By marking �N� indicates �no� on Attachment 0003-Price Matrix. NOTE: In order to be considered further for price evaluation, an offeror�s quote submission shall be determined technically acceptable for all technical sub-factors.� C.1.2. PRICE:� The Government will determine the lowest price based on the calculation of the Total Evaluated Price submitted on the Price Matrix, Attachment 0003. The FAR requires that contracts be awarded at prices that are fair and reasonable. The total evaluated price shall be provided in the Price Matrix, Attachment 0003. The Price Matrix shall be evaluated by the total evaluated price calculated (quantity * unit price= Total CLIN price = Total Evaluated Price) on the submitted Price Matrix, Attachment 0003. The Government will make a fair and reasonable price determination IAW FAR 13.106-3.� The Government reserves the right to require submission of data other than certified cost or pricing data to determine the reasonableness of a quote if required. Failure to provide prices for all items/options is unacceptable and may render ineligible for award.� The Government will use price analysis to determine price reasonableness. In the event price analysis cannot determine the proposed prices fair and reasonable, cost quotes will be required. The Contracting Officer may use additional analysis techniques as determined necessary. These methods of evaluation may include the use of information/input from sources such as (but not limited to) other Government agencies and personnel. D. BASIS FOR AWARD:� The Government intends to make a single award to the vendor that is determined to be the lowest priced technically acceptable quote, is determined responsible IAW FAR Part 9, and whose quotation is found responsive with the terms of this RFQ.� The Government will determine responsibility by determining compliance with the requirements of FAR 9.1. This shall include but shall not be limited to a review of Federal Awardee Performance and Integrity Information System (FAPIIS) including information that is linked to FAPIIS, such as from the System for Award Management Exclusions. E. EXCHANGES� It is the intention of the Government to award without exchanges, but the Government reserves the right to enter into clarifications, communications, or exchanges if determined to be in the best interest of the Government. The Government need not enter into exchanges with all Vendors. Clarifications are defined as limited exchanges between the Government and the Vendors that may occur when award without communications is contemplated. It is the intention of the Government to award without meaningful exchanges with the quoter, but the Government reserves the right to conduct meaningful exchanges if determined to be in the best interest of the Government.� F. DELIVERY, INSPECTION AND ACCEPTANCE:� Period of Performance: � � � � � � � �Origin Inspection and Acceptance: � � � � Destination Further Period of Performance, Inspection and Acceptance Information will be provided at time of award. As annotated in sections 1.6.5 and 1.6.1 PWS, Attachment 0001 JMC Leader Assessment Project, the work to be performed under this contract shall be performed at the Vendor�s location. All work will be communicated to the Agency via electronic media, i.e., telephone, email, virtual transmission media (WebEx, MS Teams, etc.) G. QUESTIONS: It is requested submission of questions reference the particular section/subsection of the RFQ. Answers to submitted questions may be provided to all Vendors as an amendment to the RFQ. Therefore, questions/comments shall not be marked with a restrictive legend and shall not include proprietary information. The identity of the person/company asking the question will not be disclosed. The Government is not obligated to provide responses to all questions. Questions shall be submitted in writing by 4:00PM Central Time, June 5, 2023. H. ACKNOWLEDGEMENT OF RFQ AMENDMENT(S): Should an amendment be issued against the RFQ, the Vendor shall acknowledge each amendment. Acknowledgement of all amendments shall be made part of the quotation submittal. For each amendment issued, an attachment will be provided with the amendment for the contractor to sign and submit with its quotation submittal. I. DUE DATE AND TIME FOR QUOTE SUBMISSION The due date and time for quotation submission is 4:00PM Central Time on June 7, 2023. Submissions shall be submitted electronically via email to the POCs listed in Section L of this RFQ. J. TERMS AND CONDITIONS Quotes submitted in response to this RFQ shall remain valid for a period of 60 days. K. PROVISIONS AND CLAUSES Refer to Attachment 0002 for a list of provisions and clauses that pertain to this requirement. Note: IAW Paragrah B.2.2.
