Description
MODIFICATION ONE: The question and answer period is being extended until September 8, 2023. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, and a separate written solicitation will not be issued. This solicitation number is RFQ-NICHD-23-265 and is issued as a Request for Quotation (RFQ). The solicitation /contract will include all applicable provisions and clauses in effect through Federal Acquisition Circular 2023-04. The North American Industry Classification (NAICS) Code is 562211 with a size standard of $47.0M. This acquisition is being conducted using Simplified Acquisition Procedures in accordance with FAR Part 13. The National Institutes of Health, The Eunice Kennedy Shriver National Institute of Child Health and Human Development (NICHD), in support of the work of the National Institute of Diabetes, Digestive, and Kidney-Disease (NIDDK) has a requirement to acquire services for Detailed Decommissioning. The Phoenix Epidemiology and Clinical Research branch has had a Nuclear Regulatory Commission (NRC) License to use radioisotopes in basic, animal, and human clinical research since 1971 to the present. Materials were use in laboratory and related spaces in multiple locations on the Phoenix Indian Medical Center (PIMC) campus located at 4212 North 16th street in Phoenix, 85016. A second site at 445 North 5th Street Phoenix, AZ 85004 was added to the license but never used. Over the course of the license the following isotopes have been in use: 3-Hydrogen, 14-Carbon, 32-Phosphorus, 33-Phosphorus, 35-Sulfur, 45- Calcium, 51-Chromium, 63-Nickel, 86-Rubidium, 125-Iodine, and 131- Iodine. The use of these materials declined in the late 1990s/early 2000s and ceased completely in 2011. In addition, sealed sources used as reference standards and in check sources have been stored at the Government�s facility. There have been no records of leaks. The Government is seeking decommissioning of the NRC License and are requesting a decommissioning survey to submit to NRC. The Government is also requesting removal of all sealed sources. OBJECTIVE The objective of this requirement is: 1. To complete decommissioning survey for submission to NRC 2. To dispose of all sealed sources 3. To assure NRC approves decommissioning survey report Scope The goal is to decommission our NRC license. To accomplish this, the Contractor shall 1) remove sealed sources from the Government�s unit and 2) provide a comprehensive decommissioning survey to be performed to document that there is no evidence of residual isotopes. This will need to be submitted to the NRC for decommissioning. Completion of the project will be dependent on NRC acceptance of this final decommissioning survey. Task Area 1 � Delivery Timelines 1. The majority of sealed sources (n = ~70 total) will be removed within 60 days of purchase order award. A small number of sealed sources will be kept on site for use during decommissioning survey. 2. Decommissioning survey will be conducted within 60 days of purchase order award. 3. Final decommissioning report will be available for submission to NRC within 90 days of purchase order award 4. Upon approval from NRC that decommissioning survey is approved, remainder of sealed sources will be removed. PERIOD OF PERFORMANCE The period of performance for this requirement is: TBD Evaluations will be conducted in accordance with the provision at FAR Clause 52.212-2. Offerors will be evaluated as follows: EVALUATION-COMMERCIAL ITEMS (OCT 2014) (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: � Technical Approach � Organizational Capability and Past Performance � Personnel All evaluation factors other than cost or price, when combined are significantly more important than cost or price. (b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). (c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer�s specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. The offeror must include a completed copy of the following provisions: 1) FAR Clause 52.212-1 Instructions to Offerors � Commercial items; 2) FAR Clause 52.212-2, Evaluation � Commercial Items. As stated in FAR Clause 52.212-2 (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be advantageous to the Government, price and other factors considered. The following factors will be used equally to evaluate offers: Technical Evaluation, Price, and Past Performance. The Government will make award based on Best Value. Note: Past Performance Information: Vendors must submit a listing of the most recent contracts/awards (minimum of 3) which demonstrate similar work in nature to this Solicitation. Contracts/awards may include those entered with the Federal Government, state and local governments and commercial concerns. Include the following information for each contract or subcontract: 1. Description of Requirement 6. Contracting Officer�s Name and Telephone Number 7. Program Manager�s Name and Telephone Number 3) FAR Clause 52.212-3, Offeror Representations and Certifications � Commercial Items; 4) FAR Clause 52.212-4, Contract Terms and Conditions � Commercial Items; 5) FAR Clause 52-212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders � Commercial Items � Deviation for Simplified Acquisitions. The DUNS Number is no longer valid for federal award identification, the Taxpayer Identification Number (TIN), and the certification of business size. All offerors must have an active registration in the System for Award Management (SAM) www.sam.gov . The clauses are available in full text at http://www.arnet.gov/far. NOTE TO CONTRACTOR: The contractor shall provide a completed copy of FAR 52.204-24, ""Representation Regarding Certain Telecommunications and Video Surveillance Services or Equipment"" with their quote in response to this solicitation. Responses to this notice shall contain sufficient information to establish the interested parties� bona-fide capabilities for fulfilling the requirement, unit price, list price, shipping and handling costs, the delivery period after contract award, the prompt payment discount terms, the F.O.B. Point (Destination or Origin), the Vendor/Contractor�s (UEI) the Unique Entity ID from SAM.gov is now the authoritative identifier for those doing business with the federal government. All offerors must have an active registration in the System for Award Management (SAM) www.sam.gov . The contractor will be evaluated on their capability to provide the service to the specifications as stated on the solicitation.
