Amendment 0001 - The date of the site visit is changed from 29 Auust 2023 to 28 August 2023 @ 10:00 Pacific Time.�� STATEMENT OF WORK Program Office:�� USCIS/MD/OI/DMD/FCB Project Location:��� 5160 Richton Street, Montclair, CA Completion Date:��� Must be completed no later than 180 days after contract award. Work�Hours:���� 7:00am - 6:00pm Monday thru Friday. OVERVIEW: The U.S. Citizenship and Immigration Services (USCIS) needs to increase its shelving capacity to store PPE, secure/non- secure items and printing supplies at its Western Forms Center (WFC) in Montclair, CA. The project is to increase additional rack storage capacity to remove pallets from aisles. Currently, the storage racks cannot be secured to the floor resulting in safety hazards and repeated safety violations as determined by the Safety Officer. The Orbiter Pallet Shuttle System or equal system in Bay E. The service includes providing an SSI Orbiter rack or equal, two (2) SSI Orbiter shuttle or equal, remote controls, an extra set of batteries, safety fence, impact protection, charging cabinet & mobile charger, freight to site, and installation/commissioning supervision. The installation work performed by the contractor shall comply with all applicable international building codes and other local codes and ordinances required at WFC. Demolition Services: It is the contractor's responsibility to carefully dismantle the existing shelving system and prepare the space for the installation of the new shelving system. The existing shelving will be stored in a location (to be determined) within the warehouse and reutilized in another area of the warehouse. NOTE: The contractor shall not be required to re-install this shelving. The Government will ensure that all the materials are removed from the existing shelving prior to the system is dismantled. DESIGN SERVICES: A representative authorized by the successful vendor must be available in the following metropolitan area to provide design services, to generate a bill of materials and to conduct routine coordination: USCIS WFC facility in Montclair, CA. Electrical Construction: All required electrical work will be done via a separate General Services Administration (GSA) contract. Fire Suppression Work: All required fire suppression work will be done via a separate General Services Administration (GSA) contract. Construction Performance Requirements: General Contractors performing construction and alteration work in buildings, on behalf of the USCIS, shall adhere to the following general provisions of performance while performing services in these buildings. These conditions are in effect regardless of the contracting organization whether it is the GSA or USCIS. DELIVERY:�������������������� On each individual project, pricing shall include shipping FOB (Free On Board) destination, inside delivery and removal of packaging material and unused product for both the initial installation and any reinstallation. The vendor will be provided a staging area within the building as well as access to a freight elevator. Work shall occur during normal business hours (7:00 am to 6:00 pm Monday thru Friday. Contractors shall be required to submit Facilities Access Request (FAR) Forms at least ten (10) business days prior to delivery and installation. FAR Forms will be provided by the Contracting Officer (CO) & the designated Project Manager (PM). No USCIS employees will assist with unloading the truck(s). The Contractor shall provide all personnel and equipment needed for unloading, moving, assembling, and installation of the shelving system. REQUIREMENTS: The Contractor shall participate in a pre-installation meeting at the installation site. This meeting will include the Government�s contact information, the building�s owner (or representative), the USCIS CO, the USCIS PM and others. This is to ensure a clear and orderly installation coordinated with other Contractors working in the building. The contractor shall appoint in writing a representative for all communications. This appointment will be provided to the CO & PM at the pre-installation meeting. GENERAL PROVISIONS FOR GENERAL AND SUBCONTRACTOR PERFORMANCE: CIRCULATION: All corridor and hallways shall be kept clean and free of debris during contract performance. Corridors and hallways shall be vacuumed and wet mopped at the end of each work shift. CREW SUPERVISION: The Contractor shall provide on-site supervision at all times during contract performance. All debris shall be removed and disposed of offsite. DAILY VACUUMING: All office areas where work is taking place, as well areas adjacent to the work area shall have the carpet and any furniture that is soiled due to the construction work vacuumed at the end of each workday. DUST BARRIER: A dust barrier shall be required to be provided and installed to protect personnel near the work area from any dust during the construction work execution. This dust barrier shall be installed