DEPARTMENT OF VETERAN AFFAIRS Wilkes-Barre VA Medical Center STATEMENT OF NEED Title of the project: STRYKER - MAKO ROBOTIC ARM SYSTEM AND APPLICATIONS for Wilkes-Barre VAMC Introduction: The Department of Veteran Affairs Medical Center, located at 1111 East End Blvd., Wilkes-Barre, Pa, has determined the need to purchase The STRYKER - MAKO ROBOTIC ARM SYSTEM AND APPLICATIONS. History/Background/Purpose: As a Level II Intermediate Facility Wilkes-Barre VA Medical Center has a new requirement to procure the MAKO ROBOTIC ARM SYSTEM AND APPLICATIONS to perform surgical procedures in the operating room. This advanced technology is a remarkable tool in the battle against knee and hip pain and will extend and enhance the quality, precision, and safety of total knee and total hip replacement procedures presently being completed at our facility for the Veterans we serve. WBVAMC has four (4) full time Orthopedic Surgeons, and the procurement of this system and application would allow for expansion of the current surgery program to include thoracic or vascular surgery services, transplant surgery, cardiac surgery, bariatric surgery, neurosurgery, or total joint replacement and initiation or expansion of advanced technology applications to already approved procedures, i.e., radiofrequency ablation, cryotherapy, robotic surgery, and sentinel node evaluations (pathology and nuclear medicine). The addition of this system in WB VAMC would increase the volume of surgical procedures possible, provide better surgical outcomes, improve patient satisfaction, strengthen the Choose VA conversation for Referral Coordination Initiatives, and VHA DIRECTIVE 1043 APPENDIX A A-4 reduce Wilkes Barre s reliance on the community and VISN partner sites. Orthopedic robotic surgery will allow the facility to offer an advanced form of joint replacement procedures where surgeons use a computer-controlled robot to assist. By offering this service at the WB VAMC, we can facilitate a significantly shorter recovery period for our Veterans, as well as high-quality technologically advanced care closer to home. Value to our Facility: Resources for implementing the orthopedic robotic surgery program correspond to the space and staff requirements currently in use for joint replacement procedures performed at the WB VAMC. No construction or modification of the operating room suites will be required. The incorporation of orthopedic robotic program will assist with highly skilled provider recruitment and retention and place our Surgical Program in line with those offered at community Medical Centers within our catchment area. (5) Describe how your small business takes ownership of the equipment in the manufacturing, assembly, or delivery process. (6) If you re a small business and you are an authorized distributor/reseller for the items identified above or an equivalent solution, do you alter; assemble; modify; the items requested in any way? If you do, state how and what is altered; assembled; modified? (7) If you intend to subcontract any work on this contract, what portion of the total cost will be self-performed/will be performed by your organization? Please provide estimated detailed percentage breakdowns related to subcontracted work and completion of job. (8) Does your company have an FSS contract with GSA or the NAC or are you a contract holder with any other federal contract? If so, please provide the contract number. (9) If you are an FSS GSA/NAC contract holder or other federal contract holder, are the items/solution you are providing information for available on your schedule/contract?
