- This is a SOURCES SOUGHT only pertaining to brand name or equal item(s) for the LTC Charles S. Kettles VA Medical Center, Ann Arbor, MI. Information collected during this Sources Sought may be used in a set aside. If a solicitation is issued, the Government will do so in accordance with the latest Federal Acquisition Circular (FAC). The North American Industry Classification System (NAICS) number is 337214. The NAICS size is 1,100 employees. Any contractor that believes they are capable and desires to claim preference for small business status must be registered with the SBA and be able to be located through SBA - Dynamic Small Business Search. Any contractor that believes they are capable and desires to claim preference for veteran owned small business status must be registered with in VetCert at Veteran Small Business Certification (sba.gov) as an SDVOSB or VOSB. Contractors that deem themselves capable of meeting the requirement shall provide the below information to James Howard, Contract Specialist, at james.howard3@va.gov no-later-than Monday, August 21, 2023 at 4:00PM, EDT, referencing 36C25023Q1055. Responses shall include: (1) Business Name and Address, (2) Contract Number items are covered under, if applicable (3) Point of Contact Name, Phone Number and E-mail Address (4) SAM UEI and NAICS code (5) Business Size SMALL or LARGE (6) Type of Business: service disabled veteran owned, veteran owned small business, 8a, HUBZone, woman-owned, etc. (7) Information for the product you would propose and which if any of the salient characteristics below it does not meet. (8) Where the proposed product is manufactured. (9) A concern that is not a manufacturer of the supply shall provide the manufacturers name and size standard and an authorized distributor letter. (10) If the concern is the manufacture and the sole distributor, please provide documentation to that effect. Kettles VA Medical Center 2215 Fuller Rd Ann Arbor, MI 48105 See Statement of Work Below: ANN ARBOR VAMC INPATIENT MENTAL HEALTH FUNISHINGS AND RELATED SERVICES: DESCRIPTION/ SPECIFICATIONS/ WORK STATEMENT SCOPE OF WORK: The intent of this procurement is to order, design, fabricate, and coordinate delivery/installation of inpatient behavioral health furniture. The space is located at 2215 Fuller Rd., Ann Arbor, MI 48105, on the 1st floor. This order is for various furnishings as listed below. Receiving and Installation will be required for proper fit and finish. The LTC Charles S. Kettles VA Medical Center is ready for the delivery and install of this product as soon as 10/15/23. Delivery and install to be coordinated with the VA Interior Designer. Open Market Services: The vendor shall provide all installation and related services, including order, receipt, any necessary off-site storage and delivery services directly to the building, consisting of a timed order of requested furniture/furnishing items coordinated with anticipated VA substantial occupancy dates, all special and/or proprietary installation parts/pieces required for install of the order and typical operation of items, installation as typically provided by manufacturers, special instructions, and written warranties for VA records, receipt of furniture at their facilities, and coordinated furniture deliveries to the Ann Arbor site. Elevators will be available to move product. Install will occur during normal business hours. PRODUCT The following OEM items are distributed by many vendors but are what is strictly approved. Vendor must be able to provide all items listed below. The below pieces encompass patient safety, have been deemed necessary for the requirements on the unit, and are compliant with the CFM TIL guidelines set forth for the inpatient mental health design guide. All of the following are behavioral health furniture companies; behavioral health furniture IS NOT on the Strategic Acquisition Center s furniture IDIQ contract. This requirement will cover 4 phases as listed below including installation and removal of old products. Manufacturing should be delayed per phase to meet dates listed below so there will be no storage fees incurred. Removal of existing furnishings and disposal. Contractor is responsible for all trash removal. This will occur in 4 phases. All furniture in patient rooms must be bolted to the floor, LVT over concrete. All furniture in the common areas must be weighted. Phase 1: install furniture and remove existing from next phase 10/23/23-11/23/23 Install 3 patient rooms, dayroom, visitation, lounge, dining room These furnishings are required to be bolted to the floor, LVT or carpet over concrete. Remove furnishings from Group Room, Day Room, (4) Offices, Resident Room, Exam Room, Interview Room. Group Room, Storage Room Many of these items are bolted to the floor. Phase 2: install furniture and remove existing from next phase 12/18/23-1/3/24 Install Group room, interview rooms, exam room, seclusion day room, 3 patient rooms These furnishings are required to be bolted to the floor, LVT or carpet over concrete. Remove furnishings from lounge, laundry, (5) patient rooms. Many of these items are bolted to the floor. Phase 3: install furniture and remove existing from next phase 2/16/24-3/1/24 Install (5) patient rooms, lounge areas These furnishings are required to be bolted to the floor, LVT or carpet over concrete. Remove furnishings (6) patient rooms, lounge Many of these items are bolted to the floor. Phase 4: Install furniture 4/16/24 Install (6) patient rooms, lounge areas These furnishings are required to be bolted to the floor, LVT or carpet over concrete. See attached phasing plan. CONFORMANCE STANDARDS: All services provided under this contract must be performed in conformance with Federal regulations and VA Ann Arbor Healthcare System policies and requirements. PREVENTIVE MAINTENANCE: The Contractor will provide all warranty work during active warranty period. All preventive maintenance will be done by trained staff or an appropriate designated party after the warranty expires. Applicable warranties will be provided for all products at time of full delivery. PARTS: Work under warranty and/or provision of any missing and/or damaged items found during delivery review must be conducted within a reasonable amount of time depending on the availability of supplies and material for all furniture and accessories. REPORTING REQUIREMENTS: Approximate delivery date and time is to be pre-arranged, coordinated with and approved by VA Interior Designer. Vendor shall be required to report to the Ann Arbor Interior Designer on arrival each day. Normal working hours are considered Monday-Friday 8:00am to 4:30pm, but hours may be flexible to facilitate efficient install if coordinated with VA Interior Designer. Check in and departure notice are mandatory. Vendor will be responsible for conduct of installers, avoiding as much as possible noise and interference in consideration for ongoing patient care or staff working in the same general area as installations are occurring. Any other concerns regarding the install within this occupied campus will be brought to vendor s attention prior to delivery. Parking (for delivery vehicles) may occur at the site; there are also limited parking spaces at the south end of the campus for contractors. Vendor must immediately alert the VA Interior Designer of any potential delays that may impact each activation date, this order will also require vendor consideration during outside activities. Vendor will be responsible for site coordination so that it doesn t interfere with adjacent pedestrian and/or vehicular traffic during delivery. Only installers and authorized VA staff will be permitted in actual activation areas. The vendor and a VA representative shall document items delivered. This report will serve as receipt of all items delivered; any missing, incorrect items or damaged boxes will be noted. This must occur in addition to an install punch list. Unpacking/staging and removal of all packaging materials and other refuse is a VA requirement of this Contractor. VA representative (Interior Designer) shall be notified immediately of any missing/incorrect and/or previously unseen damage as documented during installer staging and/or install. The Vendor shall log out as agreed, coordinating with the Ann Arbor VA Medical Center Interior Designer, and submit the field service report to the Contracting Officer Representative for a signature, during normal working hours. Work outside of normal working hours, if needed, must be pre-arranged with the VA Interior Designer. If the Contracting Officer Technical Representative is unavailable for this sign out, a signed authorized copy of the field service report shall be sent to the Contractor after the delivery has been reviewed and receipt of all items confirmed. A walk-thru punch list will be done at completion of each installation, with a final status report of the completed install noting any remaining items still to be installed and/or replaced and the solution agreed upon mutually. A final status report will confirm completion of this sequentially installed order.
