Page 4 of 4 This is a SOURCES SOUGHT NOTICE in support of the Rocky Mountain Regional VA Medical Center, 1700 N. Wheeling Street, Aurora, CO 80045, Arapahoe County, for market research purposes only to determine the availability of potential businesses with capabilities to provide the services described below. Potential offerors are invited to provide feedback via e-mail to john.anderson-cooley@va.gov. Responses will be used to determine the appropriate acquisition strategy for a potential future acquisition. All responses due by August 24, at 12:00PM MT. Potential contractors shall provide, at a minimum, the following information: 1) Company name, address, and point of contact, phone number, e-mail address, and DUNS. 2) Is your firm eligible for participation in one of the following small business programs? The Capabilities Statement for this sources sought is not expected to be a Request for Quotations, Request for Proposals or Invitation for Bids, nor does it restrict the Government to an ultimate acquisition approach, but rather the Government is requesting a short statement regarding the company's ability to provide the services outlined in the draft SOW below. Any commercial brochures or currently existing marketing material may also be submitted with the capabilities statement. Submission of capabilities statement will assist our office in tailoring the requirement to be consistent with industry standards. The capabilities will be evaluated solely for the purpose of determining to Set-Aside for the Small Business (SB) Community or to conduct as an Unrestricted Procurement. Other than small businesses may respond to this notice in the event the market does not indicate SB interest. This synopsis is for information and planning purposes only and is not to be construed as a commitment by the Government. The Government will not pay for information solicited. Respondents will not be notified of the results of the evaluation. Note: Do not include Proprietary, classified, confidential, or sensitive information in responses. WHAT WE ARE LOOKING FOR Recalibration, repair, replacement of parts and furnishing parts for a Phototherapy booth --- UVB Meter Recalibration PHOTOTHERAPY UNITS: ULTRAVIOLET 554 MX1754915, SN: 39987 Quantity: 1 UVB Meter Recalibration PHOTOTHERAPY UNITS: ULTRAVIOLET 554 MX1754912, SN: 39986 Quantity: 1 UVA/UVB Sensor Recalibration PHOTOTHERAPY UNITS: ULTRAVIOLET 554 MX1754902, SN: 010540 Quantity: 1 Houva IV UVA Replacement Bulbs Quantity: 24 Houva IV UVB Replacement Bulbs Quantity: 24 Preventative Maintenance Services include thorough cleaning & sanitizing inside & outside of the Phototherapy unit. Interior reflectors and lamps are removed, cleaned, sanitized, and replaced (or replaced with new lamps if ordered). The floor of the booth is cleaned of any debris that may inhibit fan operation. Unit is tested before and after cleaning for proper operation PARTS The contractor shall furnish all spare and replacement parts needed to service the equipment. The price of all parts used to perform all scheduled preventive and unscheduled maintenance shall be included in the coverage. The Government will not pay for any parts provided (during either scheduled or unscheduled maintenance), by the contractor during the period of the equipment being covered. Only new standard parts (manufactured by the maker of the equipment, or equal) shall be furnished to maintain the equipment. The contractor will not be responsible for any cabling within the wall. The government understands there are long lead times on the following spare parts: network video recorders, VMS workstations, veridt biometric readers, and 3 PIV card readers. SERVICE MANUALS/TOOLS/EQUIPMENT The contractor shall provide tools, (test) equipment, service manuals or service diagnostic software. The contractor shall obtain, have on file, and make available to its FSE's all operational and technical documentation (such as; operational and service manuals, schematics, and parts list) necessary to meet the performance requirements and repair of the medical equipment. A reference list of applicable service manuals and their respective locations shall be provided to the Biomedical Engineer upon request. SCHEDULED MAINTENANCE All service/repairs shall be performed during normal hours of coverage unless requested or approved by the Biomedical Engineer. Work performed outside the normal hours of coverage, at the request of FSE, shall be considered service during normal hours of coverage and shall be exempt from an extra labor hour charge. UNSCHEDULED MAINTENANCE The contractor shall provide repair service which may consist of calibration, cleaning, oiling, adjusting, replacing parts, and maintaining the equipment, to include all intervening service calls performed between scheduled services and calibrations. All required parts shall be furnished by the contractor to the VA at no cost. The contractor shall repair the equipment to functional level equivalent to the original manufacturer s specifications. The contractor's FSE shall respond on site within two (2) hours after receipt of request for service. The contractor shall provide parts, if required to effect repair, within 24 hours of diagnosing the problem.
