- Statement of Work Project Name: Developing Tools for Dialysis Decision Support 1.0 Project Scope / Background: Services proposed on this contract are in support of developing a web-based dialysis decision aid/guide - The Kidney Roadmap to support informed decision-making and goal-concordant care for Veterans with advanced chronic kidney disease (CKD). The contractor is needed to complete web design and development of the decision aid taking advantage of the scope offered by the web (e.g., animation, interaction, stock photos), and publish a functional prototype. The contractor will develop the wireframes/logic flowchart and logical branching paths with outcomes incorporating stock art. web modules comprised of content, link-outs, and will be responsible for the tool maintenance. 2.0 Required Experience / Qualifications: 2.1 The contractor must have at least 4 years of experience in training design, development, and/or delivery. 2.2 They must have experience building behavioral interventions and the unique needs of these users. 2.3 They must possess prior experience working with the Department of Veteran Affairs and the Veteran population. 2.4 They must be familiar with the regulations of the Department of Veterans Affairs. 2.5 They must have broadband internet, necessary hardware (e.g., personal computers) and be proficient with software, such as Microsoft Word and Moodle. 2.6 They must demonstrate that the organization has the time, resources, and facilities to conduct and carry out this project in the time allotted, including a sufficient number of experienced staff to complete the work and project management skills. 3.0 Specific Tasks: The contractor will be responsible for: 3.1 Collaborating with VA/Stanford Subject Matter Experts (SMEs) to design, develop, and publish a functional prototype of the Dialysis Decision Support tool, The Kidney Roadmap . 3.2 Working with VA/Stanford SMEs to develop wireframes/logic flowchart which may lead up to 5 outcomes. 3.3 Providing branding and design templates for the tool. 3.4 Building a functional prototype based on approved templates and approved wireframes/logic flow. The functional prototype will incorporate stock art (icons, photos, and images) identified by the contractor and VA/Stanford SMEs. 3.5 Performing functional testing of the prototype to validate the branching logic. 3.6 Maintaining open communication with the study team throughout project and contact Ci2i personnel promptly if a problem arises. 3.7 Meeting with study team as needed to provide technical support, for instance, to troubleshoot log-in issues or difficulties with accessing through different devices and operating systems. 3.8 Incorporate actionable feedback and modifications to the tool based on peer review collected by VA/Stanford SME. 3.9 Assisting study team with providing data regarding analytics and usage statistics. 4.0 Deliverables: 4.1 Publish a functional prototype for pilot testing within 6 months of the beginning of the contract. 4.2 Meet monthly and as needed with study personnel and investigator to review work completed and review deliverables and delivery dates. Meetings may be conducted virtually. 4.3 Maintain the decision tool and ensure that is operable for the duration of the contract. 4.4 Provide hosting at no additional cost for 12 months after the completion of the project. 4.4 All published versions of decision tool are work products of VA and source files will be provided to study team at completion of the contract to enable VA/Stanford team to take over hosting of tool. 5.0 Estimated Level of Effort: 5.1 The anticipated level of effort for this task order is Labor Categories Qty Senior Program Manager 240 Web Specialist 840 *Instructional Technologist 240 *Graphic Designer 88 6.0 Payment Method: Send Electronic invoicing through Tungsten website to the Department of Veterans Affairs Financial Service Center, Austin, TX. Direct Deposit as set up through Austin Payment will be requested monthly by Individual according to the schedule of their work completion. 7.0 Task Manager Quality Assurance Monitoring: 7.1 Task manager (Dr. Tamura), project staff, and selected contractor shall meet at least once every month or more frequently as needed to review work completed and review delivery dates. Meetings may be conducted virtually. 7.2 Weekly status reports outlining progress in terms of stated material development goals will be sent to Dr. Tamura regarding any new content. 8.0 Place of Performance: 8.1 All work will take place remotely from vendor s place of business. As this work does not involve any direct interaction with patients or study participants, it can be completed remotely. 9.0 Space furnished: N/A working remotely 10.0 Access to VA computer: No 11.0 Contractor access to PII or PHI: No
