- Intent to Sole Source-Martin AIJS, LLC SPECIAL NOTICE: 36C24923Q0541 The Department of Veterans Affairs intends to issue sole-source firm-fixed priced contract to Martin AIJS, LLC. 751 Lynne s Cove Wadsworth, OH 44281. In accordance with Federal Acquisition Regulation (FAR) 13.106-1(b)(1)(i) and VAAR 819.70/38 U.S.C. 8127-8128, on behalf of the Louisville Healthcare System, Network Contracting Office (NCO) 9 intends to negotiate on a sole source basis with Martin AIJS, LLC located at 751 Lynne s Cove Wadsworth, OH 44281. The rationale basis for the sole source acquisition is that Martin AIJS, LLC. (SDVOSB) is the only responsible source that can provide the original equipment manufacturer (OEM) for the NuTrace with OME warranty as well as installation and training support for Louisville Healthcare System regions. The NAICS code is 339112 and has a size standard of 1,000 employees for this procurement. Background. Louisville Healthcare System requires the purchase of laser engraver, Training, NuTrace R Scanner and Reader. ITEM INFORMATION The Louisville VA Health Care System has a requirement for a Laser Engraver for Sterile Processing Service. The Laser Engraver will allow Sterile Processing Service to produce a laser etch on all reusable instrumentation. Reusable instruments must be etched to provide tracking of instruments through the hospitals multiple departments. Items Quantity NuTrace Fiber Laser 1064nm 20W. NuTrace Select Set of Assorted Vectors to identify surgical instruments. 2 hours of laser safety awareness guidance online by LIA (Laser Institute of America). 18 hours of training on site included. Instructional videos and manuals At least a 20W 1064nm MOPA air cooling laser (similar to NuTrace Laser NUX20NG). 110/220V 50/60Hz 10Amp. Compact workstation the unit can t be larger than 12x12x14 Marking items as small as 1.5mm x 1.5mm area 5 ft fiber cable for easy module Fume extraction port and laser protection glass. Top handle for easy carrying FDA Compliant Approved for use on VA Network by the Department of Veteran Affairs Office of Information Technology. Must be able to mark existing instruments all types of surgical instruments including titanium, stainless steel, bronze, anodized aluminum, anodized titanium including cylindrical, hard plastics, and glass including cylindrical items. Mark instruments with unique marking for current instrument for trackability. Have a scanner and reader to read each instrument that interface and enter inventory information into Inventory Management System. Must be able to verify GS1 U.S. FDA Unique Device Identification (UDI) compliance for new instrument. Other important considerations: Cite any other information that is necessary for potential respondents to understand the nature of the potential requirement or proposed acquisition. Capability statement /information sought: Response to this announcement shall not to exceed 10 pages and should include the following information: (1) A tailored capability statement indicating the firm s ability to provide a product and related services; (2) The respondents Unique Entity Identifier (EUI) number, organization name, address, point of contact, and size and type of business (e.g., SDVOSB/VOSB, etc,) pursuant to the applicable NAICS code; (3) Information should be provided electronically in a Microsoft Word or Adobe PDF format. (4) The offered must be registered in the Small Business Administration Veteran Small Business Certification (VetCert) as Service-Disabled Veteran Owned Small Business (SDVOSB) (5) Original Equipment Manufacturers(OME) authorized distributor letter must be provided. This notice of intent is not a request for competitive quotes. Responses received will be evaluated; however, a determination by the Government not to compete the proposed procurement based upon responses received to this notice is solely within the discretion of the Government.
