NOTE: In addition to completing page 1 and 2 of this RFQ, please also include your quote on company letterhead to confirm all items, quantities, and FOB delivery destination. Government will make award of a firm fixed price purchase order to the contractor who is lowest priced and technically acceptable per the specifications.
