- Combined Synopsis/Solicitation Office Furniture- 151 ISRG Bldgs. 302 & 304 Reference RFQ# W50S9A-23-Q-0004 AMENDMENT 003: 20 July 2023 The purpose of this amendment is as follows: To include a revised Statement of Work (SOW) which clarifies the technical requirements and incorporates additional details about the sizes and shapes of the desks/worksurfaces. Please see the attached revised document, all changes made to the SOW have been annotated in red text. To clarify what the vendor shall include with the submission of their quote. Quotation shall include the following: Signed copy of this Combined Synopsis/Solicitation document showing acknowledgement of all the terms, conditions, and amendments. A company created layout rendering of the spaces to show conformance with the statement of work. Product data (as applicable) A company created pricing sheet. See the attached revised terms and conditions/SOW documents for full amendment information dated 20 July 2023.�Please submit any additional questions or concerns on the requirements as stated prior to the submission of a quotation. All other solicitation terms/conditions remain the same. The solicitation closing date has not been extended and remains Monday 31 July 2023 at 2:00PM MT. AMENDMENT 002: 19 July 2023 The purpose of this amendment is as follows: To include DFARS provisions 252.204-7024 Notice on the Use of the Supplier Performance Risk System, 252.225-7059 Prohibition on Certain Procurements from the Xinjiang Uyghur Autonomous Region�Representation and clause 252.225-7060 Prohibition on Certain Procurements from the Xinjiang Uyghur Autonomous Region as they apply to this solicitation. To include a revised Statement of Work (SOW) which clarifies the technical requirements and incorporates additional details based upon the additional questions received up to this point. Please see the attached revised document, all changes made to the SOW have been annotated in red text. Provide answers to all additional question on this requirement up to this point. SOLICITATION DATE: 22 June 2022 Requesting Agency: 151 Intelligence Surveillance and Reconnaissance Group (ISRG), Utah Air National Guard Purchasing Agency: 151 ARW/MSC, Utah Air National Guard, Roland R. Wright ANG Base, Salt Lake City, Utah 84116 This is a combined synopsis/solicitation for a commercial item prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a separate written solicitation will not be issued.
