Description
NOTICE OF INTENT TO SOLE SOURCE GE IMAGING DEVICES The Veterans Health Affairs, Network Contracting Office 16, is issuing this Notice of Intent to Sole Source to identify any potential sources that have the resources and capabilities to provide preventative maintenance of GE imaging equipment as described below in the statement of work. The government is contemplating a Firm Fixed Price contract to satisfy this requirement and meet the needs of the patient population at the Gulf Coast Veterans Health Care System. Period of Performance is Date of Award thru 09-30-2024. The period of performance is base plus four (4) option years. STATEMENT OF WORK (SOW) 1. Contracting Officer s Representative (COR). Name: Section: Address: Phone Number: E-Mail Address: 2. Contract Title. Preventive (PM) and Corrective Maintenance (CM) Services for 4 GE Imaging Devices listed below. 3. Background. G.V. (Sonny) Montgomery VA Medical Center is requesting a PM and CM service contract for 3 Nuclear Medicine Devices and 1 CT Simulator. These PMs must be completed in a timely manner for them to stay in use. If they are not PMed on correct OEM schedule, they will have to be pulled out of service, and any services that depend on this equipment would come to a halt. Any CMs needed from the vendor should have a corrective plan in action within one business day. 4. Scope. 1.Services must ONLY be performed by GE trained technicians, who have evidence of current factory training on the specific device being serviced. 2.Must provide evidence of equipment standards and traceability and final device performance testing that follows and complies with all manufacturer s procedural requirements. 3.Must guarantee that safety inspections performed will comply with all applicable federal, state and regulatory requirements, manufacturer standards, and meet FDA alerts and recall requirements. 4.Each technician must be certified on the applicable GE Device prior to performing any service and/or maintenance on GE manufactured equipment. 5.Must provide evidence of recent attendance and successful completion of the GE Factory Training Seminars for each product serviced. 6.Must maintain continuous access to the most up to date GE software revisions of the equipment listed. 7.Must guarantee execution of all Manufacturer Service Recommendations during each service incident, including the use of ONLY manufacturer planned maintenance and repair parts. (Note: Failure to utilize original manufacturer parts may result in the improper operation of the equipment.) 8.Must possess the legal rights to propriety equipment diagnostic software. (Note: Failure to utilize the Propriety Software may result in the inability to identify equipment operating parameters and to accurately calibrate the equipment.) 9.The service technician must have completed training for each specific device by a GE Authorized Trainer prior to performing service on said device. 10. The vendor must provide all parts necessary to PM and CM the units applicable. The G.V. Sonny Montgomery will not be charged for any parts necessary to maintain the service. 11. The equipment to be serviced/repaired/maintained under this agreement is listed in the tables below. All of the equipment listed is owned by the G.V. (Sonny) Montgomery VA Medical Center. 12. The vendor must keep equipment functioning at a 95% uptime rate. 13. The vendor must have an updated MOU/ISA for remote diagnostics. Normal hours of coverage are Monday through Friday 8am to 5pm. Service will be performed during normal hours of coverage unless requested or approved by COR. B. Federal Holidays observed by the VAMC are: New Years' Day Labor Day Martin Luther King Day Columbus Day Presidents' Day Veterans' Day Memorial Day Thanksgiving Day Independence Day Christmas Day Juneteenth Day Identification, Parking, Smoking, and Security: Contractor FSE shall wear visible identification at all times while on the premises of the GVSMVAMC. It is the responsibility of Contractor FSE to park in the appropriate designated parking areas. Information on parking is available from the VA Police Service. The GVSMVAMC will not invalidate or make reimbursement for parking violations of Contractor s FSE under any conditions. Smoking is prohibited inside any buildings at the GVSMVAMC. Possession of weapons is prohibited. Enclosed containers, including tool kits, shall be subject to search. Violations of VA regulations may result in citation answerable in the United States (Federal) District Court, not a local district, state, or municipal court. Place of Performance. Service and maintenance work will be completed on site at the G.V. (Sonny) Montgomery VA Medical Center. Period of Performance. The service for the base period will start 10/1/2023 and end 9/30/2024. The following four option periods will start and finish on 10/1 and 9/30, respectively, for the following years. Record Management 1. Contractor shall comply with all applicable records management laws and regulations, as well as National Archives and Records Administration (NARA) records policies, including but not limited to the Federal Records Act (44 U.S.C. chs. 21, 29, 31, 33), NARA regulations at 36 CFR Chapter XII Subchapter B, and those policies associated with the safeguarding of records covered by the Privacy Act of 1974 (5 U.S.C. 552a). These policies include the preservation of all records, regardless of form or characteristics, mode of transmission, or state of completion. 