- SPECIAL NOTICE - REQUEST FOR QUALIFICATIONS (SF-330) This request for SF-330s is issued as full and open competition. All businesses, small and large, are invited to submit qualifications. NAICS Code: 541310 Architectural Services (Small Business size standard $12.5 million dollars) A. INTRODUCTION: The VA Connecticut Healthcare System (VACHS), West Haven Campus, located at 950 Campbell Street, West Haven, CT, 06516, has a requirement for a firm fixed price contract to provide design services to include site investigation, development of complete drawings, specifications, cost estimates, and construction period services for Project 689-22-113, Install Hybrid Operating Room. The VA West Haven CT currently has a design in process to upgrade all utilities in the Operating Room Suite (identified as Project 689-21-111). The Upgrade OR Utilities project incudes locating and planning through 35% for future investigation of a new Hybrid OR into the Operating Room Suite located on the third floor of Building 1. The 689-21-111 project includes physical location and utility capacity considerations to reduce the impacts of construction of a future Hybrid OR. This new requirement is seeking design services to take the previously completed 35% submittal from the Upgrade OR Utilities project through completion to design a new Hybrid OR Suite. The new Hybrid OR also known as Transcatheter Aortic Valve Replacement Surgery (TAVRS) design includes reconfiguring and modernizing existing Operating Room space including a new Hybrid OR, control room, equipment room, etc. and storage considerations for Sterile and Reusable Medical Equipment (RME). See attached Statement of Work for complete details. B. CONTRACT INFORMATION: The Magnitude of Construction is between $5,000,000 and $10,000,000. This acquisition is not set-aside for any socio-economic status and is full and open competition. All businesses capable of performing the work associated with this requirement are invited to submit qualifications. Firms must be registered in System for Award Management (SAM www.sam.gov) by the due date for SF-330s and remain registered through award and performance. SF330 packages shall address how each firm anticipates meeting the evaluation criteria. This requirement is in accordance with the Selection of Architects and Engineers as implemented in Subpart 36.6 of the Federal Acquisition Regulation and Subpart M836.602-1 of the VA Acquisition Manual. All submissions will be evaluated in accordance with the evaluation criteria. A ""short list"" of the three to four most highly rated firms after initial source selection will be chosen for discussions through interview presentations, format to be determined by the Contracting Officer. The most highly rated firm at the conclusion of the interview process will be invited to submit a proposal in anticipation of a contract award. The Government will not pay, nor reimburse, any costs associated with responding to this request. The Government is under no obligation to award a contract because of this announcement. Respondents are put on notice that firm(s) selected by the Selection Authority will be required to submit a Fixed Price Rate Proposal on the VA Form 6298 to include labor, overhead, profit, and other costs for this project for negotiation by the Contracting Officer prior to contract award. The negotiated agreement will be incorporated into the resulting contract. C. THIS IS NOT A REQUEST FOR PROPOSAL: This is a request for SF 330s Architect/Engineer Qualifications packages only. Any requests for a solicitation will not receive a response. No material will be issued and no solicitation package or bidder/plan holder list will be issued. D. PROJECT INFORMATION: Provide architectural services to develop a complete design package to include drawings, specifications, cost estimates, and construction period administration services associated with Project 689-22-111, Install Hybrid Operating Room. This project is a follow-on project to the 689-21-111 Upgrade OR Utilities project. The 689-21-111 provides a conceptual 35% floorplan and the utilities to support the Hybrid OR. Thorough review of the 689-21-111 project documents is required. The Hybrid OR will contain imaging equipment as well as support spaces such as scrubbing area, imaging component room, control room and other associated areas. The work will occur within a fully functioning surgical suite requiring strict adherence to maintaining all mechanical, electrical, and plumbing systems during construction of the Hybrid OR and associated areas. Compliance with VA design guideline standards, manuals and referenced codes as listed on the VA Technical Information Library (https://www.cfm.va.gov/til/index.asp) is paramount and no deviation shall occur. The purpose of the Hybrid OR is to provide advanced cardiothoracic, vascular, urologic, and neurosurgical procedures requiring fixed imaging that are currently being outsourced into the community. It will allow vascular surgery to perform the full range of vascular and endovascular procedures. The attached statement of work for this project provides competed details of the requirements of this project. E. EVALUATION SELECTION CRITERIA: Each respondent s SF 330 submittal package will be evaluated in accordance with FAR 36.6 procedures. The selection criteria, in descending order of importance, however, factors 1-4 are significantly more important and will be weighted twice that of factors 5-8. The factors are as follows: 1. Knowledge and Relevant Experience 2. Professional Qualifications 3. Surgical Suite Subject Matter Specialist 4. Past Performance 4. Experience in Construction Period Services 5. Geographic Location 6. Capacity 7. Commitment to the use of SDVOSB, VOSB and SB as Subcontractors F. SELECTION CRITERIA DESCRIPTIONS: The selection criteria descriptions (1 through 8) are provided below. Factor 1: Knowledge and Relevant Experience The A/E firm shall demonstrate specific relevant experience by submitting no less than three (3) examples of completed and operational projects. These projects shall include a combination of Hybrid OR with endovascular suites, cardiac