- THERE IS NO SOLICITATION AT THIS TIME. This is a request for information (RFI) only and does not constitute a request for proposals. Submission of any information in response to this RFI is purely voluntary. The Government assumes no financial responsibility for any costs incurred associated to this request for capability information. Notice: This advertisement is for market research purposes for a potential opportunity. This advertisement is not a solicitation for offers, nor is it a request for proposals. The purpose of this advertisement is to identify potential sources and suitable locations and is not intended to pre-qualify or disqualify any potential offers. The Government will not pay for any costs incurred because of this advertisement. The Government cannot provide any warranty, expressed or implied, as to the accuracy, reliability or completeness of contents of the furnished information found within this advertisement. Respondents are advised that the Government assumes no responsibility to award a lease based upon responses to this advertisement The US Department of Veteran Affairs (VA) is issuing this RFI to conduct market research for sufficient Rentable Square Footage (RSF) of real property space to yield up to approximately 12013 Net Usable Square Feet (NUSF) in Ashtabula County, Ohio for use as a Community Based Out-patient Clinic (CBOC). Space must be contiguous, outside 100-year flood plain, and VA prefers space located on the first floor; space must be accessible by elevators if not on first floor. The Department of Veterans Affairs will consider existing space that complies with all current building, seismic, and ADA code requirements. On-site, dedicated parking for 75 vehicles is required. A total term of up to 20 years (firm and soft terms) will be considered. Delineated Area: Space within or fronting on the following boundaries will be considered: South: Area in and abounding the intersection of Blake Road and State Route 84 (South Ridge Road East) and heading west on State Route 84 (South Ridge Road East) to the intersection of State Highway 11. Heading south on State Highway 11 to Plymouth Ridge Road. Heading west on Plymouth Ridge Road to the intersection of Plymouth Ridge Road and Bunker Hill Road and heading west on Bunker Hill Road to the intersection of Bunker Hill Road and State Route 225 (Sanborn Road). West: Area in and abounding the intersection of Bunker Hill Road and State Route 225 (Sanborn Road) heading north on State Route 225 (Sanborn Road) to the intersection of State Route 225 (Sanborn Road) and U.S. Route 20 (West Prospect Road). North: Area in and abounding the intersection of U.S. Route 20 (West Prospect Road) and State Route 225 (Sanborn Road) and heading East on U.S. Route 20 (West Prospect Road) which turns into U.S. Route 20 (North Ridge Road East) to the intersections of U.S. Route 20 (North Ridge Road East) and Blake Road. East: Area in and abounding the intersections of U.S. Route 20 (North Ridge Road East) and Blake Road and heading south on Blake Road to the intersections of Blake Road and State Route 84 (South Ridge Road East.) Space must accommodate PACT (Patient Aligned Care Team) space model (http:/www.cfm.va.gov/til/dGuide/dgPACT.pdf)The lease space will be used to provide medical, mental health, and specialty services to Veteran. The following is an estimate of needed rooms/space: SDVOSB and VOSB firms are invited to provide information in response to this RFI to contribute to the market research for this project. SDVOSB and VOSB firms must be registered in the U.S Small Business Administration Veteran Small Business Certification (VetCert) at https://veterans.certify.sba.gov and have the technical skills and financial capabilities necessary to perform the stated requirements. Only SDVOSB and VOSB firms are requested to submit a Capabilities Statement to VA for review. SDVOSB and VOSB firms may provide a Capabilities Statement if they are interested in participating in this project. A list of items is provided below for firms to submit, which will serve as the firms Capabilities Statement. Capabilities Statement Will Include: 1. Company name, address, point of contact, phone number, Dunn & Bradstreet number, System for Award Management identification (if registered at www.SAM.gov), and e-mail address; 2. Evidence of SDVOSB or VOSB status through registration through VetCert at https://veterans.certify.sba.gov. 3. Evidence of ability to offer under NAICS Code 531120 and listing in the System for Award Management (www.SAM.gov), including a copy of the representations and certifications made in that system; 4. A summary describing up to three (3) projects of similar size and scope completed in the past five (5) years that demonstrate your company s experience designing, constructing, and managing Federal leased facilities or health care facilities relevant to the project described above. (3-page limit); 5. Evidence of capability to obtain financing for a project of this size. If desired, the company may also submit a narrative describing its capability. Although this notice focuses on SDVOSB and VOSB, we encourage all small businesses and other interested parties to respond for market research purposes. Expressions of Interest must be submitted by email to the Lease Contract Specialist (ashanica.mccardle@va.gov) by the publicized response date/time and must include the following information: 1) Building name, address and location of available space 2) Amount of offered space in Rentable Square Feet (RSF) and Net Usable Square Feet (NUSF) 3) Building ownership information, brokers or legal representatives must show written acknowledgement and permission to represent the property 4) Amount of parking available on-site 5) Business socio-economic category if other than a large business The Government is limited by law (40 USC 278a, as Amended 10-1-81) to pay no more than the appraised fair rental value for space.
