Description

The Navajo Area Indian Health Service, Tsaile Health Center, Optometry Department intends to award a non-competitive, sole source to proposed contractor, Proaim Americas for the purchase of an Carl Zeiss OS Cirrus HD-OCT, Anterior Segment Module Cirrus 5000 and Optime complete service. Authority cited FAR Part 6.302-1, Only one responsible source and no other supplies or services will satisfy agency requirements. The Proposed Contractor shall provide the Carl Zeiss OS Cirrus HD-OCT, Anterior Segment Module Cirrus 5000 and Carl Zeiss Optime Service Complete Cirrus 5000 Angio services at the Tsaile Health Center. The Proposed Contractor is the only certified service vendor uniquely qualified and the only source for providing services/supplies, service contracts, parts, installation and software for the CARL ZEISS OS CIRRUS HF-OCT(Model 5000) with AngioPlex OCT Angiography Version 11, includes GCGPA Smart HD Dcans Fast Trac Adv RPE GCA Ant Seg Basic, ZEISS OS ANTERIOR SEGMENT PREMIER MODULE CIRRUS 5000(IMAGING ONLY) includes external lenses-measurements will be released following FDA clearance, CARL ZEISS MEDITEC INC OPTIME SERVICE COMPLETE CIRRUS 5000 ANGIO. All technical services weather remotely or on-site will be delivered between the hours of 8 am and 5 pm Monday - Friday. Clinical training is limited to 8 hours per day Monday - Friday and may be arranged to accommodate nursing shift changes as required by the facility. Installation and training dates will be mutually agreed-upon during project planning. Customer shall notify Proaim Americas of any changes to scheduled on-site activities at least 14 days prior to such scheduling event. If notification is less than 14 days prior to the on-site activity, rescheduling fees will apply. The Proposed Contractor is the only vendor to maintain the life of the instrument, including but not limited to any instrument enhancements, modifications and/or software updates. This notice of intent to award a sole source contract is not a request for competitive proposals. There will be no solicitation available for competitive quotes. Any response to this notice must show clear and convincing evidence that competition would be advantageous to the Government. Respondents are advised that the Government is under no obligation to acknowledge receipt of the information received or provide feedback to respondents with respect to any information submitted.� If no affirmative written response is received by August 10, 2023, 5:00PM MST, the contract will be issued without further notice. A determination by the Government not to compete the proposed procurement base upon responses to this notice is solely within the discretion of the Government. Information received will be considered solely for the purpose of determining whether to conduct a competitive procurement in the future.� Any solicitation resulting from the analysis of information obtained will be announced to the public in Federal Business Opportunities in accordance with the FAR Part 5. However, responses to this notice will not be considered adequate responses to a solicitation. No proprietary, classified, confidential, or sensitive information should be included in your response. The Government reserves the right to use any non-proprietary technical information in any resultant solicitation(s).