catheterization laboratories, vascular surgery suites and /or electrophysiology laboratories, or any combination thereof up to five (5), but no less than three (3) each of which shall be completed and clinically operationalized within the last five years. The A/E shall provide a summary of the design and construction challenges for each type of project, include name, telephone and email address of each project contact, design and construction period and year projects were completed and accepted by Facility, Biomedical Engineering and Clinical staff. Projects that are still in progress will not be considered as examples of experience. Each of the project examples shall be of similar size or larger as that of the proposed Hybrid OR in scope and complexity. Greater weight will be given to A/E s experience as a prime contractor rather than as a consultant. More recent and relevant experience will have a greater impact on the assessment than less recent and relevant effort. Each project example submitted attesting the experience required shall include: Name of Client Client contact information (telephone number, email address) Location of Project and Mailing address Scope and Complexity of Project Year of Project completion and acceptance. Factor 2: Professional Qualifications The A/E team shall submit a Project Organizational Chart that includes all team members. The chart shall identify the licensed/registered Key Personnel. Key personnel shall have and demonstrate their acumen, knowledge and experience in the planning and design of Hybrid ORs and endovascular suites. The summary shall clearly describe team member s prime responsibilities as well as the roles and responsibilities of individuals proposed as consultants and subcontractors. The proposed personnel and staffing shall be evaluated in accordance with the Statement of Work. Provide resumes for each proposed team member, including sub-consultants, indicating the specific healthcare planning and design experience for their proposed role on the project. Particular emphasis shall be noted of the specific surgical service planning and design experience of each proposed team member. The Contractor shall identify Key Personnel in the technical quote, including the Medical Research and Development Subject Matter Expert(s). Limit one (1) page per each Key Personnel and for the subject matter expert. Identify the qualifications of key personnel. For A/E teams, each subconsultant shall provide letters of commitment to the project team. The A/E firm agrees to assign to the contract those persons whose resumes, personnel data forms or personnel qualification statements were submitted. If proposed personnel, for whatever reason, become unavailable for work under the contract for a continuous period exceeding 30 working days, or are expected to devote substantially less effort to the work than indicated in the proposal, the A/E firm shall propose a substitution of such personnel immediately. All proposed substitutions shall be submitted, in writing, to the Contracting Officer at least 15 days prior to the proposed substitution. All proposed substitutes will have qualifications equal to or higher than the qualifications of the person being replaced. For each proposed professional acting as a key technical resource in each specific discipline, provide resumes demonstrating specific experience in the services related to their discipline in planning and design of Hybrid ORs and endovascular suites. The qualifications evaluation will consider the breadth of education, training, certification, overall and relevant experience and longevity with the proposed team for all proposed key personnel. Provide biographical data, to include the following: Name of individual. Company employed by. Company position title. Years with the company. Professional registration and AHJ rendering certification. Describe work experience with projects that were with healthcare facilities planning and designing Hybrid OR s and endovascular suites and the company (by name) they worked for when involved in the project. An indication of which (if any) projects submitted under Knowledge and Relevant Experience (above) the individual participated in and what the individual s responsibility was for that project. An indication of which other individuals submitted under Professional Qualifications this individual has worked with and the project they worked on together, noting if that project has been submitted under Knowledge and Relevant Experience (above). Describe job related educational experience including degrees, certificates etc. and granting institutions. Define the position that the individual will hold for this contract/project team, description of duties and what percentage of the individual s time would be committed to the project during both the design and construction phases. Demonstrate in-depth Knowledge, use and compliance with VA Standards, VA Information Library and SEPS Chapter 286. Submit A/E Qualifications SF-330, pursuant to FAR (48 CFR) 53.236-2(b). Factor 3: Surgical Suite Subject Matter Specialist The A/E team shall include a Surgical Suite Subject Matter Specialist who has the requisite knowledge and experience in the planning, design and construction of complex Integrated Surgical platforms and shall submit to the VA evidence of Surgical Suite Subject Matter Specialist s credentials that substantiate the following: 1. Knowledge, use and compliance with VA Standards, VA Information Library and SEPS Chapter 286. 2. Examples of current planning and design of complex tertiary and quaternary/academic hospitals. 3. Examples of planning and design of integrated interventional surgical suites. 4. Examples of Hybrid OR planning and design utilizing the following modality technology: Bi-plane OR and Endovascular Suites Monoplane OR and Endovascular Suites Robotic OR and Endovascular Suites 5. Evidence of thorough knowledge of Cardiovascular Catheterization Imaging Laboratories, EP Laboratories, Interventional Radiology and Vascular Surgery Suites and including radiation protection of said procedure rooms, suites and surgical operatories. 