- DEPARTMENT OF VETERANS AFFAIRS Veterans Health Administration Consolidated Mail Outpatient Pharmacy (CMOP) 3450 S. 4th Street Leavenworth, KS 66048 Page 8 of 8 The Department of Veterans Affairs is seeking interested sources for the requirement entitled: CONSOLIDATED MAIL OUTPATIENT PHARMACY (CMOP) DOMESTIC PHARMACEUTICALS This is a Sources Sought announcement only this is not a Request for Proposal. The purpose of this notice is to conduct market research and obtain information from qualified contractors with special interest. No solicitation is being issued at this time. This notice shall not be construed as a commitment by the Government to ultimately award a contract, nor does it restrict the Government to a particular acquisition approach. The Government has established an ongoing requirement for the provision of domestically manufactured pharmaceuticals. In accordance with Federal Acquisition Regulation (FAR) part 15.201(c), the Agency is encouraged to promote early exchanges of information about future acquisitions. An early exchange of information in the acquisition process can identify and resolve concerns regarding the acquisition strategy, including proposed contract type, terms and conditions, and acquisition planning schedules; the feasibility of the requirement, including performance requirements, statements of work, and data requirements; the suitability of the proposal instructions and evaluation criteria, including the approach for assessing past performance information; the availability of reference documents; and any other industry concerns or questions. The Consolidated Mail Outpatient Pharmacy program (CMOP) is seeking potential small and large business domestic pharmaceutical manufacturer sources capable of providing the following pharmaceuticals: Glucose test strips & lancets 33G used to test blood sugar (Brand Name: One Touch, Contour, & Accu-Chek; common box count: 50CT, 100CT) HCTZ 12.5MG / Losartan used to lower high blood pressure (common strengths: 100MG, 50MG; common bottle counts: 90CT, 1000CT) Clomiphene Citrate used to induce ovulation (egg production) (common strength: 50MG; common bottle count: 30CT) Diltiazem HCL used to lower high blood pressure (common strengths: 60MG, 90MG, 120MG; common bottle count: 100CT) Timolol Maleate 0.5% - used to treat high pressure inside the eye due to glaucoma or other eye disease (common bottle: solution 5ML, Gel) Azathioprine - for the prevention of rejection in renal homotransplantation (common strength: 50MG; common bottle count: 100CT) Acamprosate CA EC TAB used to help overcome alcoholism (common strength: 333MG; common bottle count: 180CT) Polyvinyl Alcohol 1.4% Lubricating Eye Drops used to relieve dry, irritated eyes (common strength: 1.4%; common bottle count: 15ML) Clindamycin 1% topical solution used to treat acne lesions (common strength: 1.4%; common bottle count: 60CT) Rizatriptan Benzoate used to treat the symptoms of migraine headaches (severe, throbbing headaches that sometimes are accompanied by nausea and sensitivity to sound and light) (common strength: 5MG & 10MG Disintegrate, 5MG & 10MG Tab; common bottle count: 9CT, 12CT, 18CT) Lansoprazole treatment of ulcers (common strength: 15MG & 30MG Disintegrate; common bottle count: 100CT) Abaloparatide used for those who suffer from high risk osteoporosis (common strength: 80MCG; common bottle count: 30CT) Varenicline a nicotinic receptor partial agonist indicated for use as an aid to smoking cessation treatment (common strength: 1MG; common bottle count: 56CT) Camphor/Menthol/Methyl Salicylate Patch - temporary relief of minor aches and pains of muscles and joints associated with arthritis, simple backache, strains, bruises, and sprains (common box count: 60 patches) Tandem Autosoft 90 insulin infusion set (brand name) used to administer insulin injections (common box count: 10CT) Potential contractor(s) will manufacture the aforementioned pharmaceuticals domestically for an estimated seven facilities and will be responsible for providing all labor, materials, quality assurance, delivery and supervision in compliance with federal, state and local regulations. The North American Industry Classification System (NAICS) code associated with this request for information is: 325412, which has a business size standard of 1,250 employees. Interested manufacturers are reminded that in accordance with FAR 4.12, prospective contractors shall complete electronic annual representations and certifications in conjunction with FAR 4.11 required registration in the System for Award Management (SAM) database prior to award of a contract. The Government requests responders provide the following information: Company information including name, address, point of contact, identification of any socioeconomic categories (i.e. SDVOSB, VOSB, 8a, etc) the company falls under, business size and DUNS Number The pharmaceutical(s) which are being manufactured domestically Authorized distributors who distribute the domestically manufactured pharmaceutical(s) Average cost of common prescription quantities In the last twelve (12) months how many times has the stock not been available for shipment and how quickly are pharmaceuticals restocked The contract number of any applicable GSA/FSS/NAC schedule if one exists from which these items can be procured under Further, if you believe a different NAICS code or Product Service Code (PSC) is a better fit for the subject requirement, please provide that information to us as well PLEASE NOTE: This RFI is issued solely for information and planning purposes only and does not constitute a solicitation. All information received in response to this RFI that is marked as proprietary will be handled accordingly. In accordance with FAR 15.201(e), responses to this notice are not offers and cannot be accepted by the Government to form a binding contract.