as follows: Preparation: The Contractor shall provide a plastic sheeting barrier to prevent dirt and dust generated from the work area from entering into the adjacent occupied space. Isolate the work area for the duration of the project by completely sealing off all openings and fixtures in the work area, including but not limited to, heating and ventilation outlets (ceiling mounted diffusers) ductwork penetrations, interior doorways, corridor doors used for entrance and exits, windows, and ceiling. Plastic sheeting must be of a 6-mil thickness or greater to adequately maintain the purpose of its use. This barrier must be inspected by the Contractor daily at the beginning of each work- shift. If any penetrations have occurred and any other sealing integrity has been lost, it must be repaired or replaced prior to any work activities taking place that day. Build double barriers of plastic sheeting at all entrances and exits to the work area so that the work area is closed off by one barrier when workers enter or exit. Cover all floor and wall surfaces in the area with two (2) layers of plastic sheeting taped securely in place to protect from damage. As part of this project, some of the existing shelving will be damaged and will become salvage which is the responsibility of the contractor to dispose of off-site. The shelving that is not damaged will be relocated to a Government storage area that will be identified in the initial site visit. For those shelving materials that are to be relocated, the Contractor is to clean all removable items and equipment by wiping them down with clean dust catching/treated fabrics then relocate the items from the work area and store as directed. Cover all non-removable items and equipment with plastic sheeting. Provide negative pressure ventilation to control dust and prohibit it from entering the work area. Use of an air filtration system or machine shall be required. This device shall have a HEPA filter and it must direct its discharge through a water baffle apparatus. If necessary, a solid barrier of metal studs and drywall will be needed. The barrier will be subject to inspection by the USCIS Safety Officer for compliance to the appropriate standards for this application. GOVERNMENT PROPERTY: All Government property, or systems displaced, altered, or damaged during contract performance shall be restored to its original condition at no cost to the Government. These repairs, if needed, must be executed immediately when notified by the Contracting Officer that such repairs are required due to actions by the contractor or his/her subcontractors. POWER/COMMUNICATION INTERUPTIONS: All unscheduled power and communication outages caused by the Contractor as a result of demolition or construction activities shall be restored within four (4) hours. The Contractor is to take all necessary precautions to minimize the possibility of causing such a disruption from occurring by employing a thorough pre-construction survey. Failure to provide corrective actions from causing an outage as described above will result in the Government executing the repair and charging the Contractor for the full cost of the repair performed by others as well as the administrative cost associated with executing the repair. CIRCUIT-PANEL INDEX: N/A FIELD VERIFICATION: All demolition and new construction work floor plan measurements are the responsibility of the Contractor to verify. Floor plans may not be drawn to a specific scale. JOB START MEETING: A pre-construction meeting with the successful offeror shall take place within three (3) days after the award of the contract. CONSTRUCTION DEBRIS: Contractor is not allowed to use any Government trash dumpster for the disposal of construction debris. FIELD ALTERATIONS: For instances where the Contractor is unable to match a color or material shortage, the Contractor shall notify the Contracting Officer. PARKING: USCIS will not allow parking within the USCIS facilities during the performance of this contract. CREW DISCLOSURE: The Contractor Foreman shall possess GSA issued HSPD- 12 badge prior to commencing contract performance. All other contractor/subcontractor personnel require a background check to be conducted at least two (2) weeks prior to installation. The Contractor shall provide the names, social security numbers, and date of birth for each employee and each subcontractor employee scheduled to work on this project at least two (2) weeks prior to actually working at any USCIS occupied building. AUDIO: No radio playing or other noise producing devices are allowed during construction work without permission from the USCIS PM. BADGE DISPLAY: USCIS issued building badges shall be worn above the waist and below the neck and shall be visible at all times while in an USCIS secure building. CONDUCT: No alcoholic beverages or chemical substances, abusive or profane language, or other disruptive or illegal activities will be tolerated.
