Page 4 of 4 The Bath VA Medical Center located at 76 Veterans Avenue, Bath, NY 14810 has a requirement for a Herman Miller Furnishings for the Bath VA Pharmacy. This is a Combined Synopsis Solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 and 13.5, as supplemented with additional information included in this solicitation. This announcement constitutes the only solicitation. Quotes are being requested and a written solicitation will not be issued. Interested vendors that respond to this Combined Synopsis Solicitation must provide evidence that they are able to meet all the requirements identified herein. This requirement is being solicited as an SDVOSB Set Aside for Brand Name items. The referenced Brand Name is Herman Miller. The proposed contract action is for supplies for which the Government intends to solicit competitive offers to award a firm fixed price contract. Interested vendors may submit quotes, and all eligible quotes received prior to the date listed in SAM.gov shall be considered by the Government. The NAICS code for this requirement is 337127 and the size standard is 500 Employees. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2023-03. VA Finger Lakes Healthcare System, Bath Facility 76 Veterans Avenue Bath, NY 14810 B76 Pharmacy Rework POC/COR: Julie Monagan Barnard, Interior Designer STATEMENT OF WORK 1.0 GENERAL SCOPE The Veterans Health Administration (VHA) has a requirement for healthcare furniture for VA Finger Lakes Healthcare System, Bath Facility, 76 Veterans Ave, Bath, NY that meet the VHA high-quality threshold while integrating excellent environmental stewardship. This requirement includes healthcare furniture, design, project management, furniture removal, professional installation and maintenance/warranty services. Service required is detailed in SV1. Healthcare Furniture technical requirements per VHA Healthcare Furniture multiple award IDIQ. The Contracting Officer Representative (COR) is Julie Monagan Barnard, Bath VA MC Interior Designer. Healthcare furnishings include, modular systems, work surfaces, shelving, electrical components and related accessories and related phased removal and installation services for existing and operational outpatient pharmacy service. The contractor shall provide all labor, materials, equipment, transportation, and supervision necessary to satisfy the needs of each ordering activity. The following equipment is required for the Bath VA Pharmacy: QTYs: E0660_13, Shelf, 9 double sided: Qty 2 E0660_13, Shelf, 12 double sided: Qty 6 E0660_16, Shelf, 9 single sided: Qty 1 E0660_16, Shelf, 12 single sided: Qty 3 E0660_13, Shelf, 12 single sided: Qty 3 U0002, Workstation, 12 : Qty 2 U0002, Workstation, 9 : Qty 4 2.0 PERFORMANCE REQUIREMENTS The contractor shall provide and be responsible for the technical assistance, development and generation for final specifications. Duty hours will be 8-4:30 M-F. The Government recognized US holidays are: New Year s Day, Martin Luther King, Jr. Day, Presidents Day, Memorial Day, Juneteenth Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day and Christmas Day. If the holiday falls on Sunday, it is observed on Monday. The contractor shall: Coordinate and provide project management of all products and services to manage, design, order, ship, deliver and install new furniture from the manufacturer until final acceptance by COR/POC Interior Designer Julie Monagan Barnard. Contractor is required to have on site staff supervisor with OSHA 10 certification. Contractor is required to remove existing furniture per phase schedule just prior to the new furniture installation. Contractor is required to completely install all systems/modular furniture and associated components and accessories. Project phasing and scheduling requirements to include planning, manufacturing, project management and installation. Only include products specified within the RFQ. Prepare and provide as built furniture drawings in PDF and AutoCAD. Provide maintenance manuals for all products specified. 3.0 KICKOFF MEETING The prime contractor(s) shall participate in a kickoff meeting within ten (10) days of task order issuance, in person at 76 Veteran Ave. Bath NY 14810, Building 76. Time and date will be determined by VA Point of Contact. 4.0 SUBMISSION OF QUESTIONS Questions: All questions submitted for this solicitation must be electronically sent to the following email address: sam.demuzio@va.gov. Questions received after the stated date and time, may be addressed at the discretion of the Contracting Officer. 5.0 SUBMISSION OF QUOTATION Quote Submission: The contractors shall submit complete emailed electronic copy portable document format (.pdf) or Microsoft Excel (.xls) format of: Volume 1: (technical) drawings, (floorplans, isometrics, shop drawings, typicals etc.) complete manufacturer s product specifications Bill of Materials tagged per CLIN/ floorplan department and room etc. quantity of man hours for labor/installation project management plan product literature project staffing plan physical finish samples sustainability certification documents complete testing results / certificates mock up samples (list any needed here) warranty. Self-certifying statement confirming ability to meet project deadlines Volume 2: (pricing) price quote with separate line for labor/installation Bill of Materials with subtotals and tagged per CLIN/ floorplan Submissions can be sent in multiple emails to avoid computer system email size limitation, to Sam DeMuzio at sam.demuzio@va.gov. Quotes submitted by any other method will not be considered. All proprietary information shall be clearly marked. The use of hyperlinks in quotes is prohibited. Late quotes will not be accepted for evaluation. The contractor is responsible for confirming the Government s receipt of the contractor s quote. Standard Installation Warranty Required: All products are required to be free from defects in workmanship and materials, under normal use and conditions, for a minimum of one year from the date of installation unless otherwise stated. During the warranty period, any defect in material or workmanship that requires repair or exchange of a product of comparable nature will occur by seller without charge incurred by the VA. Such repair or replacement is subject to verification and approval by the VA due to Medical Center governing policies. 