- This is a combined synopsis/solicitation for commercial products or commercial services prepared in accordance with the format in subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number 36C26223Q1481 is issued as a Request for Quotation (RFQ) in conjunction with FAR 13.106-I(c) and constitutes the only solicitation. The Government intends to award a single Firm-Fixed Price (FFP) award contract. This solicitation intends to award this procurement to a Small Business, associated with NAICS Code 335999, All Other Miscellaneous Electrical Equipment and Component Manufacturing. Vendor must be active and currently registered at SAM.gov to be awarded. This solicitation is an unrestricted, full and open competition. The Department of Veterans Affairs (VA) seeks to a contractor to provide Medical Grade Surge Protectors that are same or equal to Leviton 53C4M-1S7 to serve the medical equipments at the San Diego VA Healthcare System. Offeror must demonstrate meeting the salient physical, functional, or performance characteristic specified in this solicitation, otherwise it will be considered non-responsive. The interested parties bear full responsibility to ensure their submission demonstrates to the government that they can provide the brand name or equal equipment being requested. Delivery Requirements: Place of Delivery: VA San Diego Jennifer Moreno Medical Center, 3350 La Jolla Village Drive, San Diego Ca 92161 Delivery Timeframe: 08/31/2023 09/30/2023 Shipping and Delivery fee must be included to product cost. Submission of Offers: All offeror quotes for this solicitation must be received electronically through email to Sam Choo at sam.choo@va.gov no later than Friday, July 28th, 2023, 10:00 AM PST. Please reference solicitation number 36C26223Q1481 within subject line of the email. Any offeror quotes received after the prescribed date/time shall be considered non-responsive and will not be considered for award. In order for an offer to be considered valid, the following must be submitted by offerors via email at the time the offer is submitted: Offerors shall fill out all unit price, total item cost, and net item cost conforming to requirements stated by the salient characteristics in Item Information. Offerors shall fill out Standard Form 1449 and section B.1 and return it with offer. Offerors shall fill out and sign all sections of solicitation Notice including section D.2, 52.204-24 REPRESENTATION REGARDING CERTAIN TELECOMMUNICATIONS AND VIDEO SURVEILLANCE SERVICES OR EQUIPMENT (NOV 2021) and return it with offer. Offerors shall fill out and sign all sections of solicitation Notice including section D.5 52.212-3 OFFEROR REPRESENTATIONS AND CERTIFICATIONS COMMERCIAL PRODUCTS AND COMMERCIAL SERVICES (DEC 2022) and return it with offer.
