Description
The Department of Health and Human Services (HHS), National Institutes of Health (NIH), National Cancer Institute (NCI), Center for Cancer Research (CCR), Radiation Oncology Branch (ROB) has a need for additional ARIA software licenses, and software maintenance services for Varian ARIA, Eclipse, and InSightive Radiotherapy Treatment Planning Systems. This is a Combined Synopsis/Solicitation for commercial items, prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, as supplemented with additional information included in this notice, and shall be processed in accordance with FAR 13.5 - Simplified Procedures for Certain Commercial Products and Commercial Services. This announcement constitutes the only solicitation that will be provided in relation to the stated need contained herein. This solicitation is issued as a Request for Quote (RFQ) and includes applicable provisions and clauses in effect through FAR Federal Acquisition Circular (FAC) 2023-04 (06/02/2023) (Enter most recent FAC number and date from www.Acquisition.gov prior to posting.).� The requirement�s North American Industry Classification System (NAICS) code is 811210 (Other Electronic Equipment Repair and Maintenance) with a size standard of $34.0 Million. One Firm-Fixed-Priced award is anticipated to be made as a result of this solicitation. �� � QUESTION SUBMISSIONS Please submit questions concerning this RFQ via email by no later than 10:00 A.M. �EST on Wednesday, August 09, 2023 (08/09/23) to the points of contact listed below: � � � Name: John Russell, Contract Specialist � � � Email: John.Russell3@nih.gov � Note: Questions submitted by a method other than email will not be accepted. Any questions received after the specified cut-off time and date above will not be accepted or answered unless at the Contracting Officers discretion is deemed necessary. Additionally, all questions and answers of common interest will be provided to all prospective offerors via an amendment to the posting. All emails must include the Solicitation number 75N91023Q00156 in the subject line. � �� QUOTATION SUBMISSION AND FORMAT� Quotations submitted in response to this solicitation must be submitted via email no later than 10:00 A.M. EST on Tuesday, August 15, 2023 (08/15/23) to the points of contract listed below: � � Name: John Russell, Contract Specialist � � � Email: John.Russell3@nih.gov � Quoters may submit more than one quote. If multiple quotes are provided, the Government will evaluate each quote separately. The total page limit of your submission (including technical quote, price quote, and other attachments) is 30 pages. The vendor must provide one quote with pricing only and one technical quote. The technical quote must not contain any pricing information. All quotations shall provide enough information for the Government to conduct a thorough evaluation of the quotes provided, and the quoter�s ability to fulfill the stated needs within this RFQ. At a minimum all quotes shall include in the following: � 1.�� �Cover letter including (maybe included as body of quote submission email): a)�� �RFQ Number b)�� �Federal Tax Identification Number (TIN); c)�� �Unique Entity Identifier (UEI); d)�� �Cage Code; e)�� �Business size and designation (i.e., Small, Other than Small); f)�� �The applicable solicitation number for which the quote is provided; g)�� �Point of Contact Information for individuals authorized to commit on behalf of their organization. Including name, address, and telephone number of the offeror; h)�� �Acknowledgement of Solicitation Amendments (if applicable);� i)�� �""Remit to"" address, if different than the mailing address;� j)�� �Include a statement to the effect that pricing is firm for a period of at least 60 days from the date of receipt thereof by the Government. 2.�� �Price Quote - The price quote must be a separate volume from the technical quote. The quote must contain sufficient information to allow the Government to perform an evaluation of the entire quoted price for each requirement identified within the statement of work (SOW), as well as the overall quoted price for the base requirements, and all options. � 3.�� �Technical Quote � Technical quote shall be a separate volume from the price quote. The technical quote shall contain no pricing information. Technical quotes should include sufficient information for the Government to assess the offerors capabilities in accordance with the technical evaluation factors. As such, the offeror is encouraged to include the following: ��� �Detailed information pertaining to the offerors proposed technical approach, and how it will meet the requirement.� ��� �Information pertaining to the offers Organizational and Management Experience. Provide up to three (3) previous or current contracts performed within the past five (5) years that are the most similar in size, scope, and complexity to this acquisition where your company was the Prime or Subcontractor. Describe how each example is similar (and dissimilar) to this work. This must include summaries of the work performed, personnel resourcing, dollar values, and points of contact for each example. 4.�� �A completed copy of the representations and certifications at Federal Acquisition Regulation (FAR) 52.212-3 (see FAR 52.212-3(b)) for those representations and certifications that the offeror shall complete electronically) (See attachment to the Combined Synopsis Solicitation);� 6.�� �A technical description of the items/services being offered in sufficient detail to evaluate compliance with the requirements in the solicitation. This may include product literature, or other documents, if necessary. Note: It�s the Quoter�s responsibility to read the RFQ and all related documents carefully. Additionally, it is the Quoter�s responsibility to ensure/verify the Government receives its submission on or before the date/time specified above. The Government reserves the right to not accept any late quotes, any quote that does not provide items/services requested, or quotes that do not conform to the requirements of the RFQ. Please do not wait until the deadline to submit your quotation. Quotes submitted by any other means other than email will not be accepted.
