- P11 RFQ 36C24623Q1017 SOURCES SOUGHT VA CARES PGCSS DURHAM VAMC DISCLAIMER THIS NOTICE IS FOR PLANNING PURPOSES ONLY. THE GOVERNMENT DOES NOT INTEND TO AWARD A CONTRACT ON THE BASIS OF THIS RFQ OR REIMBURSE ANY COSTS ASSOCIATED WITH THE PREPARATION OF RESPONSES TO THIS RFQ. INTRODUCTION: This Request For Quote (RFQ) SOURCES SOUGHT(SS) is issued by Network Contracting Office (NCO) 6 to assist in determining if a future contract effort can be competitively awarded on a total Service-Disabled Veteran-Owned Small Business (SDVOSB) Set-Aside basis. The applicable North American Industrial Classification System (NAICS) code assigned to this effort is 541990 with the Small Business Size Standard of $17.0 million. The purpose of this notice is to conduct market research and obtain information from qualified potential offerors. No formal solicitation document exists at this time. The Government is not obligated, nor will it pay for or reimburse any costs associated with responding to this sources sought request. This notice shall not be construed as commitment by the Government to issue a solicitation or ultimately award a contract, nor does it restrict the Government to a particular acquisition approach. The Government will in no way be bound to this information if any solicitation is issued. REQUIREMENTS: The VHA Network Contracting Office 6 (NCO 6) is conducting a market survey to find qualified vendors who can deliver the following services for the Durham VA Medical Center (DVAMC) 508 Fulton Street, Durham, NC 27705. PURPOSE: The DVAMC is seeking potential sources capable of providing VA CARES PROGRAMMING SURVEY (Program of General Caregiver Support Services (PGCSS)) to be used with Durham VA s Center of Innovation to Accelerate Discovery and Practice Transformation (ADAPT), Health Service Research & Development Center. The Durham VA Medical Center requires the PGCSS survey to examine caregiver support services. Durham Physical Medicine & Rehabilitation Services specialist need these program surveys to initiate 1500 veteran caregiver PGCSS activities. PERFORMANCE WORK STATEMENT (PWS): The purpose of this PWS is to outline the tasks and responsibilities for a contract focused on Veteran caregiver health and wellbeing. Specifically, this PWS outlines the tasks and responsibilities of the successful Vendor to complete data collection for a telephone/web-based survey of fifteen hundred (1500) caregivers and a one-year follow-up survey. The Vendor will provide high-level survey assistance for an operations project within the Durham VA s Center of Innovation to Accelerate Discovery and Practice Transformation (ADAPT), Health Service Research & Development Center. The VA Caregiver Support Program Partnered Evaluation Center (VA CARES) is a partnered evaluation initiative supported by the VA Caregiver Support Program (CSP) and the VA Quality Enhancement Research Initiative (QUERI). The purpose of this caregiver health and wellbeing (CHASE) survey is to identify how participating in the Program of General Caregiver Support Services (PGCSS) is associated with changes in caregiver health and well-being outcomes and trust in VA over time and describe how caregivers engage in PGCSS services. We are also surveying caregivers who are not enrolled in CSP services as a comparison group. Each survey participant will be invited to participate at least twice (baseline, 12 month follow up) in this longitudinal survey. The Vendor shall furnish all labor materials, and any other supplies or equipment necessary to perform the survey. SCOPE: The survey contractor will work solely with Durham VA s ADAPT project team staff and be responsible for conducting a multi-mode survey of 1500 caregiver participants. Modes will include a web-based survey, and a Computer-assisted telephone interviewing (CATI). The vendor will use a sample of Veteran patients and caregivers provided by the ADAPT project team staff and will be responsible for providing all resources necessary to accomplish the deliverables described in this PWS, except as otherwise specified.