2. In accordance with 36 CFR 1222.32, all data created for Government use and delivered to, or falling under the legal control of, the Government are Federal records subject to the provisions of 44 U.S.C. chapters 21, 29, 31, and 33, the Freedom of Information Act (FOIA) (5 U.S.C. 552), as amended, and the Privacy Act of 1974 (5 U.S.C. 552a), as amended and must be managed and scheduled for disposition only as permitted by statute or regulation. 3. In accordance with 36 CFR 1222.32, Contractor shall maintain all records created for Government use or created in the course of performing the contract and/or delivered to, or under the legal control of the Government and must be managed in accordance with Federal law. Electronic records and associated metadata must be accompanied by sufficient technical documentation to permit understanding and use of the records and data. 4. GVSM and its contractors are responsible for preventing the alienation or unauthorized destruction of records, including all forms of mutilation. Records may not be removed from the legal custody of GVSM VAMC or destroyed except for in accordance with the provisions of the agency records schedules and with the written concurrence of the Head of the Contracting Activity. Willful and unlawful destruction, damage or alienation of Federal records is subject to the fines and penalties imposed by 18 U.S.C. 2701. In the event of any unlawful or accidental removal, defacing, alteration, or destruction of records, Contractor must report to GVSM VAMC. The agency must report promptly to NARA in accordance with 36 CFR 1230. 5. The Contractor shall immediately notify the appropriate Contracting Officer upon discovery of any inadvertent or unauthorized disclosures of information, data, documentary materials, records or equipment. Disclosure of non-public information is limited to authorized personnel with a need-to-know as described in the contract. The Contractor shall ensure that the appropriate personnel, administrative, technical, and physical safeguards are established to ensure the security and confidentiality of this information, data, documentary material, records and/or equipment is properly protected. The Contractor shall not remove material from Government facilities or systems, or facilities or systems operated or maintained on the Government s behalf, without the express written permission of the Head of the Contracting Activity. Contractor personnel shall sign VA Form 10-0708 Employee Clearance from Indebtedness before resigning or termination from Contractor working for GVSM VAMC Facility Records Officer must sign form. A copy of completed signed VA Form 10-0708 (Contractor personnel and Facility Records Officer signatures) shall be place in each competency folder. When information, data, documentary material, records and/or equipment is no longer required, it shall be returned to GVSM control, or the Contractor must hold it until otherwise directed. Items returned to the Government shall be hand carried, mailed, emailed, or securely electronically transmitted to the Contracting Officer or address prescribed in the contract. Destruction of records is EXPRESSLY PROHIBITED unless in accordance with Paragraph (4). 6. All Contractor employees assigned to this contract who create, work with, or otherwise handle records are required to take VHA provided records management training. The Contractor is responsible for confirming training has been completed according to agency policies, including initial training and any annual or refresher training. The Contractor shall incorporate the substance of this clause, its terms and requirements including this paragraph, in all subcontracts under this contract and require written subcontractor acknowledgment of same. b. The results of this notice will assist in the development of (1) the requirement, and (2) the acquisition strategy (e.g., small business set-aside, full and open competition, etc.). VA assumes no responsibility for any costs incurred associated with the preparation of responses submitted as a result of this notice.
