(i)�� This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in subpart 12.6 of the Federal Acquisition Regulation, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii)� This solicitation is issued as a Request for Quotation (RFQ). (iii)� The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2023-04. (iv)� This procurement is set aside for small business.� The NAICS code is 562991and the Small Business Size Standard is $9 Million. Estimated Usage: 5-30 personnel at any time during the performance period. In Accordance with FAR 52.212-4(c) Changes in the terms and conditions of this contract may be made only by written agreement of the parties. Government may order increase PP/HW or cleaning service if needed due to increased usage, any increase or decrease in services/ordering must have a General Message issued through dispatch to the Contracting Officer Representative for communication with the Contractor. Government must provide the Contracting Officer a General Message to request a modification to extend the performance period. One award will be made from this solicitation. (vi) Description of Requirement (including a list of any attachments) GENERAL: This is a non-personal services contract to initially provide two (2 ea.) portable toilet units and 1 handwashing station, with one (1) time weekly servicing.� The Government shall not exercise any supervision or control over the contract service providers performing the services herein.� Such contract service providers shall be accountable solely to the Contractor who, in turn is responsible to the Government. 1.1 Description of Services: The contractor shall provide all personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, and other items and non-personal services necessary to provide portable toilets and handwash stations in support of aviation incident operations at Dallesport � Columbia Gorge Regional Airport for the Mt. Hood & Gifford Pinchot National Forests. �The contractor shall perform to the standards in this contract. 1.2 Background: USDA FOREST SERVICE requires portable toilets and handwashing stations with service for Incident Support during the 2023 Wildland Fire season.� The portable toilets will be positioned in the weather at the airport to provide restroom facilities for project personnel. �The contract shall also include delivery, setup, servicing, and retrieval. �Throughout this specification, the portable toilets, will be referred to as �PP/HW�. 1.3 Objectives: Provide PP/HW to supply restroom facilities for project personnel. 1.4 Scope: Provide portable toilet and handwash service at Dallesport � Columbia Gorge Regional Airport.� Initial Requirement is: At a Minimum two (2) PP, and one (1) HW shall be provided at site 1 for the ordering period date of contract award through October 31, 2023. Site 2 � optional � request when staffing levels rise; 2 porta potties and 1 hand wash to be placed at the South/East Ramp. Delivery Location(s): Site 1: North end of airport � 2 portable toilets and 1 hand wash station Site 2 (Optional � dependent on staffing levels/aviation resources): South/East Ramp � 2 portable toilets and 1 hand wash station 1.5 Accidents: Report any accident within 24 hours from occurrence to the COR. (Deliverable 01) 1.6.����� Contractor Work Plan: The contractor shall submit a work plan for acceptance to the COR within 14 calendar days of award. (Deliverable 02). At a minimum the work plan shall include: 1) Key Personnel. 2) Safety Plan. 4) Fire Plan. 5) Contractor�s Quality Control Plan 1.7 Contractor�s Key Personnel The Contractor shall provide a Contract Manager and designated alternate, as applicable, who has full authority to act for the Contractor on all contract matters relating to this contract. The Contract Manager shall be available via telephone and email. The Contractor shall submit the name and contact information of the contract manager in writing to the COR within 14 calendar days of award. (Deliverable 03) 1.8 Quality Control: The contractor shall develop and maintain an effective quality control program to ensure services are performed in accordance with this PWS. �The contractor shall develop and implement procedures to identify, prevent, and ensure non-recurrence of defective services. �The contractor�s quality control program is the means by which he/she assures their selves that work complies with the requirement of the contract. Submit with quote a quality control plan (QCP). �After acceptance of the quality control plan the contractor shall receive the contracting officer�s acceptance in writing of any proposed change to his QC system. 1.8.1 Quality Assurance: The government shall evaluate the contractor�s performance under this contract in accordance with the Quality Assurance Surveillance Plan. �This plan is primarily focused on what the Government must do to ensure that the contractor has performed in accordance with the performance standards. �It defines how the performance standards will be applied, the frequency of surveillance, and the minimum acceptable defect rate(s). 1.8.2 Standard: Toilets will be clean and stocked with toilet paper. Handwash facility will be clean. Water shall be operational. L. 115-232) prohibits the head of an executive agency on or after August 13, 2019, from procuring or obtaining, or extending or renewing a contract to procure or obtain, any equipment, system, or service that uses covered telecommunications equipment or services as a substantial or essential component of any system, or as critical technology as part of any system. Nothing in the prohibition shall be construed to� ��������������� (i) Prohibit the head of an executive agency from procuring with an entity to provide a service that connects to the facilities of a third-party, such as backhaul, roaming, or interconnection arrangements; or ��������������� (ii) Cover telecommunications equipment that cannot route or redirect user data traffic or cannot permit visibility into any user data or packets that such equipment transmits or otherwise handles. ���������� (2) Section 889(a)(1)(B) of the John S. McCain National Defense Authorization Act for Fiscal Year 2019 (Pub. L. 115-232) prohibits the head of an executive agency on or after August 13, 2020, from entering into a contract or extending or renewing a contract with an entity that uses any equipment, system, or service that uses covered telecommunications equipment or services as a substantial or essential component of any system, or as critical technology as part of any system. This prohibition applies to the use of covered telecommunications equipment or services, regardless of whether that use is in performance of work under a Federal contract. Nothing in the prohibition shall be construed to� ��������������� (i) Prohibit the head of an executive agency from procuring with an entity to provide a service that connects to the facilities of a third-party, such as backhaul, roaming, or interconnection arrangements; or ��������������� (ii) Cover telecommunications equipment that cannot route or redirect user data traffic or cannot permit visibility into any user data or packets that such equipment transmits or otherwise handles. ����� (c) Procedures. The Offeror shall review the list of excluded parties in the System for Award Management (SAM) (https://www.sam.gov) for entities excluded from receiving federal awards for ""covered telecommunications equipment or services"". ����� (d) Representation.
