STATEMENT OF WORK Concrete repair/replacement SIOUX FALLS VA REGIONAL OFFICE 2501 W. 22nd Street Sioux Falls, SD 57105 1 Scope 1 2.1 Non-Personal Services 2 2.2 Business Relations 2 2.3.1 Contract Management 2 2.3.2 Contract Administration 2 2.4 Location and Hours of Work 2 3 Performance Requirements 3 4 Special Requirements 4 4.1 Security and Safety 4 4.2 Government Furnished Materials 4 4.3 Quality 5 4.3.1 Quality Control 5 4.3.2 Quality Assurance Surveillance Plan (QASP) 5 5 Related Documents 5 Related Images .. 6 1 Scope The existing concrete slabs have been cracked and/or damaged due to age, wear, and use.  There is concrete on the South side of the VARO which will require the demolition, removal, and replacement of approximately 22.5 cubic yards of damaged and damaged-adjacent concrete slabs. The project s work will include: -Erecting safety barricades -Removing old bollards -Removing old concrete -Dispose of old concrete -Removing tree FboNotices and other obstructions from under the slab -Constructing form work for pouring concrete -Pouring concrete including curbs and gutters -Level concrete surfaces flush with the existing concrete on all ends -Finishing the concrete to a smooth surface consistent with surrounding concrete slabs -Providing new bollards -Limited tuck pointing and repair of bricks in the dumpster screen -Replacement of limited metal flashing on the top of the brick dumpster screen -Properly removing and disposing of the construction debris -Removing barricades when complete -Clean construction area of all trash and debris 2.1 Non-Personal Services The Government shall neither supervise contractor employees nor control the method by which the contractor performs the required tasks. Under no circumstances shall the Government assign tasks to, or prepare work schedules for, individual contractor employees. It shall be the responsibility of the contractor to manage its employees and to guard against any actions that are of the nature of personal services or give the perception of personal services. If the contractor believes that any actions constitute, or are perceived to constitute personal services, it shall be the contractor's responsibility to notify the Contracting Officer (CO) immediately. 2.2 Business Relations The contractor shall successfully integrate and coordinate all activity needed to execute the requirement. The contractor shall manage the timeliness, completeness, and quality of problem identification. The contractor shall provide corrective action plans, proposal submittals, timely identification of issues, and effective management of subcontractors. The contractor shall seek to ensure customer satisfaction and professional and ethical behavior of all contractor personnel. 2.3.1 Contract Management The contractor shall establish clear organizational lines of authority and responsibility to ensure effective management of the resources assigned to the requirement. The contractor must maintain continuity between the support operations at 2501 W 22nd Street, Building 38, Sioux Falls, SD 57105, and the contractor's corporate offices. 2.3.2 Contract Administration The contractor shall establish processes and assign appropriate resources to effectively administer the requirement. The contractor shall respond to Government requests for contractual actions in a timely fashion. The contractor shall have a single point of contact between the Government and Contractor personnel assigned to support contracts or task orders. The contractor will be working with the assigned COR. The contractor shall assign work effort and maintaining proper and accurate time keeping and payroll records of personnel assigned to work on the requirement. 2.4 Location and Hours of Work Accomplishment of the results contained in this SOW requires work at 2501 W 22nd Street, Building 38, Sioux Falls, SD, 57105. Normal workdays are Monday through Friday, except US Federal Holidays. Workers typically work eight (8) hours per day, 40 hours per week. The bulk of the work performed will be done during normal business hours. Some of the work that impacts employees working creating excessive noise such as any grinding needed around Building 38 may need to be coordinated and scheduled over weekends and after normal business hours of 6am to 6pm. 3 Performance Requirements The following section specifies the Performance Objectives and Performance Elements for the contract. 3.1 Performance Period The contractor shall complete the work required in this SOW as directed by the contracting officer (CO). If the contractor proposes an earlier completion date, and the Government accepts the contractor s proposal, the contractor s proposed completion date shall prevail.  Work at the Government site shall not take place on Federal holidays or weekends unless approved by the COR/owner. Contractor shall provide a schedule for the project and shall update the schedule at least every two weeks 3.2.1 Prebid site investigation Contractor may contact owner to see the site before submitting a bid. 3.2.2 OSHA Safety Training. All contractors must have completed OSHA safety training. Performance Standards a) STD: 30hrs of training for supervisors and 10 hrs. for laborers     AQL: 100% 3.2.3 Obtain all materials and permits. Contractor will ensure that all materials necessary to perform this statement of work are on hand prior to commencement of work. The contactor will also ensure that all work permits, licenses, bonds, accident prevention plan, scheduled of products used (MDS) are on hand prior to beginning construction. Performance Standards a) STD: Compliant with all federal, state and county laws and regulations.     