6.0 EVALUATION CRITERIA Task order will be evaluated on: Bill of Materials with subtotals and tagged per CLIN / floorplan department and room etc. PDFs of detailed and dimensioned: Plan, 2-D and 3-D drawings of each workstation (and/or) products with multiple parts list to represent exactly what is being included in task order and to determine accuracy. Cut sheets with product detail for each line item. Labor/Services appropriate to the scope of work. (no pricing included) Project staffing plan appropriate to scope of work Project management plan appropriate to scope of work Sustainability Certification 7.0 PRODUCT General All finished surfaces shall be free of scratches, mars, dents, or blemishes, and withstand staining, and exhibit no flaking, cracking, or loss of adhesion. Furnishings shall have smooth finishes with no hazardous projections, sharp corners, or detail, which can be hazardous and cause personal injury or damage to clothing. 8.0 SERVICES Service required: Contractor is required to provide basic design services and layout for commodities purchased. All designs of furniture layouts, including original designs and any subsequent redesigns, must be accomplished by a qualified professional Interior Designer with either a four (4) year Interior Design degree from an accredited school by the Council for Interior Design Accreditation (CIDA/FIDER), or 5 years of experience, and one year experience using contractor s software similar to that proposed for this contract. Design Contract Tasks/Requirements The contractor shall attend a minimum of three (3) in-person meetings at the Bath VA MC Facility to review final award with VA Interior Designer and End Users and to make necessary revisions. During first meeting the contractor shall provide the Interior Designer with the Auto-Cad drawings to [1/4 1/8 ] scale drawings showing layouts of awarded product. The contractor shall allow for three (3) revisions per line item included in design services including updating Auto-Cad Drawings and PDFs as request by VA Interior Designer. The contractor shall be responsible for taking and applying accurate field measurements to ordered product for verification of correct sizing. Auto-Cad drawings may not be accurate finished dimensions and may not be used in place of field measurements. The Vendor shall be liable for any incorrect field measurements leading to incorrect product order. The contractor shall be responsible for providing all sample materials for awarded product (2-Day Delivery) as requested by VA Interior Designer. The product must not be placed into production without clearance from the VA COR. The contractor shall provide final, clean [1/4 1/8 ] scaled drawings of product layout for sign-off prior to scheduling manufacturing. The contractor shall track manufacturing schedule and notify VA Interior Designer with updates of estimated completion date by email. The contractor shall attend a post-installation meeting to assess, address, and document any punch-list items and shall submit to the VA Interior Designer a remediation plan within 5 business days. The contractor shall provide free storage until site and scheduled installation is confirmed and completed. The Government maintains the option to modify the normal work week, days and hours, as necessary for the accomplishment of VHA mission. Work Required by Others All work required by other trades shall be identified under this section by the Bidder. 52.212-2 EVALUATION COMMERCIAL ITEMS (October 2014) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price, and other factors considered. The following factors shall be used to evaluate quotations: (1) Technical or Quality (2) Past Performance (3) Price (a) Evaluation Approach. The Government will evaluate quotations using the comparative evaluation process outlined in FAR 13.106-2 (b) (3), where quotations will be compared to one another to determine which provides the best benefit to the Government. The Government reserves the right to consider a quotation other than the lowest price that provides additional benefit(s). Quotations may exceed minimum requirements of the solicitation. The Government reserves the right to select a quotation that provides benefit to the Government that exceeds the minimum requirements of the solicitation but is not required to do so. Each response must meet the minimum requirements of the solicitation. The Government is not requesting or accepting alternate quotations. The evaluation will consider the following: (1) Technical or Quality: The quotation will be evaluated to the extent to which it can meet and/or exceed the Government s requirements as outlined in the solicitation and based on the information requested in the instructions to quoters section of the solicitation. (2) Past Performance: The past performance evaluation will assess the relative risks associated with a quoter s likelihood of success in fulfilling the solicitation s requirements as indicated by the quoter s record of past performance. The past performance evaluation may be based on the contracting officer s knowledge of and previous experience with the supply or service being acquired; Customer surveys, and past performance questionnaire replies; Contractor Performance Assessment Reporting System (CPARS) at http://www.cpars.gov/; or any other reasonable basis.
