The maintenance of fitness center equipment is needed so that users will be able to safely use well-maintained exercise equipment. The Buckley Fitness Center has received new LifeFitness strength and cardio equipment. Most of the large equipment is through LifeFitness; however, we have utilized several different makes and models of fitness equipment for physical training and rehabilitation in the past. This equipment includes electrically powered treadmills, recumbent bikes, upright bikes, assault bikes, stair climbers, ellipticals, and numerous and various pieces of strength equipment both selectorized/circuit and plate loaded machines. This equipment requires periodic maintenance and service that is beyond the ability of personnel at The Fitness Center. �As such, The Fitness Center requires a maintenance contract to perform maintenance and repair of this equipment. 06 JUN 2023 Performance Work Statement (PWS) 1.0 BACKGROUND 1.1 The maintenance of fitness center equipment is needed so that users will be able to safely use well-maintained exercise equipment. The Buckley Fitness Center has received new LifeFitness strength and cardio equipment. Most of the large equipment is through LifeFitness; however, we have utilized several different makes and models of fitness equipment for physical training and rehabilitation in the past. This equipment includes electrically powered treadmills, recumbent bikes, upright bikes, assault bikes, stair climbers, ellipticals, and numerous and various pieces of strength equipment both selectorized/circuit and plate loaded machines. This equipment requires periodic maintenance and service that is beyond the ability of personnel at The Fitness Center. As such, The Fitness Center requires a maintenance contract to perform maintenance and repair of this equipment. 2.0 OBJECTIVES 2.1 Establish an annual (12 month) maintenance contract with reoccurring service every three months after contract start. This contract to is maintain gym equipment in compliance of the manufacturer�s recommendation. After each service visit, a report logging the hours on site and maintenance done is required. 3.0 SCOPE 3.1 The contractor shall provide all management, supplies, equipment, and labor necessary for the repairs, on-call maintenance, and quarterly preventative maintenance for the government-owned exercise equipment at the Buckley Space Force Base (SFB) Fitness Center equipment in Aurora, Colorado. 2 3.2 The contractor shall provide all personnel, tools, parts, supervision, and other items necessary to perform preventative maintenance and repairs to include inspecting and testing of the equipment and service according to the Original Equipment Manufacturers recommendations. This service is required quarterly and includes lubricating, calibrating, adjusting, internal cleaning and repairs as required, as necessary. Task orders for quarterly preventative maintenance will be issued annually. Any additional repairs and service calls will be issued via individual task orders. The equipment to be maintained will be treadmills (12), cross trainers (2), stationary bikes (3 upright/3 recumbent), 13 spin bikes, 1 large (MJLPD-STA)/1 small cable machine, stair climbers (6), manual treadmills (3), squat stations (6), std/adj floor benches (11), strength equipment selectorized/circuit machines (17), strength equipment free weight machines (35). These numbers are subject to change based off retiring of equipment and receiving replacement equipment. 4.0 REQUIREMENTS 4.1 Quarterly Maintenance - The Contractor shall provide quarterly maintenance every three months after the contract start date and provide troubleshooting services on equipment listed in Attachment 1 Equipment Model List. Maintenance and repair of equipment shall meet the manufacturer�s recommendations. Contractor/personnel shall be qualified/certified service providers as applicable for all equipment. The contractor shall perform quarterly preventative maintenance as follows: 1. Ensure all fitness center equipment has received quarterly preventative maintenance in accordance with the manufacturer's recommendations. 2. Ensure all fitness center equipment is in good working condition, free of defects and operates in a safe manner. 3. Ensure all fitness center equipment is clean and free of dust and grease build-up. These services shall include but are not limited to (please also refer to Section 3.0): 4.1.1 Inspection, cleaning, and lubrication of equipment 4.1.2 Calibrate equipment when required 4.1.3 Troubleshooting of malfunctioning treadmills making recommendation of parts to purchase to facilitate repairs (which shall be added via separate Task Order). 4.1.4 Service report of all work performed and suggested repairs which are outside the scope of the quarterly maintenance. 5.0 CONTRACTOR MANPOWER REPORTING 5.1 The contractor shall report ALL contractor labor hours (including subcontractor labor 3 hours) required for performance of services provided. At the completion of each site visit, complete a locally prepared Preventative Maintenance Service Report Form/Inspection Performed Checklist (Attachment 2) and provide it to one of the government points of contact. For any repairs found outside the preventative maintenance, the contractor shall provide a formal written quote to the Government Point of Contact and shall not purchase parts or perform work without prior approval from a Contracting Officer. The quote shall include the cost for parts and labor and the estimated time frame to complete the repair. 6.0 PERIOD OF PERFORMANCE: 12 months 7.0 CONTRACTOR TRAVEL 7.1 Any travel associated with performance of these services shall be included within the Firm Fixed Price of the service. No travel CLIN will be included. 8.0 SECURITY 8.1 Contractor shall establish appropriate administrative, technical, and physical safeguards to protect all nonpublic Government data to ensure the confidentiality, integrity, and availability of Government data. 8.2 All contractor personnel are required to be citizens of the United States. 9.0 QUALITY CONTROL 9.1 The contractor shall implement and maintain a Quality Control Plan (QCP) to ensure work performed conforms to the scope of work and meets the requirements under this PWS. The QCP shall, at a minimum provide a method for performing inspections. identifying, correcting, and preventing problems/defective service; addressing customer complaints, and improving the quality of services over the life of the contract. 10.0 PLACE OF PERFORMANCE Buckley SFB Fitness Center 17890 E.