AQL: 100% 3.2.4 Shop drawings Contractor shall provide a shop drawing for the concrete and rebar for approval by the owner before any concrete is poured. 3.2.5 Construction barriers and existing planters Contractor shall provide construction barriers around the job site to prevent unwanted access that could cause injury. Barriers to be metal fencing. No orange construction fence is allowed. After work has been completed the contractor shall remove the barriers. 3.2.6 Removal of the concrete All bulk waste will be disposed of properly by the contractor in accordance with all applicable laws and regulations. 3.2.7 Staging of dumpster and misc. tools and equipment. Contractor will coordinate with the COR for placement of dumpster and any equipment on job site. 3.2.8 Demolition & Construction The contractor will provide all necessary materials, tools, support equipment, expertise and labor to perform the removal and replacement of the concrete at the Sioux Falls VA Regional Office. Contractor shall cut and remove any tree FboNotices or any other obstructions under the new concrete. New concrete shall be approximately 22.5 cubic yards of M6 concrete, 6 thick, with 3 compacted granular base, 4000 psi, #4 rebar 24 center on center, doweled to adjacent concrete. Also, to be included are two (2) curbs, approximately 31 feet each, two (2) standard 4 steel bollards filled with concrete and painted, two (2) removable steel bollards with locks placed 4 apart, and tuck pointing/repair of approximately 10 bricks in a 3 x3 area. One (1) flashing approximately 36 x12 will be placed on the top of the brick wall where the bricks are being repaired. Contractor shall take precautions to locate and protect all underground utilities in the construction area. Owner will provide to the contractor any drawings that show the estimated location of existing underground utilities but cannot guarantee the accuracy of these drawings. Contractor must locate and protect all underground utilities before any work begins. 4 Special Requirements The bulk of the work performed will be done during normal business hours. Some of the work that impact employees working may need to be coordinated and scheduled over weekends and after normal business hours of 6am to 6pm. 4.1 Security and Safety Equipment - The Contractor shall provide all safety equipment/devices, MDS, personal protective equipment and clothing as required for its employees.  Copies of all MDS shall be provided to the COR upon request. Compliance - The Contractor shall comply with all applicable Federal, State and local legal requirements regarding workers health and safety.  The requirements include but are not limited to, those found in Federal and State Occupational Safety and Health Act (OSHA) statutes and regulations, such as applicable provisions of Title 29, Code of Federal Regulations (CFR) Parts 1910 and 1926.  Contractor is solely responsible for determining the legal requirements that apply to activities, and shall ensure safe and healthful working conditions for its employees During normal business hours (6am-6pm) the Contractor is required to enter and exit the Sioux Falls VARO through the front entrance if required to sign in.  Entering or exiting the building through any other door will require VARO approval and an escort will be required.  Upon entering the building, the contractor and/or contractor employees will be required to show proof of identity (must have a valid photo ID) as well as pass through security screening. 4.2 Government Furnished Materials The Government will make available space for tools and equipment while working on this project. 4.3 Quality This section describes the Quality Control components for this effort. The following sub-sections provide details of various considerations on this effort. 4.3.1 Quality Control The Contractor shall develop a contract Quality Control Plan (QCP) and maintain an effective quality control program to ensure services are performed in accordance with this SOW.  The Contractor shall develop and implement procedures to identify, prevent, and ensure non-recurrence of defective services.  The Contractors QCP is the means by which he/she assures themself that the work complies with the requirement of the contract.   The finalized QCP will be accepted by the Government at the time of the award of the contract.  The Contracting Officer may notify the Contractor of required modifications to the plan during the period of performance.  The Contractor then shall coordinate suggested modifications and obtain acceptance of the plan by the Contracting Officer.  Any modifications to the program during the period of performance shall be provided to the Contracting Officer for review no later than 10 working days prior to effective date of the change. The QCP shall be subject to the Governments review and approval.  The Government may find the QCP ""unacceptable"" whenever the Contractors procedures do not accomplish quality control objective(s).  The Contractor shall revise the QCP within 10 working days from receipt of notice that QCP is found ""unacceptable."" 4.3.2 Quality Assurance Surveillance Plan (QASP) The Government shall monitor the Contractors performance under this contract in accordance with the Governments QASP. 5 Related Documents The following Document(s) are related to this project: -Regional Office site plans concrete repair (all to be provided at the site visit) -Existing plans showing the best estimation of where underground utilities are located.