6. Names, mail and email addresses, telephone numbers of contacts for exemplified hospitals submitted. Factor 4: Past Performance The A/E firm shall submit a minimum of three and maximum of five (5) past performance evaluations on recent projects relevant to the one being procured under this notice. Firms will be evaluated for past performance on recent and relevant contracts with Government agencies (emphasis on VA work) and private industry in terms of cost control, professional performance, general management, cooperativeness, quality of work, compliance with performance schedule and a record of significant claims against the firm due to improper or incomplete design services (references required). Recency is defined as performance occurring within five (5) years of the date of this Notice, except that ongoing projects must have begun no less than one (1) year prior to the issuance of this Notice. Relevancy is defined as performance of work on projects that are similar in scope to the type of project anticipated under the resultant contract. Respondents with no previous past performance shall so state when addressing the selection criteria. Documentation supplied should detail firms ability to work with government entities and design standards or similar relevant experience with other entities. Provide a copy of all performance evaluations (A/E Services) issued for government contracts (as applicable). For Non-Federal contracts, provide facility owner documentation of the firms performance issued on that contract. If known documentation exists, so state. Provide accessible owner points of contact: name, title, address, telephone number, e-mail, and fax number and provide copies of awards and letters of appreciation/recommendation as desired. If there is no CPARS data available, Past Performance Questionnaires (PPQs) will be accepted. Please utilize the attached PPQ document. NOTE: We recommend you contact every reference listed and let them know we may contact them for information. Their timely replies would be appreciated. Factor 5: Experience in Construction Period Services: Respondents must provide one recent project and provide a narrative on how the A/E firm incorporated the following into their project: Professional field inspections during the construction period Review of construction submittals Support in answering requests for information during the construction period Support of construction contract changes to include drafting statements of work and cost estimates Any project provided in addition to the one will not be evaluated as part of this criteria. Factor 6: Geographic Location: The A/E Firm must demonstrate that it has general knowledge of the locality of the project, location in the general geographical area of the project and knowledge of the locality to include specific knowledge of certain local conditions or project site features such as: geological features climate conditions local construction methods construction firms and trades labor availability permit requirements local laws and regulations Location of the firm is measured by the driving distance between the principal business location and the VA CTHCS West Haven Campus, 950 Campbell Avenue, West Haven, CT. Determination of the mileage will be based on Google Maps (https//www.google.com/maps/dir/) Factor 7: Capacity: The A/E team shall show current and projected workload within the firm overall and for the specific team members. Documentation supplied shall support that the firm has the capacity to accomplish the work in the required time. A signed statement shall be provided stating that the firm acknowledges that prior to award of the contract, they are required to notify the Government of any changes in their capacity that could limit their ability to perform the work within the required time. Factor 8: Commitment to the use of SDVOSB, VOSB, and other Small Businesses as Subcontractors: Respondents must provide a narrative on how they will commit to the use of Service-Disabled Veteran-Owned small businesses, Veteran-owned small businesses, and other types of small businesses as subcontractors. Respondents are responsible for ensuring SF 330 submittal packages, any revisions, and modifications are submitted by this time. b. Submission of information incorporated by reference is not allowed. d. Prospective firms shall address all selection criteria factors within their submitted SF 330 package. e. Submissions are limited to 65 pages. i. Absolutely no photographs shall be submitted. ii. Title pages and tables of contents, if applicable, are excluded from the page count limitation. iii. Pages submitted more than the limitations specified in this Notice will not be evaluated by NCO 1. f. Telephone or Fax inquiries will not be accepted. I. SF 330 PACKAGE CONTENTS: a. The SF 330 submittal packages shall consist of a completed SF 330, specific information addressing each of the seven (7) selection criteria described in this Notice. b. Respondents are required to meet all requirements in addition to those identified as selection criteria. c. SF 330 submittal packages not consistent with the requirements and selection criteria of this Notice may be determined unacceptable and removed from consideration. d. Any SF 330 submittal package, modification, or revision, that is received after the exact time specified for receipt is late and will not be considered unless it is received before award is made, the contracting officer determines that accepting the late submittal would not unduly delay the acquisition; and there is acceptable evidence to establish that it was received at NCO 1 installation designated for receipt of submittal packages and was under NCO 1 s control prior to the time set for receipt of submittal packages; or it was the only submittal package received. J. SF 330 PACKAGE PREPARATION: a. Electronic copy shall be in searchable PDF format of the SF 330. b. A cover sheet should be included, clearly marked with the Respondent's name, complete address with 9 digit zip code, phone number, e-mail address, DUNs number and Unique Entity Identifier, date of response, title, and Notice identification. c.
