Sources Sought Notice Page 2 of 2 DESCRIPTION This is a Request for Information Only; it is not a request for quotations or offers. Network-9 Contracting Office on behalf of the James H. Quillen VAMC, Mountain Home, TN 37684 has developed this Request for Information (RFI)/Sources Sought Notice to solicit market research information from industry to assist the Government with analysis of a Government requirement and provide commercial insight on performance-based solutions, best practices, possible contractual arrangements, and potential issues. This RFI is to determine the availability of Small Business (preferably Service-Disabled and/or Veteran-Owned Owned) entities within the Nashville and Murfreesboro area. THIS IS A REQUEST FOR INFORMATION James H. Quillen VAMC, Mountain Home, TN 37684ONLY. It is not a Request for Proposal, a Request for Quotation, an Invitation for Bids, a Solicitation, or an indication that Network-9 Contracting Office (NCO-9) will contract for the items contained in the RFI / Sources Sought Notice. This RFI/Sources Sought Notice is part of a Government market research effort to determine the scope of industry capabilities and interest, in accordance with Federal Acquisition Regulation (FAR) Part 15.201(e), response to this RFI / Sources Sought Notice are not offers and cannot be accepted by the Government to form a binding contract. Responses to this RFI / Sources Sought Notice are strictly voluntary and are considered as information only. The Government will not pay respondents for information provided in response to this RFI / Sources Sought Notice. Responses to this RFI / Sources Sought Notice will not be returned and respondents will not be notified of the result of the review. If a solicitation is issued, it will be announced at a later date, and all interested parties must respond to that solicitation announcement separately from any response to this announcement. This RFI / Sources Sought Notice does not restrict the Government s acquisition on a future solicitation. STATEMENT OF WORK Mold Remediation Bldg. 34 Attic Please correct title PART A GENERAL INFORMATION A.1 INTRODUCTION The contractor shall provide all labor, materials, supervision, and transportation needed to conduct the mold remediation for Building 34 attic at James H. Quillen VAMC, Mountain Home, TN 37684. A.2 BACKGROUND Building 34 is undergoing a renovation and as part of the renovation repairs were made to the roofing system. However, it was discovered that the attic has sustained water intrusion that resulted in a significant mold growth that covered the entire attic space above the previous research lab area. The roof has been repaired and the resulting mold will need to be remediated. A.3 SCOPE OF WORK The vendor shall provide all labor, materials, supervision, and transportation for access, removal, cleanup, disposal, and visual inspection of the attic in Building 34 PART B WORK REQUIREMENTS B.1 Provide all materials, labor, transportation, and supervision for tasks to be accomplished: Provide and install plywood floor decking to be used as a working surface for the remediation crew. Provide mold remediation in the attic of Building 34 Provide critical barriers Provide a decontamination unit for access to and from the work area Decontaminate decking, columns, and roof joists using HEPA vacuum and microbial disinfectant Apply mold encapsulant to attic components Final visual inspection of the area once remediation is completed. Transport and dispose of mold-contaminated materials. Comply with all applicable OSHA/EPA requirements. All materials and labor shall be furnished by the contractor/vendor. PART C SUPPORTING INFORMATION C.1 Place of Performance Department of Veterans Affairs Medical Center, Mountain Home, Sidney & Lamont St., Building 34, Mt. Home, Tennessee 37684 C.2 Period of Performance Work shall be accomplished during regular business hours 7 a.m. to 4:30 p.m. Monday through Friday, except for Federal Holidays, unless approved by Contracting Officer or COR. U.S. Legal Holidays: The following is a list of U.S. Government holidays. Any other day not listed, but specifically declared by the President of the United States to be a national holiday shall be observed. If the holiday falls on a Saturday, the proceeding Friday is observed as the holiday; if the holiday falls on a Sunday, the following Monday is observed as the holiday. Work shall be scheduled for proper execution to completion of this contract. C.3 Special Considerations: n/a C.3.1 Contractor Furnished Materials The contractor shall provide all services, labor, and associated materials to conduct mold remediation. C.3.2 Government Furnished Materials and Services: None C.3.3 Qualifications of Key Personnel Personnel shall be certified to conduct mold remediation. C.3.4 Security Requirements see VAMC contact for Security clearance. The contractor shall identify which employees will be working under the awarded contract. Contractor personnel shall include employees, sub-contractors, subcontractor employees, suppliers, and delivery personnel entering the Medical Center. This does not apply to suppliers and delivery personnel making deliveries to the VA warehouse dock only. C.3.5 The Contracting Officer's Representative (COR) is responsible for the inspection of the work. The Contracting Officer's Representative will be identified by name, telephone number, and location. Inspections made by the COR are for the sole benefit of the Government and do not relieve the Contractor of any quality control responsibilities. C.3.6 Smoking Policy. Smoking is prohibited on VA property. C.3.7 Parking/Traffic Regulations. Parking Permit Request form must be completed if required. The Contractor shall not park on grassy areas unless approved by the COR and the Contractor agrees to restore areas to VA standards. Drivers should be particularly concerned with pedestrian traffic. Yield to pedestrians in crosswalks. Posted speed limits and all other traffic controls are to be observed by operators at all times. Seat belt use is mandatory on the VA grounds. C.3.8 Working Conditions/Safety All work shall be performed in accordance with Mt. Home VAMC rules & regulations. The contractor/vendor is solely responsible for working conditions on the job site, including the safety of all persons and property during the performance of work, and for compliance with OSHA regulations. If interested, please provide a capability statement. Also, provide e-mail responses to this Request for Information (RFI)/Sources Sought Notice no later than June 30, 2023.
