- VA CETC WRIISC Questions/Answers Reference Question/Answers (yellow highlight) Section E, (d) Product Samples: When required by the solicitation, product samples shall be submitted at or prior to the time specified for receipt of offers. Unless otherwise specified in this solicitation, these samples shall be submitted at no expense to the Government, and returned at the sender's request and expense, unless they are destroyed during preaward testing. Are product samples required? NO E.2 Supplemental Instructions to Offerors Offerors must: (a) Complete Box 12, 17, and 30 of the SF 1449 (Page 1) of the solicitation. Provide the legal entity name, address, telephone number and SAM Unique identifier in Box 17. (b) Complete Section B.1., contractor POC information. Are these requirements for the proposal or award? FOR THE PROPOSAL Submission Does the Government want responses in 1 document/volume, to include all required sections? ONE (1) If separate volumes are required, would the government please provide specific instructions for each? Section E.1, b. Submission of Offers, ""At a minimum, offers must show"" (2) The time specified in the solicitation for receipt of offers Does ""the time specified in the solicitation for receipt of offers"" mean the due date? YES Section E.2 Evaluation Factors for Technical Capability, 2.(a) Provide the certificate required in VAAR 852.219-75: VA Notice of Limitations on Subcontracting Certificate of Compliance for Services and Construction. Where should this section go in the proposal? END OF PROPOSAL Section E.2 Evaluation Factors for Technical Capability, 2.(b) The Offeror shall complete FAR Provision 52.212-3. The Offeror shall complete only paragraph (b) of this provision if the Offeror has completed the annual representations and certification electronically in the System for Award Management (SAM) accessed through https://www.sam.gov. If the Offeror has not completed the annual representations and certifications electronically, the Offeror shall complete only paragraphs (c) through (v)) of this provision. END OF PROPOSAL Section E.1 (b) Submission of Offers, (8) A completed copy of the representations and certifications at Federal Acquisition Regulation (FAR) 52.212-3 (see FAR 52.212-3(b) for those representations and certifications that the offeror shall complete electronically) Where should this section go in the proposal? END OF PROPOSAL Section E.2, Evaluation Factors for Technical Capability, (d) Offeror must complete the contractor fill in sections of the attached Business Associate Agreement, unless contractor has an existing National BAA. The Business Associate Agreement is not attached to RFQ. Please provide. - BAA is not needed per confirmation from Privacy Officer there is a National BAA for this service
