Description
Operating Room ENT Sinus Navigation Statement of Work (SOW): Asheville VA Medical Center (VISN 6) requests to purchase the Stryker navigation kit. The Charles George VAMC otolaryngology currently do not have a functional sinus navigation tool. Sinus navigation allows the surgeon to use registered instruments to help localize where they are in the sinuses. When there is significant sinus disease landmarks can be obscured and patients can be put at risk for injury to the eye including blindness, cerebrospinal fluid leaks, and life-threatening hemorrhage. Sinus navigation allows use of CT scans obtained preoperatively to coordinate with the patient's anatomy and the surgical tools and give visual feedback on the CT scan in multiple dimensions to the operator. 1 unique aspect of the Stryker machine is that they have ""plug-in play"" sections which do not require registration. This greatly improves the ability to perform such surgeries when the scrub tech has not had a high volume of surgeries to train on. Additionally, such plug and play suctions can serve as a resource to ensure that a registered instrument is properly registered. Lastly, we currently use the Stryker balloon for sinus balloon dilation and these can also function with this system if navigation is required. This is particularly helpful in the frontal sinus. Currently patients with frontal sinus disease and sphenoid sinus disease and some with extensive ethmoid disease are sent to the community due to lack of sinus navigation. Having a sinus navigation instruments such as this would allow patients to have the choice to have their surgeries performed at this VA (unless a case required neurosurgery to be present since we do not have neurosurgeons at this VA). Sole Source: As stated above, the ability to use plug and play instruments is unique to this system and the system works with sinus balloon dilators which we already use at this VA. Delivery/Pickup: Vendor is responsible for uncrating, assembling and delivering to a specific location in the CGVAMC. Training: Some training will be needed for equipment registration and set up of the system however we have used image guidance in the remote past at this facility and similar systems are used in other services. User and Service Manuals: Stryker will provide user manuals and initial training and support.
